Surgical Kit Maintenance

Sterilization and Instrument Care Procedures

• Please follow legal regulations, as well as hygienic guidelines to prevent contamination andinfection through prevention.

• Please remember that you are responsible for the maintenance and sterility of your medical/dental products/device. It is important to use and follow proper cleaning, disinfection andsterilization procedures.

• It is also important to follow the manufacture!s recommendation on the usage of drills. Pleasekeep a log as to how many times the drills are used.

• Drills are used per implant placed not per patient. Bone density determines the life of the drills.• Replace white and red o-rings on adapters and hex drivers, if worn and dried out.• Drills should be considered for replacement around 40 uses based on bone density.

01 All instruments immediately after use must be pre-soaked for a few minutes in a germicidal bath to loosen and prevent debris from attaching to instruments. Do not soak over-night.

02 Scrub with a soft brush to remove any debris.03 For internal irrigation drills use a reamer or small gauge needle to cleanout drill internally.04 If using an ultrasonic cleaner, wrap drills in a 2 x 2 to prevent rubbing against each other.05 Rinse thoroughly under warm water.06 Clean all instrument trays with a germicidal cleaner prior to replacing instruments in kit.07 Dry completely and place back into kit.08 Always check for damage or corrosion after rinsing and drying.09 Seal the tray in a sterilization pouch.10 Sterilize using a steam autoclave in 121°C/250F for 30 minutes or refer to manufactures

recommendtions.11 Store in a dry area at room temperature.

Governing Terms :Any shipment of product shall be deemed to be according to the terms and conditions stated herein. Anyand all terms and conditions submitted by Purchaser are hereby rejected. Taxes : The prices set forth herein do not include any sale, use, excise, ad valorem, property or other taxesapplicable to the sale, use or delivery of the products, all of which shall be paid by Purchaser separatelyor added to the contract price and paid by Purchaser to Dentium USA. Pricing : The prices set forth for the products are in United States Dollars. Prices are subject to change without notice. Payment : Payment terms shall be as set forth under the “Terms” section on the front of the invoice. Delivery : Products sold hereunder shall be delivered F.O.B. manufacturer!s plant or distributor!s plant, as applicable.Purchaser shall assume reponsibility for all subsequent delivery or shipping charges.

Warranty : Dentium USA or any of its affiliates makes no warranty, express or implied, except that its products shall befree from defects in material and workmanship. This warranty applies only to the original Purchaser and isnon-transferable. In the event of a product defect, please immediately notify Dentium USA of the defectprior to returning the product. Devices shall be sterilized prior to return. As its sole obligation and asPurchaser!s sole remedy under this warranty, Dentium USA"will at its option, either repair, replace or issuecredit for such product. Purchaser assumes all risks and liability resulting from the use of these products,whether used separately or in combination with other products.

Education; Modification of Products : Dentium USA"strongly recommends completion of formal post graduate implant education and strictadherence to the procedures described in Dentium USA!s implant instruction manuals. Dentium USAcontinuosly strives to improve its products and therefore reserves the right to improve, modify or discontinueproducts at any time, or to change specifications of the products without notice and without incurring any obligation.

Return Policy : A Purchaser may return any unopened Dentium USA product that is in its original packaging within90 days of the invoice date. Dentium USA products returned within 30 days of the invoice date willbe accepted without any restocking fee to the purchaser. Dentium USA products returned between31 and 90 days of invoice date will be subjected to 15% re-stocking fee.

No returns will be permitted over 90 days from invoice date. To return an item, contact your local salesrepresentative or call Dentium USA Customer Service Department for a Return Material Authorization(RMA) number. Issuance of a RMA number does not guarantee that the product will be accepted forreturn. No product returns will be accepted without a RMA number. Dentium USA product will beaccepted for return only if the Dentium USA product is (1) returned in its original, unopened package(including autoclavable products); (2) received by Dentium USA within 90 days after the invoice datefor that product; and (3) returned freight prepaid and accompanied by the Dentium USA RMAnumber. (4) implants and prosthetic parts being returned for credit or exchange must have at leasta 6 month shelf life remaining. Individual items from a kit cannot be returned for credit.

Previously exchanged items cannot be returned for credit. Please do not give any item(s) to bereturned to your local Sale Representative. All products must be sent directly to the ReturnsDepartment of Dentium USA. Procedures and policies as stated are subject to change without priornotice.

Exchange Policy :Dentium USA products may be exchanged within 180 days of the invoice date, without a re-stockingfee, for other Dentium USA products of equal or lesser value within the same product family asdetermined by Dentium USA (ex. implant for implant, abutment for abutment, bone graft materialfor bone graft material). Dentium USA products may be exchanged within 180 days of the invoicedate for other Dentium USA!products of greater value within the same product family as determinedby Dentium USA (ex. implant for implant, abutment for abutment, bone graft material for bone graftmaterial) with the price differential paid by the Purchaser. In both cases, the exchanged DentiumUSA product must be (1) returned in its original, unopened package (including autoclavable products);(2) a currently offered Dentium USA!product ( discontinued product not included); (3) returnedfreight prepaid and accompanied by the Dentium Return Material Authorization number providedby Dentium USA"s Customer Service Department. Licenses, permits and export control :The timely securing of permits, licenses or other local, state or federal governmental approvals required inconnection with Purchaser!s use of any product hereunder shall be the sole responsibility of Purchaser andPurchaser shall bear the cost thereof. Unless otherwise agreed between Dentium USA and Purchaser,products sold hereunder are only for use in the United States. Purchaser agrees that Purchaser will complyfully with all applicable state and federal laws and regulation and Purchaser shall indemnify Dentium USAfor all losses, damages and penalties incurred as a result of any violation of applicable state or federal lawor regulations. Force Majeure :Dentium USA shall not be liable for any loss or damage due to failure or delay arising out of any causebeyond the reasonable control of Dentium USA. In the event of any failure or delay resulting from suchcauses, an equitable adjustment of delivery and any other appropriate terms and conditions shall be made.No such failure or delay shall be the basis for an increase in neither Dentium USA!s obligations nor anytermination by Purchaser.
