Lee Library, BYU Brigham Young University (“BYU”) requests permission to record on audio and/or in video the presentation (workshop, conference, open house, program, or remarks) you will be giving at the L. Tom Perry Special Collections or the Harold B. Lee Library Auditorium at Brigham Young University on the following dates October 21 and 22, 2004. This recording will allow faculty, staff, students, and other interested parties to benefit from you presentation. Copies of this presentation may be published or reproduced (using all now-known media or hereafter developed platforms and/or technologies), and distributed in whole or in part by BYU. Copies may be made available for purchase or rental by BYU or its agents at a minimum price to recover the costs. These copies will be distributed for educational purposes only.
