A3.7.2 Enhanced Optional Extended Area ServiceA. General

1. Enhanced Optional Extended Area Service (EOEAS) provides alternative billing for customer dialed station-to-stationcalls to locations outside a subscriber's extended area service local calling area, but within the same LATA. EOEAS isavailable in the following configuration:

a. Premium - This option permits residence subscribers to increase their unlimited local calling area by allowing thesubscriber to make unlimited outgoing direct dialed calls to a specified Extended Calling Service exchange orexchanges within the subscriber's LATA. Premium customers will not be billed Extended Calling Service or LocalCalling Plus rates. Customers pay a fixed monthly EOEAS additive in addition to the Individual Residence Linerate.

2. These services are offered in the exchanges specified in B.2 following subject to the availability of facilities and billingcapabilities. These services are available to business and residence subscribers except as specified herein. Customers whosubscribe to Access Line Service for Payphone Service Provider (PSP) telephones, Sharing and Resale of Basic LocalExchange Service, or lines which terminate in hotels, motels, nursing homes or hospitals to serve guests or patients maynot subscribe to this service. EOEAS is not available to users of Dormitory Communications Service. Business customersand Residence Message Rate customers may not subscribe to the Premium option.

3. EOEAS is intended for the personal use or business use of the subscriber and may not be resold to others or used on aplanned and continuing basis to intentionally avoid the payment in whole or in part of message toll charges by others.

A. Optional Extended Local Calling Between Daytona Beach and New Smyrna Beach

1. General

a. Optional Extended Local Calling (OELC) provides for optional 2-way flat rate service for customer dialedstation-to-station calls between telephones served from the Daytona Beach exchange and those served from the NewSmyrna Beach exchange. OELC customers will not be billed Local Calling Plus or Extended Calling Service rates.See A3.8.50 or A3.7.3 for rates and regulations.

b. A customer who subscribes to OELC may dial 7-digit calls to New Smyrna Beach on a flat rate basis and 7-digitcalls to his number from New Smyrna Beach will be at no charge to the calling party.

c. Credit card calls and calls that require the assistance of an operator for completion are not eligible under the OELCplan. Such calls will be charged for at the appropriate long distance rate except that an operator will reestablish aneligible call that is interrupted due to facilities or poor transmission.

d. OELC is available on all individual line residence, individual line business, residence and business trunks exchangeservice except those specifically excluded in 1.e. following. All lines with originating service capability in theaccount terminating in the same system or the same premises must subscribe to the option.

e. OELC is not available with either Message Rate Service or Access Line Service for PSPs. Resale of this service isprohibited as set forth in A2.2.1 of this Tariff.

f. OELC is not subject to suspension.

g. The minimum service period is one month.

h. OELC will be made effective or disconnected on the service date requested with fractional billing applied ifapplicable.

i. OELC is furnished subject to all applicable regulations in Section A2. of this Tariff except as stated in 1.a. through1.h. preceding.

2. Directory Listings

a. Subscribers to OELC are entitled to one listing in the New Smyrna Beach directory at no charge.

b. The listing will contain sufficient wording to advise New Smyrna Beach callers that the Daytona Beach OELCsubscriber can be called toll free.

c. Subscribers to OELC who are listed in the New Smyrna Beach exchange directory will have their telephone numberchanged with no referral of calls if the OELC service is discontinued for other than a complete disconnect ofexchange service. Once disconnected, the number will not be reassigned to the same subscriber unless OELC isreinstated.

Calls will be referred if the subscriber transfers and continues the service.

B. An incoming call to a subscriber with a non-published listing will be completed by the Company only when the calling partyplaces the call by number. The Company will not connect a call to a non-published subscriber on behalf of another caller whenthe caller does not furnish the non-published number to the Company. The Company will adhere to this practicenotwithstanding any claim of emergency the calling party may present. The acceptance by the Company of the subscriber'srequest to furnish a non-published telephone number does not create any relationship or obligation, direct or indirect, to anyperson other than the subscriber.

C. In the absence of gross negligence or willful misconduct, no liability for damages arising from publishing a non-publishedtelephone number in the directory or disclosing said number to any person shall attach to the Company. Where a non-publishedlisting is published in the directory, the Company's liability shall be limited to and satisfied by a refund of any monthly chargeswhich the Company may have made for such listing, and at the subscriber's request, the Company will change thenon-published telephone number at no charge to the subscriber. The subscriber indemnifies and saves the Company harmlessagainst any and all claims for damages caused or claimed to have been caused, directly or indirectly, by the publication of anon-published listing or the disclosing of said listing information to any person.

