SUGAR’S GIFT, INC.

THE NEED FOR THE PROJECT The mission of Sugar’s Gift, Inc. is to provide hospice, euthanasia and end-of-life veterinary services for terminally ill pets in their own homes. This allows them a comfortable, peaceful setting, where they have lived and been loved, to cross over the Rainbow Bridge with dignity. Our services overcome conditions that could be due to the pet’s inability to be moved or treated at a facility, or due to the owner’s inability to leave the home.

Our 10-year old Yorkie, Sugar, passed away from pancreatic cancer in 2013. We made sure she crossed over the Rainbow Bridge from home with comfort, dignity and peace. Sugar’s Gift to your Little Angel is to allow them the same privilege: Sugar is helping them cross over the Rainbow Bridge from home, with the help from others who have donated funds to make that happen. Every Little Angel should be able to spend their last moments in the place they are most comfortable – at home.

Sugar’s Gift, Inc. is the only non-profit, 501(c)(3) organization in the country whose mission is to help terminally ill pets cross over the Rainbow Bridge from home with comfort, dignity and peace.

OBJECTIVES AND STRATEGIES

1. Sugar’s Gift has 2 main objectives: • First, and foremost, we aim to help those who are home-bound and need to have

one of their babies cross the Rainbow Bridge from home. In addition, those animals who are terminally ill and are in pain, and cannot be moved comfortably, also need to cross the Rainbow Bridge from home with dignity and stay where they are loved and have been loved.

• Second, we will hold workshops for pet owners to help them understand estate planning for them, as pet owners. We will educate them on pet trusts and other specific language they need to include in their documents to ensure the safety and health of their babies when the owners pass on.

AUDIENCE SERVED

We will serve any pet owner who either has a terminally ill pet that cannot be moved, due to pain levels or medical issue. We will also serve any home-bound pet owner who cannot transport their faithful pet to the veterinarian for euthanasia. We do not ask anyone to pay for our services; however, if they choose to donate something to help the cost, they are welcome to do so.

We will also serve every pet owner who can attend our free workshops on Pet Trusts and the need to include your pets in your estate planning documents.

GOALS FOR OUR PROJECT Our goal is to help one pet per day cross over the Rainbow Bridge with comfort, dignity and peace. It costs us approximately $1,000 to help one pet. We expect that once our presence is known, we will need to fund one pet per day by the end of the first year. By the end of year 2, we hope to be not only in the Sarasota/Bradenton region, but in all of Florida. Then, by the end of year 5, we want to be nationwide. HOW TO MEASURE SUCCESS We will follow up with all pet owners whose pets we are able to help. Most of the time, pet owners own more than one animal. We hope to help all of the pet owners’ pets when the time comes. We will be able to gauge how many more animals are scheduled each week, compared to the previous weeks, and we will track the numbers on a daily basis. SUSTAINABILITY We are going to actively solicit funds from individuals, corporations and other grant makers. The money used from the Pollination Project will be used for the local advertising and marketing to put our organization in magazines, online advertisements and booths at local events.

Dr.AnneChauvetDr.AnneChauvetwasborninFranceandraisedinGabon,AfricaaswellasSaskatoon,Canada.ShereceivedherDoctoratefromtheUniversityofSaskatchewan,inCanada.ShecompletedaninternshipinSmallAnimalMedicineandSurgeryattheUniversityofIllinois,andaresidencyinNeurology/NeurosurgeryattheprestigiousUniversityofCalifornia,inDavis.ShereceivedherdiplomatestatuswiththeAmericanCollegeofVeterinaryInternalMedicinein1995andisoneoflessthan250neurologydiplomatesinthenation.Dr.ChauvetwasaclinicalinstructorinNeurology/neurosurgeryattheUniversityofWisconsininMadison,andwasinvolvedingenetherapyresearchforbraincancer.In1998,shedevelopedaninternationalcourseinBasicScienceinVeterinaryandComparativeNeurologyandNeurosurgerythatisnowconsideredastandardforveterinaryneurologists.Sheispublishedinbothveterinaryandhumanmedicaljournals,andcontinuestospeakinternationally.Dr.ChauvetisanactivememberoftheSWFVMA,IWF,FVMA,AVMA,andACVIMDr.Chauvethaswonamultitudeofawardsandhasbeenrecognizedbylocal,nationalandinternationalorganizationsforhercommunityinvolvementandsupport.Inhersparetime,Dr.Chauvetvolunteersatlocalchildren’scharities(Children’sFirst,SafeChildrenCoalition).Dr.Chauvethastwoboys,VincentandDevan,whokeepherenergizedandsmiling.Sheandtheboysownamultitudeofpets.

KennethG.StewartIII,Vice-PresidentandSecretary KennethwithhisLittleGirl,SugarKennethwithhisLittleBoy,ChampKennethStewartisaretiredbusinessownerandanimaladvocate.Justaftergraduatingfromschool,hestartedhisfirstcompanyandhasbeeninvolvedwithvarioustypesofsuccessfulcompaniesthroughouttheyears.Hehasanumberofpatentsinvariousfields,andcontinueshisresearchanddevelopmentofmoreideaswithhiscurrentcompany.Inaddition,Kennethhasdevotedagreatdealoftimetohelpunderprivilegedchildrenbymentoring,tutoringandworkingwithnon-profitorganizationsinvariousareasofthecountry.Aspartofhismentoring,inhisearlieryears,KennethwasaBigBrothertoyoungmenwhoneededguidance.Hehasalwaysbeeninterestedindedicatinghisextratimetoconsultingwithothersmallbusinessowners,andhelpingthembecomesuccessfulbyteachingthembasicbusinessprinciplesandworkingwiththemthroughoutthevariousstagesoftheirgrowth.Kennethhasalwayslovedanimals–hisownpets,aswellasbeinganadvocateforallanimalswhoneedavoice.Heandhiswife,Nikki,haveownedavarietyofanimalsovertheir30yearsofmarriage.HisbelovedChamp,aYorkiethatKennethrescuedin2006fromdeathatAnimalControlafterChampwashitbyacar,wasKenneth’sprideandjoy.And,conversely,KennethwasChamp’shero–fromthefirstdaytheyweretogether.AfterrescuingChamp,Kennethhadseveralveterinarysurgeonsintheareaperform
