CERTIFICATE AND/OR LETTER OF TRANSFER IF NEEDED
• Provide a baptismal certificate for all first-time registrants, including incoming kindergarteners and older children who are new to our program.
• Provide a letter of transfer if your child completed the previous year of Faith Formation at another parish.