D. Telephone numbers transmitted via Caller ID are intended solely for the use of the Caller ID subscriber. Resale of thisinformation is prohibited as described in Section A13. of this Tariff.

E. The telephone number, name and address of the subscriber may be disclosed in connection with E911 service, whether suchservice is provided by the Company or any other person. The subscriber waives any privacy interests in his telephone number,name and address in connection with E911 service.

F. For accounting purposes, the telephone number, name, and address of a subscriber with a non-published listing will beprovided to the Long Distance Carrier(s) which furnishes the subscriber long distance message telecommunications service.

A13.72 Inter-Switch Simplified Message Desk InterfaceA13.72.1 General

A. Inter-Switch Simplified Message Desk Interface (ISMDI) service provides subscribers with the capability of receiving callinformation over a data link interface for use with voice messaging system (VMS) equipment. Only call information routedover the Public Switched Telephone Network using the Common Channel Signaling/Signaling System 7 (CCS/SS7) network isavailable for ISMDI. The ISMDI data link must be associated with a voice level hunt group assigned to the VMS subscriber.Incoming call data available from the CCS/SS7 network includes the number called, the type of incoming call (direct orforwarding due to busy or no answer), the forwarding number, and the calling number. The telephone numbers are provided asten digit Directory Numbers.

B. ISMDI provides the capability for the VMS provider to transmits data messages over the data link to the ISMDI host switch foruse in delivering message waiting notification to VMS client users. These data messages will enable the VMS subscriber toactivate or deactivate message waiting indicators (MWI) on VMS client station lines served by ISMDI compatible centraloffices.

C. The subscriber had the option of specifying an SS7 Point Code to which MWI messages are to be routed.

In addition, the rules, regulations and limitations specified in A13.9.7 andA13.47 apply to Star 98 Access and Message Waiting Indication features requested as part of this package.

C. All features/services are furnished only from central offices that have been arranged to provide these services. Thefeatures/services are provided subject to availability of facilities.

A13.79 211 Dialing ServiceA13.79.1 General

A. 211 Dialing Service (“211”) is a three digit local dialing arrangement available in specified areas, with BellSouthTelecommunications, Inc. (“Company”) for delivery of general information via voice grade facilities, for communityinformation and referral services. Pursuant to Order 00-256, issued by the Federal Communications Commission (FCC) in CCDocket 92-105, the 211 code is assigned for access to community information and referral services. In addition, the 211subscriber must comply with any orders and rules pertaining to 211, adopted by the FCC in rulemaking proceeding CC Docket92-105.

B. 211 is available in BellSouth Territory only. To provide access to a 211 number to end users in an independent companyterritory or to a CLECs end users within the local calling area, the 211 subscriber must make appropriate arrangements with theindependent company or CLEC serving that territory.

C. The Local Calling Area of the 211 subscriber will be the Basic Local Calling Area as defined in A3.3 of this Tariff, as facilitiespermit. If local calling areas are merged, and a 211 number exists in both areas, the 211 subscriber who established the 211Service first in time will be entitled to retain the 211 number in the merged calling area.

2. Customers accessing BellSouth Video Conferencing Service via Broadband Exchange Line Service must utilizeBroadband Exchange Line Extensions when the customer's serving wire center is not a SAP.

3. Customers accessing BellSouth Video Conferencing Service via ISDN switched services or Switched 56 Kbps servicesmust utilize interoffice channels when the customer's serving wire center is not a SAP.

D. Contract Plans

1. Contract Plans are available under conditions specified in the Fast Packet Services Payment Plan in A40.10. of this Tariffwith contract periods as follows:

a. Term Payment Plan A - payment periods may be selected from 12 to 36 months.

b. Term Payment Plan B - payment periods may be selected from 37 to 60 months.

2. The Termination Liability Charge is applicable as stated in A40.10. and is determined by multiplying the number ofmonths remaining in the contract payment period by the contracted monthly rate by sixty percent.

