From Our Executive Director

The Grand Bend Area Community Health Centre has a dynamic, dedicated staff that shares a vision of “Health and Wellbeing for all”.

As a Community Health Centre we provide a holistic interprofessional approach to all aspects of an individual’s health and well-being. There is a strong emphasis on health promotion and illness prevention.

We operate a satellite in Hensall where social work, diabetes, physiotherapy and a community dietitian work from.

We have strengthened our relationship with South Huron Hospital in Exeter by sharing staffing in several areas. We work closely with the North Lambton Community Health Centre and many other organizations to promote a holistic approach to the health and well-being needs of individuals, families and the community we serve. This collaboration enhances the delivery of care by both organizations.

Grand Bend Area Community Health Centre

The Grand Bend Area Community Health Centre is a non-profit community governed organization that provides comprehensive primary medical care for our enrolled clients. Our health promotion strategies utilize multidisciplinary teams of health providers and are available to clients as well as everyone in the community.

The Centre’s two sites work in collaboration with the Southwest and Erie St. Clair LHINs along with various agencies and services within the counties of Lambton and Huron. Our on-going partnership with South Huron Hospital Association in Exeter enables us to seek out and provide the best care for our clients / patients along with programming for the surrounding communities of Hensall, Zurich, St. Joseph, Dashwood, Grand Bend, Mount Carmel, Thedford and Port Franks.

Primary Care Team ServicesOur Primary care team consists of Family Doctors and Nurse Practitioners supported by a Physician Assistant, Registered Practical Nurses, and Medical Office Assistants. Our health care teams work in an interdisciplinary collaborative environment that includes specialists, and other health care professionals. Patients/clients at both Grand Bend and Hensall sites can access these services by appointment.

The centre provides diagnostic services such as a twice daily lab courier, ECG, Spirometry, Ambulatory Blood Pressure monitoring, Event/Looper Monitoring, and Stress Testing (all by referral and appointment). We also offer consultants such as a Chiropodist through self-referral and an Internal Medicine Specialist or an Endocrinologist through referral. (CHC patients)

We believe in the importance of education both for our staff and for upcoming medical professionals. We provide learning opportunities for students: Residents and Medical students, Nurse Practitioner and Medical Office Assistants, Dietitians, Social Workers, Physiotherapists, Occupational Therapists and Respiratory Therapists. Family PhysiciansFamily physicians are trained in all areas of medicine and can diagnose and treat a broad array of health care problems. They are at the centre of your care, coordinating and guiding you through your health care options. They are experts in prevention, treating most acute illnesses and helping people manage their chronic diseases. To ensure your best treatment options our physicians work with Allied Health services, specialists and other health care professionals. GBACHC has several family physicians and, on occasion medical resident physicians who can address your primary care needs. If you are a new patient of the centre you will be assigned to one of our primary care doctors. We ask that when possible you arrange your appointment with your usual doctor or nurse practitioner. You may choose to see either depending on the nature of your concern. If your usual doctor is not available you will be seen by another member of the team. Please inform specialists and hospitals of your doctor’s name. This will enable us to receive copies of your reports so that they may be included in your chart. If you are currently without a family physician and you live in the catchment area please refer to the Patient Application letter on our website www.gbachc.ca for instructions on how to fill out a request for a Family Physician.

Primary Care Nurse PractitionersPrimary Care Nurse Practitioners are advanced practice nurses, Registered Nurses registered with the College of Nurses. They have the independent authority to provide wellness care, monitor infant development, screen and monitor chronic disease, diagnose and treat common illnesses and injuries and prescribe most medications and lab tests. They work collaboratively with other members of the health care team.

Physician AssistantA physician assistant (PA) is a physician extender, who provides care in a range of health care settings, under the supervision of a registered physician. A PA’s practice mirrors that of the supervising physician. A PA can conduct patient interviews and take medical histories, perform physical examinations, order and interpret diagnostic tests, prescribe medications, and provide counselling on preventive health care. A PA completes a 4-year bachelor’s degree and 2 years of medical school. A PA is NOT a doctor and they cannot work independent of a doctor. However, a PA can help increase access to care and reduce wait times for patients, which are important for improving health care in Canada!

Registered Practical NurseRegistered Practical nurses work with the Physicians, Physician Assistant and Nurse Practitioners to provide patient care. They educate patients on disease and how to lead a healthy lifestyle. They can access patients, obtain your history and give valuable feedback to the team on your health. They are also able to: take your blood work, provide immunizations, administer medications and follow ordered treatments by the physician and nurse practitioner

System NavigatorIt can be sometimes difficult for people to know what services are available to them. The Grand Bend Area Community Health Centre offers a service to help clients navigate the health and social service system. The Navigator is a Registered Practical Nurse who understands the services available within the Community Health Centre and works with other partners in the community to find the proper care.

