Brigham Young University (“BYU”) requests permission to record on audio and/or in video thepresentation (workshop, conference, open house, program, or remarks) you will be giving at theL. Tom Perry Special Collections or the Harold B. Lee Library Auditorium at Brigham Young

University on the following dates October 21 and 22, 2004.

This recording will allow faculty, staff, students, and other interested parties to benefit from youpresentation. Copies of this presentation may be published or reproduced (using all now-knownmedia or hereafter developed platforms and/or technologies), and distributed in whole or in partby BYU.

Copies may be made available for purchase or rental by BYU or its agents at a minimum price torecover the costs. These copies will be distributed for educational purposes only. Any fundsreceived in distributing your presentation will be used to recapture costs related to presenting theprogram, copying, distributing or preserving your presentation.
