DONOR ADDITION

Screening tests for sperm quality: semen analysis, SCSA for DNAfragmentation, strict sperm morphology and trial wash.

ICSI: Sperm Microinjection as required.•

Genetic screening: all Fertility Associates clinic recruited donors are screenedfor three commonly inherited genetic conditions; Cystic Fibrosis, Fragile XSyndrome and Spinal Muscular Atrophy. Donors are also screened forinfectious diseases.

Donor preparation: initial medical and counselling consultations for the donor.•

Donor addition is part of Stage 1 which is due for paymentwhen collecting medication (IVF) or before insemination(IUI).

Personal donors: a screening fee will apply:for more than one donor•if you do not proceed to treatment with yourpersonal donor.

Sperm donor addition

The reimbursement applies to all recipients, whether using aclinic or personal donor, and whether the treatment is privateor publicly funded.

Personal donors may elect to forgo reimbursement - this wouldneed to be agreed in advance on a ‘case by case’ basis betweenthe clinic and the donor.

Recipients pay the donor reimbursement when the donor startsher treatment cycle.

•

The standard reimbursement covers typical expenses such asparking, childcare, time and inconvenience. If the donor livesout of town and requires air fares and accomodation, theseexpenses will require extra reimbursement.

When all parties are confirmed to start ECART workup(doctor appointments, counselling, screening etc) priorto meeting with the ECART committee.

The ECART application process typically takes 4-6 monthsas the Committee only meets 6 times per annum.

Intending parents and surrogates may require additionalmedical assessments. Your counsellor or fertility doctorwill advise.

Legal fees are not included: Treatment involving surrogacyand donor embryos requires each party to obtainindependent legal advice.

Counselling and guidance throughout the process:Each party to the ECART application will be required toattend at least two counselling consultations.