The role of the system navigator is to engage clients with complex needs with the goal to increase client self-efficacy and autonomy.

They do this by:

• Assessing and determining client needs• Developing individualized service/care plans• Case conferencing and engaging supports• Making referrals to supports

MedicalOfficeAssistantsMedical Office Assistants (M.O.A.) duties include greeting patients/clients of the centre, booking appointments and performing clerical and clinical duties for patients, health care practitioners and administration. They are your first point of contact in the centre.

Physiotherapy Physiotherapy is offered at the Grand Bend and Hensall site of GBACHC. Our physiotherapist will see you after you have used all of your extended health care benefits that you may have. This will ensure that you receive physiotherapy in a timely manner and will not delay your healing process by being placed on a waiting list.

Our physiotherapist works together with other physiotherapists in the community to ensure that you receive the right care at the right time and return to your optimum level of functioning.

Our physiotherapist works with an Occupational Therapist and Kinesiologist to ensure that you are receiving the best care for your specific needs. The team is able to work together to restore mobility and maintain your independence.

Occupational TherapyAn Occupational Therapist (OT) is a Health Care Professional who works with individuals to assist them in improving, sustaining or restoring their highest possible level of independence in their desired daily activities.

The activities that OTs helps with often fall into either:

• Self-care: bathing, dressing, eating, etc. • Productivity: work, home maintenance, banking, etc. • Leisure: sports, reading using your computer etc.

The OT’s at GBACHC currently work with the senior population to complete Home Safety Assessments and with individuals who have been referred to the physiotherapy department related to hand or wrist injuries.

A referral for an OT assessment can be made through your physician, nurse practitioner, or other health care provider, caregiver or yourself. Once the referral is received an OT would book a home visit with you to complete the assessment. The OT can then assist you with concerns such as falls prevention, home safety, assistive device use, safe return from hospital and provide education on strategies to regain or maintain independence with daily tasks. They may also address a need for more support or programs in the community that may be of benefit. At times, OT’s also complete cognitive assessments to determine which areas of your brain are functioning well and provide strategies to improve the areas that are not functioning as well.

As a result of the home assessment OT’s work with and provide referrals to other Health Care professionals as needed. Through the Home Safety Assessment, recommendations are often made for individuals to access our GBACHC Grab Bar Program, Home Maintenance or Assisted Device program for Mobility Devices.

KinesiologistA Kinesiologist is a health care professional that works towards the prevention and management of injury, disability and chronic disease, and the improvement of overall health and performance. A Kinesiologist identifies and examines how the body moves and looks at how it can function better. A Kinesiologist provides baseline testing and introduces an individualized exercise plan to assist in pain management, increase strength, and improve balance and flexibility.

Our Kinesiologist works to complete a comprehensive review of current and past medical history, as well as assess mobility, exercise status, and overall level of physical function. After baseline testing, various exercise programs are put in place such as a home exercise program or community group exercise programs. Our Kinesiologist also works in the Physiotherapy department upon referral from the Physiotherapist to improve strength, flexibility and range of motion with clients on a one-to-one basis.

Registered Respiratory TherapistA Respiratory Therapist (RT) is a healthcare professional who monitors, assesses, and treats individuals who have difficulty breathing. RTs are “regulated”, meaning that they have to become members of the College of Respiratory Therapists of Ontario in order to work in this field. The Respiratory Therapist is an essential part of the “Better Breathing Team”. The RT assists in the care and management of Chronic Obstructive Pulmonary Disease (COPD) and other lung condition such as fibrosis or asthma. The RT assesses patients, performs spirometry (lung volume) testing, makes recommendations to optimize respiratory medications, and helps to facilitate pulmonary rehabilitation classes. The Respiratory Therapist is a certified Respiratory educator and is able to teach patients about proper breathing techniques, inhaler use and how to live with your condition.

Social WorkersSocial Workers registered with the Ontario College of Social Workers and Social Service Workers. They work with other social services, such as government assistance, and legal aid and are involved in social policy development. Social workers develop, implement, and assess programs to address social issues such as domestic violence, poverty, child abuse, senior abuse, addictions and mental health and homelessness.

