To enhance capacity of service providers for early detection and management of selected NCDs at UpHC and CC
To establish a referral linkage from CC to Upazila Health Complex
To create awareness among population about NCD risk factors

Increased referral from OPD to NCD Corner
Increased referral from CC On an average 30 NCD (diabetic and
hypertensive) patients recruited per month

educators at NCD corners Supply of medicine for hypertensive and

proper follow up.
