Revised National TB Control Programme

Dear Colleagues,

We are pleased to inform you that RNTCP Gujarat has initiated Active case finding activities for selected 40 TUs in 17 districts in focus manner, First patient is initiated on treatment for BDC CAP project at Ahmedabad DR TB site for 4 districts (Ahmedabad Rural, AMC, Kheda and Gandhinagar in this quarter, e-Nikshay piloting was initiated in Mahesana district as well as 99 DOT project initiated for all RNTCP patient on first line treatment in Ahmedabad Rural and Tapi district, UATBC expansion Rajkot and SMC area on 03/08/2016. 126 and 34 patient enrolled by quarter end in SMC and Rajkot respectively, National level STO consultant meeting was conducted, state was appreciated for its initiatives like Public health action following, telephonic phone call register at district level and individual DR TB patient tracking through Google spread sheet, ICT enabled free drug access for private patients – TB free Rajkot project’ was selected for oral presentation and ‘IBRC’ as poster presentation during National Summit carried out at Tirupati for Innovation in Health.

RNTCP performance during 3rd quarter 2016: During the 3rd quarter 1,97,397 presumptive TB patients were examined, 16,684 sputum positive TB patients were diagnosed, and a total of 22,102 all forms of TB patients were registered for treatment. The annualized total case notification rate is 135 cases per 100,000 population. With a total of 10,542 New Smear Positive patients being registered for treatment; the new smear positive TB case Notification Rate (annualized) is 62 per 100,000 population. In addition to this, 2,449 New Smear Negative patients, 2,944 New Extra Pulmonary patients registered for treatment in this quarter. The treatment success rate amongst the New Smear Positive pulmonary TB cases registered in the 3rd Quarter 2015 is 89% and the Sputum Conversion rate of patients registered during 2nd Quarter 2016 is 92%. In TB-HIV intensified services 97% patients were tested for HIV out of 22,102 total patients registered during 3rd quarter 2016. PMDT: During this quarter, 8789 MDR-TB suspects were tested for drug susceptibility. Total 587 patients were diagnosed as having drug resistant and eligible for second line treatment. Total 532 patients were put on Category-IV regimen in this quarter. Additionally, 68 patients were put on Category-V regimen. Notification of TB patients from private health providers: Annualized TB case notification from private sector is rose from 51 (2Q16) to 53.5 (3Q16). A total of 26308 TB patients are notified from private health facilities since January to 3Q16. Trainings, Supervision and Review meetings: In addition to quarterly review meeting, two state level internal evaluations were conducted during quarter in Bharuch and Gir Somnath and Supervisory visit and on site evaluation from IRL also has been conducted in the district by state level team. In the quarter, 7 batches of different kind of RNTCP training were conducted including state level sensitization workshop for DPC, DPPMC, DPS. Total 260 participants were trained.

99 DOTs – ICT Enabled Treatment Adherence Monitoring System 

Background:  99  DOTs  is  Information  &  Communication  Technology  based  intervention  currently executed for all adult TB –HIV co infected cases throughout Country and for all TB cases in Ahmedabad Rural and Tapi district while it provides monitoring support to private patient in Rajkot district. 99 DOTs is  IT  tool  which  digitalize  drug  doses  monitoring  mechanism  through  patient  initiated  phone  call. Current Mechanism of treatment supervision (DOTS) would be synergised by 99 DOTS. It provides real time access  to  treatment adherence  status of patient.  If patient misses  the dose,  in addition  to DOT provider, all  level of provider  (health worker, STS, MO PHI and DTO) will be  intimated simultaneously through SMS for timely retrieval which is really a difficult task in current system.  

Process: Once the patient diagnosed and put on treatment at PHI, patient is expected to be enrolled in 99 DOTs website. All possible patient  contact numbers  should be  collected and  recorded  in website. Patient is briefed about the mechanism and communicated about the need of giving miss call at number displayed under the pills. There are different types of envelops available for various weight band where patient will get unique toll  free number behind the pills every day.   Each call made by patient will be disconnected after a response of ‘thank you’ from other end.  This will update today’s dose is consumed on  backend  server.  If  patient  failed  to  give  call  by  3  PM,  a  reminder  SMS  (local  language) will  be generated and send to the patient. In addition to this, an auto generated SMS directly send to level 1 – person responsible  for retrieval usually DOT provider. STS will get  level – 2 SMS  for  individual patient missed dose alert. Higher level authority (MOTC & DTO) will get summary SMS on weekly basis.

 Advantages:  • To real time monitoring of treatment adherence of large number of patient simultaneously. • Auto digitalization of treatment adherence records (Tick marks) and print of treatment card • Segregation of priority patient who needs immediate attention by Staff. • Can  differentiate manually  updated  treatment  card  and  record  generated  by  call  received  from 

patient.  • Login and password are similar to Nikshay 

 Challenges: Basic mobile  literacy  and  accessibility  to mobile device  and network  for patient.  Patient those  who  do  not  have  access  to mobile,  DOT  provider mobile  will  be  registered  for  the  99  DOT monitoring. RNTCP has plan of  switching  to enikshay  from Nikshay which  is well  sync with 99 DOTs. Proposed system could be boost up by the call center support.  

 Summary Points: 99 DOTs provides opportunity  to monitor patient distally and early  identification of treatment interrupters however, timely grass root level action towards feedback received is essential to avail its benefit.