Our Social Workers provide:

• Individual, couple, family and group counselling • Assist people in managing their daily lives• Help individuals understand and adapt to illness, disability such as chronic

pain and death

To obtain an appointment you may speak directly to the social worker, health care professional or you may simply self-refer.

ChiropodyChiropody is the assessment of the foot and the treatment and prevention of diseases or disorders of the foot by therapeutic, orthotic and palliative means. Chiropodists specialize in the recognition, assessment, treatment, and ongoing care of the foot. As an essential member of the health care team, chiropodists have many skills and techniques that help clients eliminate foot pain, gain mobility and maintain healthy feet.

Chiropodist treat:

• Sports injuries• Common foot conditions such as: corns, warts, ingrown nails, calluses, etc.• Foot infections caused by medical conditions such as diabetes• Foot deformities and misalignments

Dennis Walsh, Chiropodist – is currently available on Thursdays between 9:00am and 4:30pm by appointment. A referral from a physician is not necessary. Chiropody services are not covered by O.H.I.P. To make an appointment please call 519.238.2362

Diabetes EducatorsA Certified Diabetes Educator (CDE) is a health care professional who is specialized and certified to teach people with diabetes how to manage their condition.

The GBACHC offers a diabetes education and management program in Grand Bend and Hensall. The program offers education on how to self-manage your diabetes and how to prevent or delay complications. Topics that are discussed in the program include: blood sugar control, nutrition, medication, activity, foot care etc. The diabetes team collaborates with your health care provider to help best manage your diabetes. One on one counselling and group programs are available.

Any adult person diagnosed with pre-diabetes, Type 1 or Type 2 diabetes is able to access the program through their health care provider or self-referral. Please contact the Administrative Assistant at 519.238.1556 x242.

Registered DietitiansDietitians have to meet national standards for education and training. They are also required to be registered with the College of Dietitians in whatever province they work. In Grand Bend and Hensall they provide individual and family counselling on a variety of food and nutrition issues such as high cholesterol, high blood pressure, bowel concerns, picky eating, unintentional weight loss, and a non-dieting approach to weight management. If you have diabetes, you will see the dietitian in the diabetes education program. Our dietitians accept referrals from health care providers and self-referrals from individuals within our catchment area. They also do group programming in the community.

Aging At Home ProgramThe Centre offers three programs under this strategy initiated through the Erie St. Clair LHIN.

1. Occupational Therapy (OT as described above) also includes the Home Maintenance (HM), Grab Bar Program, and Assistive Devices Program (ADP) where subsidy is available for fall prevention strategies.

2. Better Breathing Team3. Telemedicine Program 4. Social Work

Home Maintenance Program:This fall prevention initiative works in collaboration with Lambton Elderly Outreach (LEO) to provide affordable home support services. The Grand Bend Area Community Health Centre subsidizes the lawn care and snow removal program offered by LEO to ensure that everyone pays an affordable rate. Once the need has been determined through a Home Safety Assessment, the OT refers an individual to the Home Maintenance Program if applicable.

Grab Bar Program:This fall prevention initiative provides funding assistance to install grab bars within an individual home. Once the need has been determined through a Home Safety Assessment, the OT helps the individual to determine the type of grab bar that will work best given the individual and the environment.

The program covers the cost of the grab bar(s) purchase and installation in the home by a recommended vendor.

Assistive Devices Program:This is a Ministry of Health and Long Term Care program which provides funding to Ontario Residents who have long-term physical disabilities and require mobility devices as a result (i.e. wheeled walker, wheelchair). Our OT’s are ADP authorizers and once a Home Safety Assessment has been completed they can assist you with obtaining an appropriate mobility device if you meet the ADP criteria.

Better Breathing TeamThe Better Breathing Team provides service to individuals who have a diagnosis of Chronic Obstructive Pulmonary Disease (COPD) or other chronic lung impairments. The program is designed to assist clients to manage their care, decrease readmissions to hospital, decrease the length of client stay in hospital, decrease flare-ups, and enhance the client’s overall quality of life through pulmonary lung rehabilitation (exercise and education). The team will visit clients in their home if required and coordinate community based programs within the catchment area, team members include:

• Registered Respiratory Therapist• Physiotherapist• Kinesiologist• Social Worker

Telemedicine Program (OTN)Videoconferencing services are available in Grand Bend and Hensall locations. This is used for specialist appointments, education and meetings. There may be some instances where your healthcare provider will recommend and OTN visit.

Health PromotersHealth Promoters coordinate and facilitate group and individual programs that empower people to achieve their optimal health. They work in partnership with other agencies for the overall betterment of the community’s health. They focus on the prevention of disease but also concentrate on improving the lives of those with chronic diseases or disabilities. They engage the community in improving their overall health and wellbeing.

