A Just Harvest (Food/Food Pantries) 7649 N. Paulina St. Chicago, IL 60626 Tel: 773-262-2297 Contact: David Crawford [email protected] ajustharvest.org A Just Harvest’s mission is to fight poverty and hunger in the Rogers Park and greater Chicago community by providing nutritious meals daily while cultivating community and economic development and organizing across racial, cultural and socioeconomic lines in order to create a more just society. Programs: On-Site Food Distribution: On the third Monday of each month our clients are able to receive up to 40lbs. of food from the hours of 9:30am-1pm. This program will take place at our N. Paulina address and should be confirmed the Friday before distribution to make sure that no last minute scheduling changes have been made. We recommend that those who will be attending the food distribution program bring either boxes or alternative ways of transporting any received food items. Community Meal Service: Every Monday through Sunday a daily evening meal service will be provided to those in need of a hot meal. During the week the hours will be from 5:30-6:30pm and on the weekends the time will be from 5-6pm. Free Lunch Programs: Because some children come from families who cannot afford the cost of living and therefore must sacrifice what they can to survive, our organization is here to make sure that every child that needs a hot lunch meal receives one. We also run a lunch program during the summer when school is not in session from June-August. We also provide hot lunches to children during long holiday and spring breaks. Please contact us either online or over the phone to receive a schedule. After School Snack Programs: Our organization provides two opportunities for children to receive after school snacks. Monday through Friday we provide after school snacks at the Gale Community Academy and the Starfish Learning Center located in the Howard Area.

Heartland Health Outreach’s mission is to transform healthcare for the most vulnerable—particularly people experiencing homelessness, mental illness or addictions, or struggling with multiple chronic illnesses—improving health for all and the well-being of our community. The HHO provides comprehensive primary care services, including: family medicine, HIV specialty care, medical case management, prenatal care, medical interpretation, internal medicine, laboratory and prescription services, benefits and entitlement assistance, medication assistance, and referrals to specialists. MENTAL HEALTH: HHO’s mental health services include counseling for individuals and groups, addiction services, trauma and post-trauma counseling and in-network provider referrals. These services are provided in a variety of settings to address diverse populations and illnesses. These include participant homes, HHO health centers, the Resource Center, and on the streets and in shelters. COMMUNITY HOUSING: HHO leases more than 350 apartments across the city, providing supportive housing to participants who are homeless or on the verge of homelessness. Housing services target people who are homeless and severely mentally ill or addicted, individuals with chronic illnesses, including HIV, and people in treatment for tuberculosis.