3. Video bit rate changes are permitted with no Termination Liability Charge when:

a. the completed service period is twelve months, or twenty-five percent of the length of the originally selected contractservice period, whichever is greater, and

b. the service period of the new Fast Packet SPP arrangement for the new video bit rate is equal to or longer than theremaining service period of the disconnected video bit rate, and

c. the service orders to install the new video bit rate and disconnect the old video bit rate are related together andreceived by the Company at the same time, and there is no lapse in service between installation of the new video bitrate and the disconnection of the old video bit rate.

New connections of the residence low use measured or residence or businessmeasured options will not be permitted after this date.

b. Residence and business telephone customers will have the option of subscribing to one of these telephone services,with the exception of specific current services that will continue to be available under provisions of this exceptiontariff.

c. Foreign exchange service served from the Green Cove Springs exchange (that is, with a Green Cove Springstelephone number) will no longer be offered to new customers as of the initial effective date of this Tariff. Existingcustomers will be allowed to continue this service at their present location.

Customers located in the Green Cove Springs exchange serving area with foreign exchange service served fromanother exchange (that is, with a telephone number of another exchange) within the Tier 1 local calling scope cannotsubscribe to Measured or to Low Use Measured service for their co-located local service.

Customers located in the Green Cove Springs exchange serving area with foreign exchange service served from anexchange (that is, with a telephone number of another exchange) within Tier 2 or 3 of the Green Cove Springs localcalling scope do not require a co-located local service line from the Green Cove Springs exchange. If the customerdoes subscribe to local Green Cove Springs service as well, this service cannot be Measured or Low Use Measuredservice.

d. For foreign exchange service served from the Green Cove Springs exchange (that is, with a Green Cove Springstelephone number), and message rate service (with the exception of hotel, motel, and hospital service, which areaddressed in i. following), the fixed monthly local exchange rate will be:

Message rate - As listed for Rate Group 2 exchanges in Section A3. Foreign exchange - As listed for thecomparable combination service rate in this pilot.

e. Their local usage charges will be as follows:

(1) Tier 1 Calls:

Unmeasured for foreign exchange served from Green Cove Springs.

Current message rate call allowance and additional message charges apply for message rate service.

(2) Tier 2 and 3 Calls:

Measured at the same tier usage charges applicable to individual line measured rate service. No usageallowance applies.

i. The following services are not offered the option of subscribing to measured local exchange service: calling will beunmeasured to Tier 1 locations and intrastate toll rates will apply to calls to Tier 2 and 3 locations.

j. Extended Calling Service (ECS) supersedes Measured Service for calls to ECS additional exchanges listed in A3.3.1,Local Calling Areas. See A3.7.3 for ECS regulations and rates.

2. Rates and Charges

a. The Green Cove Springs local calling area will be subdivided into three tiers for measured service calling. Tier onewill include the Green Cove Springs1 exchange. Tier 2 will be the Middleburg, Orange Park, and Julington2

exchanges. Tier three will be the Jacksonville and Maxville exchanges.

b. For purposes of this pilot measured service offering, the monthly local exchange service charges for residence orbusiness measured services cannot exceed the rates as indicated in e. following. These rates are based on 125 percentof the rates for Unlimited basic exchange service that was grandfathered April 11, 1998.

c. The low use offering will not be available with rotary, or line hunting, service features.

d. Where a subscriber contracts for two or more individual measured rate lines on the same premises and agrees togrouped billing, the amount of monthly measured rate allowance per line, as specified following, will be multipliedby the number of such measured rate lines and measured usage in excess of this product will be billed at themeasured charges as indicated following. PBX trunks serving the same PBX and group billed will also have theallowance multiplied by the number of trunks so billed in determining the measured charges. All lines or trunksincluded for such allowances must have the same central office designation and be of the same class of service.

Note 1: Penney Farms is combined with the Green Cove Springs exchange effective October 24, 1988.

Note 2: The Mandarin exchange area will be subdivided; a portion becoming part of the Jacksonvilleexchange area, the remainder is renamed the Julington exchange, effective June 27, 1987.

a. The Jupiter local calling area will be subdivided into three tiers for measured service calling. Tier one will includethe Jupiter, Hobe Sound exchanges and the north zone of the West Palm Beach exchange (Riviera and Gardens wirecenters). Tier two will be the central zone of the West Palm Beach exchange (Haverhill, Royal Palm Beach, andW.P.B. - Main wire centers). Tier three will be the south zone of the West Palm Beach exchange (Greenacres andLake Worth wire centers).

b. For purposes of this measured service offering, the maximum measured service monthly billing will not exceed 125percent of the flat rate charges for the comparable service in effect on January 1, 2001.

c. The low use offering will not be available with rotary, or line hunting, service features.

