Through our local, regional, and national grant programs, Kaiser Permanente supports a wide variety of organizations serving vulnerable populations and improving community health in San Francisco and north San Mateo counties. These nonprofits were selected to address the local priority health needs identified in the latest Community Health Needs Assessment. Kaiser Permanente is proud to partner with these outstanding organizations working to improve the health of our communities.

Increase Access to High Quality Health Care and Services Bayview Senior Services Healthy Connections provides care navigation to help seniors in the Bayview learn new, healthy habits to manage their chronic diseases.

Breathe California Partnering with Mission Neighborhood Health Center, Breathe California provides targeted asthma education to asthmatic children which helps families manage their child’s disease.

Good Shepherd Gracenter of San Francisco Gracenter helps homeless women recover from drug and alcohol addiction by providing long-term transitional housing with an individualized 12-step recovery plan.

Healthy San Francisco (HSF) Asthma/Chronic Obstructive Pulmonary Disease (COPD) Program The Better Breathing Program at San Francisco General Hospital was developed by University of California, San Francisco pulmonary rehabilitation experts. The program empowers people with COPD to improve the quality of their lives through education and self-management.

Larkin Street Youth Services Movin’ Out helps prepare transition-age, homeless youth who are HIV+ to transfer into adult health care by age 25, ensuring that they manage their disease and maintain a linkage to primary care.

Increase Access to High Quality Health Care and Services, continued

Operation AccessBridging the Healthcare Gap helps undocumented, uninsured Bay Area residents who need non-emergency surgical and diagnostic procedures to receive no-cost, general and specialized surgery at Kaiser Foundation Hospitals.

SteppingStone The Mission Bay Adult Day Health Center uses a culturally and linguistically appropriate approach to treat Chinese seniors who experience chronic depression.

United Way of the Bay Area2-1-1 is an easy-to-remember phone number available 24/7 in 170 languages that seamlessly connects callers to a variety of resources which address their basic needs including food, housing, health care, senior services, child care, and legal aid.

Healthy Eating and Active Living (HEAL)

18 Reasons Youth advocates and food justice leaders take neighborhood residents on local corner store tours to identify healthy foods in the Tenderloin. The organization also teaches Cooking Matters classes at single room occupancies (SROs), service agencies, and healthy corner stores.

American Heart Association Kids Cook with Heart increases access to healthy food by conducting classes for 120 sixth grade students and their parents at Parkside Middle School in San Bruno, as well as education workshops for teachers and staff.

America SCORES America SCORES Bay Area’s Soccer for Success® program uses soccer as a tool to increase physical fitness in underserved public school youth in the Tenderloin and Central City communities.

Boys & Girls Club, Mid-Peninsula The Nutrition and Physical Fitness Program addresses childhood obesity by providing access and opportunity to develop healthy habits and active lifestyles for underserved youth in Daly City and Brisbane.

Boys & Girls Clubs of North San Mateo County Triple Play is a comprehensive health and wellness initiative at six sites in Pacifica and South San Francisco that teaches youth, ages 6 to 18, how to make smart food choices, maintain a healthy body, and form positive relationships.

Children and Nature NetworkGreen Schoolyards for Healthy Communities expands healthy, nature-rich environments for children in low-income communities by developing green schoolyards at early childhood facilities and elementary schools in priority neighborhoods of San Francisco.

Code for AmericaA new digital tool called GetCalFresh makes it easy for underserved communities to apply for and keep CalFresh food stamps to increase consumption of healthy fruits and vegetables.

CommunityGrowsThrough weekly after-school and summer cooking classes and in-school Salad Days, the Seed to Mouth program helps low-income children in the Western Addition learn about, sample, and adopt healthier diets, rich in fresh fruits and vegetables.

Donaldina Cameron House The Kitchen Medicine Program engages low-income Chinatown youth, adults, and seniors in a hands-on, multi-generational cooking and nutrition education program emphasizing plant-based Asian cuisine.

Education Outside The Grounds for Healthy Kids project provides garden-based education to public elementary students from diverse, underserved communities. The program encourages plant-based eating and teaches participants how to prepare healthy meals.

Fresh Approach Fresh Approach utilizes nutrition education and community outreach strategies to engage with low-income, food-insecure families in South San Francisco and help families incorporate fresh and healthy foods into their diets.

HOPE-SF The Peer Health Leadership Program was developed with community members and health providers to address pressing health and social issues facing children and families in public housing in San Francisco’s southeast neighborhoods.

Lemonade, A Yoga Program Young males incarcerated at San Francisco’s Juvenile Hall and Log Cabin Ranch in San Mateo are given the opportunity to increase their physical activity through yoga, with instruction and support to build the physical practice into their lives.

Portola Family Connection CenterFoods for a Healthy Life helps families increase their understanding and consumption of plant-based foods using the U.S. Department of Agriculture’s MyPlate and Meatless Monday strategies.

Project Open Hand Changing Health through Food Support (CHeFS) combines a research study with special nutrition intervention and education to improve the health indicators of food-insecure persons living with HIV/AIDS.

