Why did we want to introduce a Telemedicine Service?

• 23% rise in Care home emergency admissions over two years in AVCCG

• Airedale Vanguard site - reduced emergency admissions by 35% and A&E attendances by 53%

• Opportunity to introduce ‘New ways of working’ to support the Primary Care Sustainability and Transformation agenda

• Supports the sharing of information via SCRAI • Supports the paperless 2020 agenda

• 24/7 service • Video via PC’s • Resilient Infrastructure • Experienced nurses • Real time access to health information • Automated messaging back to GP practices

Project Deliverables

• Developing New Models of care to support care homes and sheltered housing (including extra care housing) via 24 hour clinical support via video link.

• Releasing GP Capacity – introduce GP video triage for care home residents

• More efficient management of winter pressures

• Better access to patient records and care plans through the use of digitalisation.

• First Care Home ‘Go live’ October 2016

• Currently 18 care homes accessing the telecare hub