Grand Bend Area Community Health Centre used the Canadian Index of Wellbeing as a framework for a community situational assessment. The main goal identified from the Be Well assessment was to address health equity by reducing access barriers in three main areas: resources for daily living, health services and groups, and community activities and groups. No one should be disadvantaged from attaining his/her full health potential because of social, economic, demographic, or geographic circumstances.

GBACHC is taking actions to enable everyone to fully participate and reach their health potential. Let’s work together to take action in these priority areas. Let’s learn about what each other are doing, what works and does not work, expand successful strategies and find new ways to achieve health equity. Working together, we can make a difference! We can create large-scale, lasting change related to community wellbeing by working with various partners and coordinating our efforts.

CCAC CoordinationErie St. Clair and the Southwest Community Care Access Centres have a community nurse that attends GBACHC on a weekly basis to coordinate care with our System Navigator. For care coordination for high needs patients contact Megan Elliott (RPN) at 519.238.2362.

Exercise & Wellness ProgramsHealthy Lifestyle Exercise ProgramsThis program is available fall, winter and spring in two locations. Grand Bend Area CHC in the upstairs Community room on Mondays and Thursdays at 8:45am, Port Franks at the Port Franks Community Centre on Tuesdays and Thursdays 9:00am. Both programs provide warm up, low impact aerobic exercises done to music, followed by strength work and stretching. You must bring a clean pair of running shoes and a mat if you have one. There is no fee for this program and is facilitated by a trained volunteer. Call Cindy Maxfield, Health Promoter 519.238.1556 x231.

Gentle Exercise ProgramThis program runs Mondays and Thursdays from 10:00am to 11:00am in the Community Room at the Grand Bend Area CHC and is available fall, winter and spring. Stretching, strength and gentle aerobics utilizing chairs and the wall make this a great program for individuals that are new to exercise, or have some physical limitations. This Program is facilitated by trained volunteer instructors and is free. Clean dry running shoes and comfy clothes are all you need. Call Cindy.

Functional Fitness Exercise ProgramsThe Functional Fitness program is designed for seniors with limited mobility and is run by a registered health professional. The program is offered on Monday and Thursday mornings from 11:15-12 noon in the Community room at GBACHC. A similar program is offered in Exeter on Monday and Wednesdays from 2-3 pm.

Savannah StrollersIn partnership with the Pinery Provincial Park, Grand Bend Community Foundation, Friends of the Pinery and the Lambton Heritage Museum we offer you a choice of walking programs utilizing the Trails in the Pinery Park for outdoor walking and the Galleries in the Lambton Heritage for indoor walking. This program is available every Wednesday. Please meet in the parking lot of the Museum at 9:20am for those who wish to carpool to the Pinery Park. For those who wish to stroll inside out of the elements please enter the museum through the main entrance.

The Pinery outdoor program runs year round. The Lambton Museum indoor program runs winter to spring. There is no fee for either program. You need to dress for the weather for the outdoor walk and bring dry clean running shoes for the indoor walk. Call Cindy Maxfield, Health Promoter 519.238.1556 x231.

Line dancingIf you want to have fun and do exercise at the same time there is a Line dancing program held at the Port Franks Community Centre from 8:30am to 9:00am every Tuesday and Thursday before the exercise program start. Participants need dry clean shoes comfortable to dance in. There is no fee for this program. Call Cindy Maxfield, Health Promoter 519.238.1556 x231. Program available in the fall, winter and spring.