Project Read Low-income Latino students and their families living in South San Francisco who attend the Community Learning Center’s homework club are encouraged to eat more fruits and vegetables and increase their daily physical activity.

Pacifica Gardens Garden-based education field trips for Pacifica School District’s elementary, homeschool, and special needs classes also provide community service and senior project opportunities to local high school students.

Second Harvest Food Bank Enhancing Healthy Lifestyles increases access to fresh produce and healthy foods for low income families, seniors, and individuals in north San Mateo County through enrollment in food distribution programs, promotion of CalFresh food stamps, and education to increase consumption of healthy foods.

Shape Up San Francisco Coalition During a day-long symposium to mark the 10th Anniversary of the Shape Up San Francisco Coalition, experts convened to discuss equitable and sustainable strategies to prevent chronic disease for the next 10 years. The discussion included encouraging healthy eating and physical activity, and discouraging the consumption of sugary beverages.

Starr King Elementary School The Rising Stars Afterschool Program provides students at Potrero Hill’s Starr King Elementary School the opportunity to cultivate mindfulness through kung fu as a specialized enrichment activity at this Chinese immersion school, and positive peer interaction and exercise through supervised soccer play.

The HEAL Project Growing Roots in Nutrition Education provides a hands-on experience to all South San Francisco elementary school students through the twice-annual field trip to the San Mateo County School Farm. The students plant, harvest, learn about agriculture and the environment, as well as increase their fruit and vegetable consumption.

YMCA of San Francisco Activity Rx establishes direct clinic-to-community referrals to evidence-based programming at the local YMCA that increases physical activity and reduces the risk of developing type 2 diabetes.

Safe & Healthy Living Environments / Behavioral HealthBoys & Girls Clubs of San Francisco Why Try resistance education teaches middle school youth the social, emotional, and leadership principles that can decrease risky behaviors and bullying, and increase protective factors in relationships and social settings.

Compass Family Services Homeless children and their parents are assessed for toxic stress and engaged in appropriate interventions to reduce the incidence of domestic violence and child abuse in families who are experiencing periods of homelessness.

Daly City Peninsula Partnership Collaborative This project expands access to marriage and family therapy for under-insured, low-income parents in crisis and children with behavior challenges or adverse childhood experiences.

Daly City Youth Health Center The Health Center increases access to behavioral health services for low-income youth in Northern San Mateo County by providing mental health and substance abuse counseling services at the Jefferson Union High School campuses and onsite at the clinic.

healthRIGHT 360 Project Connect Three provides prevention and early intervention mental health services for youth, parents, and teachers at Pollicita Middle School in Daly City and El Camino High School in South San Francisco. The program also provides outreach to parents and community members to increase knowledge and access to mental health services.

Huckleberry Youth Programs Huckleberry is committed to improving trauma screening and expanding trauma-informed care for at-risk teens at its residential, clinic, and juvenile justice programs.

Instituto Familiar de la Raza The Trauma Recovery and Healing Trainings expand the understanding of trauma and its effects on young people for staff addressing the unique mental health needs of Latino/Chicano/Indigena populations.

Jefferson Union High School District Incoming and at-risk freshmen at Oceana and Terra Nova High Schools in Pacifica are matched with a junior or senior who mentors them throughout the school year with weekly meetings to provide support and encourage healthy lifestyle choices and successful academic habits.

La Casa de las Madres The Teen Program delivers developmentally-appropriate education and intervention to prevent domestic and dating violence among teenagers and help girls feel safer, more confident, and less likely to engage in abusive situations or relationships.

Niroga Institute The Transformative Life Skills curriculum provides tools for stress resilience, self-awareness, emotional regulation, and building healthy relationships to reduce intentional injuries and trauma among high school students in the Mission and Bayview neighborhoods.

Prevention Institute This project advances an Adverse Community Experiences and Resilience (ACE/R) Framework to develop and disseminate an understanding of the ACE/R approach and emerging solutions to community trauma.

Rape Trauma Services Crisis mental health services address the multiple healing needs of child survivors of sexual trauma in north San Mateo County which include 24-hour intervention, counseling, advocacy, medical/legal support, and forensic accompaniment at the hospital, District Attorney’s office, police departments, and schools.

Samaritan House The Safe Harbor BRIDGE (Building, Recovering, Initiating, Developing, Growing, and Empowering) is a mental health case management program that increases access to a range of services for homeless adults in San Mateo County who reside at the Safe Harbor shelter in South San Francisco.

San Francisco Child Abuse Prevention Center Integrated Family Services is an intensive, limited-duration intervention for high-need families from the city’s poorest communities who have risk factors for violence and abuse.

St. Vincent de Paul Society St. Vincent de Paul’s Riley Center is partnering with DeMarillac Academy in the Tenderloin to provide an anti-bullying curriculum to 4th and 5th grade students, and a healthy relationships curriculum to 7th and 8th grade students.

Youth Law Center The Center works with public agencies to transform child welfare practice to support the healthy development and resilience of children in foster care who have suffered abuse and neglect by providing developmental experts, training, and technical assistance.
