Skin incision
Longitudinal incsion with one-half being over the trochanter and one-half below it
Subcutaneous dissection
exposure of the iliotibial band and tensor fascia-latae muscle.
The posterior border of the gluteus medius is retracted using a 90° angled thin Homan retractor.
The external rotators muscle group are identified.
The external rotators muscle group are tagged by sutures and incised
The capsule incised along the longitudinal posterior border of the trochanter
Hip is dislocated posteriorly
Mid-neck resection level
The angle of the resection is parallel to the line connecting the greater and lesser trochanters.
The curved starter awl is used to initiate femoral preparation
Femoral canal preparation
the direction of the broach in the medullary canal following the curvature of the neck
Exposure of the acetabulum
retractors are placed at anterior, posterior and superior borders of the acetabulum
Acetabular reaming
Acetabular trial
Stem trial
the trail neck and head are attached to the broach
Reduction trial
Trial reduction performed using the appropriate sized trial head
Implantation of the stem
The implant inserted first by hand and then subsequently impacted with the impactor into the femoral canal with the stem impactor
Final position
The head is reduced into definitive cup.