Community Support and Social ProgramsAlzheimer’s Caregiver SupportThis program is facilitated by the Alzheimer’s Society of Huron and is offered the first Monday of every month at the Grand Bend Area CHC at 1:30pm and 7:00pm. If you are caring for a loved one with Alzheimer’s you also need to take care of yourself. Join others in this supportive environment. There is no fee. The number to call is 1.800.561.5012. Canadian National Institute for the Blind Low Vision ClinicA free drop in clinic from 10am to 2:00pm the second Thursday of every second month, February until December at GBACHC in the Adult Day Wing. Parkinson’s Support GroupThis support group was created in response to an identified need in our community for individuals and their caregivers who live with Parkinson’s disease. The group has been established in partnership with the Parkinson’s Society – London chapter. The aim of this group is to enhance member’s knowledge on specific topics related to Parkinson’s disease and provide them with opportunities to dialogue with each other, ask questions to occasional guest speakers and share resources. Our hope is to create a network in our community where individuals will feel supported and encouraged. This group aims to facilitate personal growth, empowerment and increased knowledge and understanding of the disease. Chronic Disease ProgramsChronic Pain Management and Support GroupThis 8 week program at the GBACHC incorporates gentle exercise, education sessions and support. It deals with topics such as what is chronic pain, managing medications, relaxation techniques, better sleep and much more. Pre Diabetes ClassesThese classes are offered at GBACHC two to three times a year with topics that include healthy eating, physical activity and stress management. Smoking CessationHow can STOP help you or someone you know quit smoking? The STOP smoking cessation program offers counselling support and Nicotine Replacement Therapy (NRT – patch, gum, inhaler, and lozenge) at no cost to patients/clients who want to quit smoking. STOP is funded by the Government of Ontario and is part of the Smoke-Free Ontario Strategy. Choices for Change, Addiction Counselling ServicesThis Program provides assessment and treatment for people with substance abuse and problem gambling issues. Being MindfulJoin to learn and practice different mindfulness techniques and coping strategies to reduce stress, gain acceptance and find happiness. Living a Healthy LifeLiving a Healthy Life with Chronic Conditions is a licensed program from Stanford University and has been adopted by the Southwest Self-Management program. This is a free 6 week program that provides information and teaches practical skills to manage ongoing (chronic) health conditions. Participants learn to improve self-management skills and tailor them to their own needs and lifestyle. In addition to learning about healthy living, participants develop skills in areas such a goal setting, problem solving and communications. Caregivers are welcome to attend the workshop to learn self-management skills themselves and how they can provide support their family members. Opening DoorsGBACHC supported West Lambton’s application to the United Way to expand this program. It is a free physical activity and healthy eating program for adults living with mental illness.

Community Gardens Community gardens involve a group of people who come together to garden. People may decide to grow vegetables, fruit, flower and/or herbs. Would you like to join or would you like to start a community garden?

Blessings Community Store & Food BankLocated at 45 Main St Zurich. Blessings have furniture, clothing, toys and books at reasonable prices as well as offers food bank services. Hours are Monday to Friday from 10-4 pm and Saturdays from 10:00am to 3:00pm. Call 519.236.4376.

Nutrition Education Workshops – FREE!Craving Change: Why it’s Hard to Change and How to Change your Eating (several sessions)

We Can Help Fund

The Grand Bend Area Community Health Centre’s “We Can Help Fund” was established to provide an urgent one-time financial gift to benefit members of our community.

All referrals for the “We Can Help Fund” can be directed to the Social Workers at the Grand Bend Area Community Health Centre.

Any individual in the community who is facing financial hardship is encouraged to apply for assistance. Past “gifts” have included gift cards for local grocery stores or gas stations.

Grand Bend Area Health Services Foundation

As the fundraising arm of the Grand Bend Area Community Health Centre, the Grand Bend Area Health Services Foundation receives and distributes financial resources necessary to meet the capital requirements of the Health Centre, and supports health care services available in the community. Created in 2000, the Foundation was registered by the Canada Revenue Agency as a “Non Profit Corporation” in 2001, receiving Charitable Registration (#88673 8418 RR0001).

Government grants cannot meet all of the community expectations for health care services, especially in rural areas. In order to enhance existing services and enable community needs to be met in the future, private contributions and donations are often needed, sometimes on short notice.

When the Grand Bend Area Community Health Centre was established, it was understood that the Ministry of Health and Long-Term Care would not fully fund the capital requirements of the new facility. It was therefore decided to follow the example of most hospitals, health agencies, and other community service agencies, and establish a Foundation to be responsible for fundraising activities.

The current site for the Health Centre was generously donated by Rice Development. A very successful campaign by the Foundation raised substantial funds for items and equipment not funded by the government. The new Centre, officially opened in May 2004, would have been impossible without this considerable community support. Later, in December 2008, through a significant donation from the Foundation, the Centre was able to purchase .75 acres immediately north of the current facility to allow for needed future expansion. Further donations enabled the installation of Electronic Medical Records in 2012. In 2014, a sound system for the hearing impaired was purchased by us for the Adult Day Centre. In 2015 we contributed 25,000 towards centre improvements.

There are a number of ways by which you can support the Grand Bend Area Community Health Centre through the Foundation, and make a difference. Donations can be directed to a program or project of your choice. You can pledge a contribution over a period of time; make a special occasion gift in recognition of a person or life event; arrange a “planned gift” through a bequest in your will or make a memorial gift recognized by a plaque on the Memorial Wall which will designate the person’s name in perpetuity.
