The time is now! It is time to explore how to make our health system more amenable to care for our aging population. Never in the history of humankind have we had so many people over the age of 65 alive at one time. This requires new insights and perspectives on how healthcare is delivered. Not only do we need to understand disease centered care for older adults, we need consideration of what matters to each individual, their mentation, medications and mobility: the “4Ms.” It is the culmination of these nondisease-centered areas of health that bring us to a possibility of true age-friendly care. This is care that needs integration at a population level and implementation on a personal level. How we achieve age-friendly healthcare for older adults involves exploring new information and ideas for healthcare professionals to not only contemplate, but to act on.

The conference will aim to provide examples of age-friendly care: What does age-friendly care look like? How can we work together to create an age-friendly culture? And how do we provide the care? Participants will learn that older adult concerns and dilemmas are often interrelated and cross several of the “4Ms”, such as ethical decisions, determining prognosis, driving concerns, sleep difficulties in dementia patients, use of cannabis for pain control, motivating mobility, to name a few. Many of our topics are delivered in 15-minute segments so you are able to get a brief understanding of multiple considerations.

This conference is meant to enhance the knowledge base and skills of physicians, nurses, pharmacists, psychologists, dietitians, allied health and rehabilitation therapists, social workers, health facility administrators and other interested healthcare professionals.

St. Vincent Medical Group Center for Healthy Aging (CHA) is a premier patient and family focused center of interdisciplinary geriatric and gerontological clinical practice, applied research and education that disseminates research findings and best practice information from local to international levels, and serves as an authentic voice in public discourse on aging.

Designation statement: St. Vincent Hospital and Health Care Center Inc. designates this live educational activity for a maximum of 6.5 AMA PRA Category 1 Credit(s)TM. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Nursing: St. Vincent Health is an approved provider of continuing nursing education by the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation (OBN-001-91). 6.75 continuing education contact hours will be provided.

Pharmacy : St. Vincent Hospitals and Health Services Pharmacy Department is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. UAN: 0556-0000-19-001-L04-P Application Activity. This program is approved for 6.5 Continuing Education contact hours CE (0.65 CEU). Active participation in activities, attendance at all sessions, and an evaluation of speakers and programs is necessary for credit to be awarded. National Association of Social Workers: St.Vincent Social Work CEU Statement of Support: In partnership with the St. Vincent Outreach Department-an approved CEU provider by the Indiana Professional Licensing Agency — The Indiana Social Worker, Marriage and Family Therapist and Mental Health Counselor Board has approved this organization to provide 7.0 Category 1 (formal) Continuing Education for LSW, LCSW, LMHC, and LMFT. However, the licensees must judge the program’s relevance to their professional practice. The CEU’s provided through the license of St. Vincent Outreach Department.

Indiana State Board of Health Facility Administrators: Approved by the Indiana State Psychology Board to present the continuing education program for 6.0 hours.

Indiana State Board of Occupational Therapists: Approved by the Indiana State Board of Occupational Therapists to present the continuing education program for 6.0 hours.

The facility entrance is one block west of US 31 on IllinoisStreet between 116th Street and 126th Street, with the entrance behind the building on Illinois Street. Going north on Illinois Street from 116th Street, the facility will be on the right.
