SummitStone Behavioral Health Care Manager

The North Colorado Health Alliance (NCHA) and SummitStone Health Partners have partnered to provide care management for medically complex Medicaid patients. The goal of this program is to optimize client/patient health, functioning, & self-sufficiency; improve the quality of care and outcomes and control costs.

The clinical case manager/behavioral health specialist will work embedded in the NCHA Southern Larimer County (Loveland) Community Care Team. This position is responsible to provide Care Management/Care coordination Services on site and in the community to NCHA clients/patients.

Required: Bachelor's degree in behavioral science; Work experience with adults with severe and persistent mental illness or individuals with similar human-service's needs; Valid Colorado driver's license

Preferred: Bilingual/Bicultural; Trauma Informed Care experience, previous experience working in a medical setting