A103.5 Message Rate ServiceA103.5.1 General

(Obsoleted 07-21-2001, Type 3) Message rate service will be obsoleted. A. Business and residence individual line message rate service is offered in all exchanges except where noted in the LocalExceptionsA103.2of this Tariff. Residence individual line message rate service is offered in all exchanges where facilitiespermit. The rates specified entitle subscribers to the number of messages specified to all exchange access lines bearing thedesignation of central offices of the serving exchange and extended area service additional exchanges as shown in A3.3.1 ofthis Tariff.

B. Subscribers to message rate service are regularly billed monthly in advance. Messages in excess of the monthly messageallowance are billed monthly in arrears. Local messages not used in one month's allowance are not credited to the subscriber'saccount for any other month service is rendered.

C. Where a subscriber contracts for two or more individual message rate lines on the same premises and agrees to groupedbilling, the number of monthly message allowances per line, as specified preceding, will be multiplied by the number of suchmessage rate lines and messages in excess of this product will be billed at the additional local message charge as indicatedfollowing. All lines included for such allowance must have the same central office designation.

D. Message charges will not apply to calls to the Company Business Office, Repair Service, Directory Assistance, Emergency911 Service or 976 Dial-It Service.

E. Message rate service will not be provided in connection with the provision of Foreign Exchange or Foreign Central Officeservices.

F. Generally, any combination of message rate and flat rate service will not be allowed on the same premises. See A2.3.2 of thisTariff for specific exceptions to this rule.

A103.29 Outgoing Only ServiceA103.29.1 General

(Obsoleted 07-21-2001, Type 3) Outgoing Only Service for Local Measured Service and Message Rate Service will beobsoleted. Customers subscribing to Outgoing Only Service for Local Measured Service and Message Rate Service as of July21, 2001 may retain this same service as long as this Tariff is in effect. New connections Outgoing Only Service for LocalMeasured Service and Message Rate Service will not be allowed beginning July 21, 2001. Residence Outgoing Only Servicefor Local Measured Service and Message Rate Service will be eliminated after February 1, 2002.

A. Outgoing Only Service provides outgoing only access to the network for residence and business customers desiring to have aline that cannot receive calls.

B. Outgoing only lines are capable of accessing all exchange access lines bearing the designation of central offices of the servingexchange and additional exchanges as shown in A3.3.1 for flat or message rate Local Calling Areas, of this Tariff.

C. Residence and business outgoing only lines are offered on a message rate basis at rates equal to the exchange's associatedresidence or business individual line message rate. (An outgoing only message rate line has the same allowance and charge formessages over the allowance as is applicable for an individual line). In addition, where Local Measured Service is currentlyavailable, residence and business outgoing only lines are offered on a measured basis at the same associated measured accessline and usage rates and with the same allowances as the individual (two-way) measured lines. (See A103.2 Local Exceptions)

D. Calls to the telephone number associated with this service will be routed to an appropriate central office recording.

E. The telephone number associated with this service will not be published.

F. This service will be allowed to work as the open end of Foreign Exchange Service, unless otherwise specified.

G. An outgoing only line may be a customer's only Basic Local Exchange Service or may be in addition to other two-way service.However, if it is in addition to other two-way service both must be of the same type (i.e., message rate or measured).

H. Other services involving the receipt of incoming calls (i.e., Rotary service, Remote Call Forwarding, Incoming OptionalCalling Plans, Optional Extended Local Calling (between Daytona Beach and New Smyrna Beach), Call Forwarding Variable,Call Waiting, Call Forwarding Busy Line, Call Forwarding Don't Answer, RingMaster® service, TouchStar® service, etc.)cannot be offered in association with Outgoing Only Service.

I. Neither Prestige® service, Customized Dialing Packages, nor Customized Code Restrictions are available in association withOutgoing Only Service.

J. Outgoing Only Service is not available with the following Enhanced Optional Extended Area Service (EOEAS) options:
