SMEAR OPTIONS – WAM

What the heck is smear1???

A smear in WAM is an LIS orderable test that can be received from the LIS, reflexed via a rule, or added manually in WAM in order to perform a manual diff or smear review

There are 6 standard smear test codes in WAM that are supported by the SP-1000i and SP-10, as well as the Sysmex DI-60 or CellaVision Series Analyzers, which control the number of slides made and what tower the slide is pulled from

SMEAR OPTIONS

Smear print optionsA blood smear or fluid smear generated from the Sysmex SP devices has these print options for the slide itself

Sysmex WAM supports 3 lines of printed data, 15 characters each

If a 2D barcode is used, then lines 1 and 2 can only be 13 characters


SMEAR OPTIONS – STAT SLIDES

Any order marked as STAT in WAM will automatically pull a slide from tower 2Some sites use colored slides to take advantage of this for a visual reminder to staff (PINK = STAT!!)The LIS must use the priority of S before sending the order to WAM


CELLAVISION SMEAR PROCESSING

Sysmex WAM translates slide orders to a corresponding CellaVisiontab type (WBC, RBC or PLT). The standard translation of the different slide orders is described below:


CELLAVISION SMEAR PROCESSING

WAM pending test codes and internal comments can be seen on the CellaVision DM-Series Analyzers in the Order Data icon

CELLAVISION SMEAR PROCESSING CONFIRM CELL COUNTER

Is a configurable option on the CellaVision DM-Series analyzers and the Sysmex DI-60If enabled, allows the tech to confirm/accept the automateddifferential values


CELLAVISION SMEAR PROCESSING CONFIRM CELL COUNTER

A SMEAR test code must be pending in WAM in order to use confirm cell counter on the WBC tab. SMEAR test code is then resulted with #VDM ( Cell Counter confirmed)


SMEAR* RESULT WORKFLOW

If a SMEAR* test code is pending and a man diff or morphology review is not needed, the test can be resulted:

In the result validation screen, click on validate allA pop-up button appears with these options:

• Once the CBC ADIFF is validated the sample will be removed from the SID list

• If Quit is used, the SMEAR* test remains pending


ADD ON SMEAR* TEST WORKFLOW

From result validation screen, click on the ACTIONS icon

- Click Add- Select the Smear order from the pop-up and click ok- Load the sample on the automation line so the smear can be made


SMEAR* AND NEUT

If both SMEAR* and NEUT are present a manual diff must be performed. Perform the man diff, click on SAVE to trigger final calculations, and validate all.

SMEAR will be resulted with verified


BODY FLUID MANUAL DIFFS

Smears must be made manually and then stained

The Sysmex DI-60 or CellaVision Series analyzers requires a slide prepared on a cyto-centrifuge

FNEUT/FSMEAR are the test codes used in WAM for body fluid samples

In WAM, multiple manual fluid diff keypads can be defined



BF WAM rules can be configured to reflex a FNEUT or FSMEARBy going to the manual diff tab the fluid diff keypad can be selected



Counting less than 100 cells is allowed, the same as it is for a manual diff

Use Query for pending SMEAR orders


SMEAR STATUS SCREEN SLIDE MADE?

Tracks the creation of slides based on the test codes SMEAR* and NEUTInformation is from SMEAR* or NEUT ordered or added to the SID, status messages sent from the SPStatus will be either pending, done or failed

MANAGEMENT REPORTS - SMEAR

Sysmex WAM provides a pre-defined SMEAR counter for counting the number of SMEAR tests and/or monitoring the turnaround time. Additional counters can be defined by the user if more than one SMEAR test is used (e.g. SMEAR 1, SMEAR 2 etc). You can use the management reports to compare MDIFF to SMEAR counts for a given time period
