If your district has moved to the new physical disability reporting categories then for 2012-2013, if you shade Primary Disability bubble 7: Physical Disability, you must also specify the physical disability by marking one of the “State Use” bubbles:

For Orthopedic Impairment, shade State Use 7

For Other Health Impairment, shade State Use 8

For Physical Disability, shade State Use 9

Only authorized individuals may have access to the test materials.

Test materials must be locked up and be checked out to authorized individuals.

Test materials should be tracked on a Chain of Custody document to ascertain who had what materials at what time (see sample on next slide).

The Chain of Custody document should be reviewed at the end of each testing session to ensure that all test materials have been returned.
