“Classification (e) (- D-FMEA -)This column may be used to highlight high-priority failure modes and their associated causes . As a result of this analysis, the team may use th is information to identify special characteristics. Customer specific requirements may identify special product or process characteristic symbols a nd their usage. A characteristic designated in the design record as special without an associated design failure mode identified in the DFMEA is an i ndication of a weakness in the design process.
(- P-FMEA -)This column may be used to highlight high priority failure modes or causes that may require additional engineering assessment. This column may also be used to classify any special product or process characteris tics (e.g., critical, key, major, significant) for components, subsystems, or systems that may require additional process controls. Customer specific requirements ma y identify special product or process characteristic symbols and their usage. Whe re a special characteristic is identified with a severity of 9 or 1O in the PFMEA, the design responsible engineer should be notified since this may affect the engine ering documents. “

“Example (SOD): Severity, S = 7 Occurrence, 0 = 3 D etection, D = 5 The resulting SOD is 735The SOD, when sorted in numerical, descending order , will prioritize the scenarios first by severity, second by occurren ce and lastly by detection.
Just as with RPN, use of the SOD/SO index should be used in context of team discussion. Defining priorities simply base d on the SOD has limitations just as with the RPN. For example, a fa ilure mode with a SOD of 711 would be ranked higher (i.e., have to be considered before) a failure mode with 599.”
