TENDER SUMMARY

Tender Title: TENDER FOR THE “RAKAN UNIFI SARAWAK” FOR UNIFI SERVICE INSTALLATION AND RESTORATION FOR TELEKOM MALAYSIA BERHAD (TM)

Tender Ref: CSS/2018/T/02/RAKAN UNIFI SARAWAK

Closing Date: 19 February 2018

1. The tenderer shall be required to complete end to end Installation and Restoration inclusive of External,

Internal and Activation section of the HSBB Network in a Single Truck Role (STR) Mode based on the following services. a) Standard Unifi Installation

(i) Unifi Over Head Network (OH) (ii) Unifi on Under Ground Network (UG) (iii) Unifi on VDSL Network (iv) High-rise Fiberisation

b) Non – standard unifi

(i) Over (inside) the ceiling (ii) On pole to UG (iii) > 50m internal cabling

c) Unifi restoration

(i) Assurance per TT basis (ii) Fixed assurance team

2. For HSBA Service, contractors shall be required to complete external and internal installation of the network

up to BTU only while other Broadband Service Operator shall execute the service activation activities themselves.

3. The tenderer shall be required to execute all types of service order not limited to new installation, modify, relocate and modify intermal premise.

4. The tenderer at anytime required by TM shall cover the Installation and Restoration of pre unifi services under contract cross scoping.

5. The tenderer's scope of responsibility shall include the following: a) Fiber Distribution Point (DP) b) Drop fiber c) Internal wiring d) Fiber wall socket (FWS) e) BTU f) Residential Gateway (RG) g) Set Top Box (STB) h) Dect Phone ( (if provided by TM)

6. Tenderer is required to fully comply with the clauses marked as “Critical (CRQ) Clause” in the Technical Specifications failing which, the Tenderer shall be disqualified from the tender exercise and their tender submission shall be rejected. The list of CRQ clauses are as below:

ORGANIZATIONAL STRUCTURE AND MANPOWER REQUIREMENT

Upon tender submission, the tenderer shall submit details of minimum number of teams as below.

Each of team shall be:

a. Equipped with tool & test gear with priority marked Critical as per Appendix B3.

b. Equipped with OSH equipment with priority marked Critical as per Appendix B4.

c. Equipped with vehicles with priority marked Critical as per Appendix B5 For the team identification and item (a) to (c), the tenderer shall complete the form as per appendix B8 (Team Information Form). Sharing of teams amongst the tenderers, false evidences or incomplete forms shall lead to team disqualification. If the tenderer fails to provide the minimum team as per required, the tenderer shall be disqualified.

The tenderer shall submit the copy of related identification card (IC), resume (if any),academic qualification certificate (if any), competency certification (if any), testgear calibration certificate/purchase receipt /rental agreement (if any), OSH equipment calibration certificate/purchase receipt /rental agreement (if any), road tax and vehicle grant/ rental agreement (if any) for verification during the submission of the tender documents.

CRITICAL

7. The Tenderer shall deposit a sum of RINGGIT MALAYSIA Ten Thousand Only (RM10,000.00) as a

Tender Deposit and to be submitted in the Tender Proposal during Tender Closing Date.

8. The Tenderer shall fully comply with the Rakan unifi Payment Rate as shown in the Part VI of the Invitation to Tender Documents –Rakan unifi Payment Rate.

9. Tenderer shall only operate in Sarawak only. The rates provided shall be applicable for area in Sarawak. 10. Tender Mandatory Requirement

(a) The Tenderer shall be a Bumiputera Company incorporated in Malaysia. A valid copy of Certificate

from Ministry of Finance (MOF) or PKK Bumiputera Certificate or any Registered Body as a proof on Bumiputera Status Company shall be submitted during Tender Closing Date. In progress to apply or invalid certificate shall be disqualified.

(b) The Tenderer shall be a company with Sendirian Berhad or Berhad or Koperasi status incorporated in Malaysia with main office located and registered in Sarawak. A valid copy of Company Registration Certificate issued by Suruhanjaya Syarikat Malaysia (SSM) or Suruhanjaya Koperasi Malaysia (SKM) or any Local Authorities shall be submitted during Tender Closing Date.

(c) The Tenderer shall have a valid Sijil Perolehan Kerja Kerajaan (SPKK) Certificate or valid CIDB certificate under code E07 (Internal) or E08 (External) issued by CIDB or Unit Pendaftaran Kontraktor (UPK) Sarawak Heads V Sub Head 8. A copy of valid certificate shall be submitted during Tender Closing Date. In progress to apply or invalid certificate shall be disqualified.

(d) The Tenderer shall free from TM’s suspension or blacklisting as at Tender Closing Date.

(e) The Tenderer shall free from legal action by or against TM as at Tender Closing Date.

(f) The Tenderer shall free from bankrupt and/or winding up status throughout the tender process and during the validity of the Contract if the Tenderer is successful in the Tender.


(g) The Tenderer shall have a minimum paid up capital of RM250,000.00 during tender submission. This

requirement is to be maintained throughout Contract Period.

(h) Tenderer is advisable to have a minimum working capital of RM1 million.

11. Evaluation Process

(a) The Tenderer shall provide complete team within two (2) months after Letter of Award (LOA) issuance.

Site Inspection will be performed by TM at tenderer’s premise to confirm on the compliance to the

requirement as set out in Technical Specification;

(b) Works shall only commence subject to the completion of item (a) above.
