What is the South West IPC programme?

• We are one of nine demonstrator sites nationally • Bringing together local government, VCSE sector and

the NHS to work differently to support people with complex care needs

• It is about a power shift from services to the people they are there to serve

• IPC aims to use person led approaches, with the option of a personal budget to integrate support for people

• People will have one care plan focused on what matters to the person.

How are we making it happen?• Starting with small scale

implementation sites across the region, testing and learning in different settings, working with people with different needs

• Developing a systems change programme, taking the learning from sites and making the changes needed to scale up
