Dear Patients, We received numerous requests to join our new Patients Portal, along with suggestions and comments. In this letter, we will answer most common questions you asked about services covered by BLOCK FEE. In Ontario, many of health care services are covered by the Ontario Health Insurance Plan (OHIP). Some of the medical and other services provided by doctors or doctor’s offices are not covered by OHIP. College of Physicians and Surgeons of Ontario (CPSO) in conjunction with the Association of Physicians (OMA) has been introduced the recommended system of payment for non-‐insurance services. The list of services not covered by OHIP provided by our clinic can be found at the secretary desk or on the web site www.diseramedical.com (updated monthly). You may consider the BLOCK FEE in order to get these services for free or with a significant discount. What services are fully covered by BLOCK FEE? At the Patients' Portal: Requesting prescription renewal (if applicable); Requesting and downloading available Medical Forms ,for example: Massage/Physio /Orthotics Note, Sick Note/Back-‐to-‐Work Note etc.; Reviewing and downloading available laboratory report results; Reviewing and downloading available imaging reports ie US/X Rays/CT scans etc.; Reviewing and downloading available appointments’ summary notes; Communicating with your doctor via email to save time using a secure connection code; At the office: Work/School/Medical Forms; Daycare-‐medical clearance to return/note; Disability Form; Federal Disability Tax Credit (CRA); Fitness Certification/Club Form; Massage/Physio /Orthotics Note; Pre-‐Employ Certificate of Fitness; Private Insurance/Sickness/Disability; School/Camp Physical Forms; Sick Note/Back-‐to-‐Work Note;
Travel Cancellation Insurance Form; Unemployment Insurance Form; Nurse’s fee for injections not covered by OHIP i.e. all injections needed for travel or teenager/seniors immunizations (pt supplies vaccine), some vitamins, non-‐prescribed and compound medication injections; TB Testing (Mantoux), including 2 visits:1st -‐ providing test and 2nd -‐reading test and forms; Phone/Fax Prescription Renewal; Missed or Cancelled Appointments (up to 3 per year); What is the cost of BLOCK FEE? In our clinic, rather than a one-‐time payment for the entire year, you have the opportunity to pay block fee monthly. Monthly payment will be from 10 to 20 Canadian dollars, depending on the number of your family members. How can I pay BLOCK FEE? In order to be registered for the monthly payment you have to fill out the Block Fee Form and send it to our office by e-‐mail, fax or regular mail. You can also fill out the Block Fee Form directly in the clinic. Please remember, that your decisions to pay Block Fee will not affect in any way yours or your family ability to access health care services at our office, will not affect in any way the physician-‐patient relationship, or pose any barrier to accessing health care services in our office.
