The Rides for Wellness Program provides door-to-door transportation to essential medical andbusiness appointments.

a resident of Salt Lake County
without other transportation

$A $2.00 donation is accepted perclient, per one-way ride. There areno income eligibility guidelines toqualify for the Rides for WellnessProgram.

transitions of aging

Vital medical appointments
such as dialysis, chemotherapy,
and radiation treatments as well
as doctors appointments.
Trips to pick up prescription
medications and other essential
business.
Individual & group trips to the
grocery store.
Trips to visit spouses in
hospitals and nursing homes.
Rides are provided between 8:00 amand 4:15 pm, Monday, Tuesday,Wednesday, and Friday; and onThursday between 9:30 am and
4:15 pm. Rides can be scheduled upto one week in advance.
