This report presents (1) standardization of the US scanning
procedure, techniques of US-guided fine-needle aspiration,
and reporting of findings; (2) definition of criteria for classi-
fication of malignancy risk based on cervical US imaging
characteristics of neck masses and lymph nodes; (3) indica-
tions for US-guided fine-needle aspiration and for biological
in situ assessments; (4) proposal of an algorithm for the fol-
low-up of thyroid cancer patients based on risk stratification
following histopathological and cervical US findings, and (5)
discussion of the potential use of US-guided localization and
ablation techniques for locoregional thyroid metastases.

Abstract
Cervical ultrasound scanning (US) is considered a key exam-
ination, by all major thyroid and endocrine specialist societ-
ies for the postoperative follow-up of thyroid cancer patients
to assess the risk of recurrence. Neck US imaging is readily
available, non-invasive, relatively easy to perform, cost-ef-
fective, and can guide diagnostic and therapeutic proce-
dures with low complication rates. Its main shortcoming is
its operator-dependency. Because of the pivotal role of US in
the care of thyroid cancer patients, the European Thyroid As-
sociation convened a panel of international experts to re-
view technical aspects, indications, results, and limitations of
cervical US in the initial staging and follow-up of thyroid can-
cer patients. The main aim is to establish guidelines for both
a cervical US scanning protocol and US-guided diagnostic
and therapeutic procedures in patients with thyroid cancer.

RÉFÉRENTIEL DE BONNES PRATIQUES TECHNIQUES
• R1. L’échographie cervicale doit être effectuée par un opérateur expérimenté. Une sonde à large bande de 12Mhz au moins est conseillée pour le mode B et l’étude doppler doit y être intégrée: Grade: QOE = +; SOR=grade 1 L’échographie doit inclure la loge de thyroïdectomie et les ganglions des secteurs II à VI. Les ganglions peuvent être classés en trois groupes: normal, intermédiaire et suspect. Ils peuvent être représentés sur un schéma indiquant les secteurs: Grade: QOE= +; SOR= grade 1
• Sonde de haute fréquence 12Mhz à large bande passante +/- sonde microconvexe
• Doppler: PRF basse (800Hz) et filtres peu importants
TECHNIQUE
• Extension cervicale + rotations
• Etude transversale de tous les compartiments
• Etude longitudinale des ganglions vus
• Doppler dès que le hile n’est pas vu
LA LOGE DE THYROIDECTOMIE
ASPECT PENDANT LES TROIS PREMIERS MOIS
La loge n’est pas bien étudiable les trois premiers mois après la chirurgie.
ASPECT NORMAL - RELIQUAT
Une lobectomie ou thyroïdectomie quasi-totale laisse en place un reliquat de 0,1cm3 ou moins .
Un reliquat normal est isoéchogène (sauf antécédent de thyroïdite ou Basedow) et faiblement vascularisé.
Sa forme est ovale, son orientation parallèle au plan cutané.
ASPECT NORMAL SANS RELIQUAT
R4. Les lésions suspectes de la loge de thyroïdectomie sont
caractérisées par:
– Une forme irrégulière, plus épaisse que large
– Leur hypoéchogénicité (95%)
– Des microcalcifications et/ou des logettes kystiques
– Des contours irréguliers
– Une vascularisation présente (≈100%)
Grade: QOE= +; SOR= grade 1
20% des récidives surviennent dans la loge de thyroïdectomie.
DIFFÉRENTIATION DES IMAGES BÉNIGNES ET SUSPECTES
• Récidive du carcinome connu
• Reliquat et antécédent de thyroïdite ou Basedow
• Granulome
• Adénome parathyroïdien

• R2. La cytoponction échoguidée des ganglions suspects doit être faite aux moyens d’aiguilles de petit calibre (24-27 gauge). Le produit de ponction doit être analysée cytologiquement et par dosage de la thyroglobuline in situ: Grade: QOE= +; SOR= grade 1
• R3. Les antiagrégants et anticoagulants oraux peuvent être maintenus du moment que l’INR est ≤ 2,5 – 3. Les précautions suivantes doivent être prises: utilisation d’aiguilles de faible calibre (24-27G), guidage échographique, nombre minimal de passages, compression post-procédure jusqu’à 10 minutes et contrôle échographique à la recherche d’hématome à la fin de la compression: Grade: QOE= +; SOR= grade 1
RÉFÉRENTIEL DE BONNES PRATIQUES TECHNIQUES
• Des prélèvements cyto-biologiques sont indiqués quand un ganglion est suspect, en tenant compte toutefois de sa taille. QOE = +++; SOR= grade 1
• Les indications des prélèvements cyto-biologiques dans les ganglions indéterminés en échographie devront tenir compte à la fois du stade TNM, de l’histologie, de la taille et de la valeur de la thyroglobulinémie. QOE = +++; SOR= grade 1
• La mesure de la Tg in-situ est très fiable pour le diagnostic de métastase cervicale des carcinomes thyroïdiens papillaires, même dans les cas où la Tg stimulée est négative ou les anticorps anti-Tg sont positifs. Toutefois, des concentrations élevées d’Ac anti-Tg sériques peuvent interférer avec les dosages de Tg in situ et provoquer des résultats faussement négatifs de Tg in-situ. • Tout ganglion suspect de taille significative doit conduire à une cytoponction à l’aiguille fine avec dosage de Tg in situ.
