For each unit completed more than 12 months ago, you will be asked to
provide a certified copy of a detailed reference letter, performance reviewreport, or equivalent from a supervisor or equivalent in your workplace.

For each unit, ask your third party reporter to:
1. describe recent, relevant workplace activities you have
undertaken and
2. comment on how well you completed these tasks.

Workplace documents to submit as evidence may include:
Samples of worke.g. session plan, assessment instrument, etc. Correspondence / administratione.g. emails, minutes of meetings
Work performance reportse.g. performance review, etc.
You must submit workplace documents as evidence of your current
competence if:
the third party report/s you provide are not satisfactory (see above) or
you are unable to provide a third party report for one or more units; in
this case, you may submit workplace documents instead.
