Praised be Jesus Christ!
As I wrote in my column last week, I would like to honor Respect Life Month in October by addressing certain life-issues in my columns throughout the next few weeks. To begin, I would actually like to start at the end, with certain end of life concerns.
Two years ago, our Diocesan public policy weekend focused on the issue of new legislation being introduced in various areas of the country at the state level, which would legalize what is euphemistically called “physician-aid-in-dying.” Several states have already passed such legislation.
The term is highly misleading because the goal of aid-in-dying “treatment” is nothing other than the direct and intentional termination of human life of patients who are seriously ill. In actual fact, it is either physician-assisted suicide, or physician-induced murder, depending on who specifically administers the lethal dose of drugs.
Such practices are entirely contrary to genuine Catholic end of life care which not only forbids direct termination of life, but also places great value in the reality of redemptive suffering.
Our Divine Savior won our salvation on the cross through his suffering. Thus, when we, who are connected to Christ through Baptism and faith, offer our sufferings in union with his perfect sacrifice on the Cross, even our pains and sufferings are transformed and elevated in HIM, so as to redound to our own spiritual growth, and the spiritual benefit of those for whom we offer our prayers.
I can honestly say, as a first-hand witness, that the experience of true suffering, as a situation involving great humility, can often open up a patient to repentance and conversion in ways that no homily or moral exhortation could ever achieve.
As the great C.S. Lewis explained: “God whispers to us in our pleasures, speaks to us in our conscience, but shouts in our pains.” It is his megaphone to wake a sleeping world.
Physician-aid-in-dying is also entirely different from good palliative care, which focuses on pain management for terminally ill patients, and should, ideally, also take into account the emotional and spiritual needs of the patient (i.e. the reception of the sacraments of confession, anointing, and Holy Communion).
When it comes to end of life decisions from a Catholic perspective, there are a couple of important principles to keep in mind:
1. In terms of end of life care, it is not the case that a person is bound to continue and/or undergo any and all possible treatments, no matter how limited the chance of success or how grave the burdens involved.
Whenever a particular treatment has become excessively burdensome relative to the reasonable expected outcome, such treatments are called “extraordinary care” and can be refused and/or discontinued.
Those treatments are not morally obligatory for that particular patient, at that particular time. For example, in cases when life support is removed from a patient who has no serious hope of recovery, this is not morally problematic. It is simply a matter of allowing death to take its course.
2. On the other hand, it is also not the case that a person is free to refuse all types of medical treatment without qualification.
Whenever a medical treatment is not seriously burdensome relative to the expected outcome in that particular situation, the treatment is considered “ordinary care” and is morally obligatory for that person.
3. A special case for consideration involves the issue of nutrition and hydration. The principle here is that nutrition and hydration are not to be withheld or refused by a patient whose body is still able to assimilate it, even if the nutrition and hydration are administered by artificial means (i.e. a feeding tube or an IV).
In other words, the patient’s cause of death should not be due to starvation and dehydration, except when the body itself is “shutting down” and no longer able to process that basic nourishment. Patients should make this principle abundantly clear at the outset whenever palliative care is discussed, as it is not presumed by all palliative care teams.
4. Finally, in light of the above principles, when it comes to advance directives regarding end of life care, I generally encourage people simply to appoint a healthcare proxy who can make decisions for that person in the event of their incapacity.
Other instruments such as Living Wills or even DNR orders are not able to factor in all possible medical scenarios, and can unintentionally limit one’s options in unique cases.
There are exceptions to this rule, in particular regarding DNR orders, when it becomes obvious that such measures would always be extraordinary care for a particular patient, but it is generally safe just to appoint a healthcare proxy, with the stipulation that all end-of-life decision be made in accordance with the teachings of the Church.
