Certificates: Birth, Death or MarriageThe Office of the Registrar General provides copies of birth, death, and marriage certificates which occurred in the Province of Ontario. If you were born or married in another province or country, you must write to the proper authorities there. There is a cost attached to the issuance of certificates.

If you are applying for a social insurance number, passport or for a pension, you need your birth certificate for identification. Certain vacation spots do not require passports but do require a birth certificate—please ensure you check with your travel agent when making your arrangements.

You can apply to any M.P.P.’s constituency office for certificates, but you must allow a minimum of 6 to 8 weeks for receipt.

Ombudsman of OntarioThe Ombudsman of Ontario is an officer of the Provincial Legislative Assembly, independent from government and political parties. The Ombudsman investigates complaints about services provided by the Government of Ontario and its organizations. He also conducts systemic investigations on important issues about government services, which can affect large numbers of people. The Ombudsman cannot deal with matters regarding federal or municipal governments (except in the case of closed meetings), courts or judges. The Ombudsman is a place of last resort when other approaches to resolve complaints have been tried. The Ombudsman represents neither the government organization or the person complaining. Services are free and confidential. Complaints may be made in writing, by telephone, in person (appointment is recommended) or by fax. No fees. Hours: Monday to Friday, 9 a.m. to 4:30 p.m.

Eligibility for the Old Age Security pension and the Guaranteed Income SupplementThe Government of Canada proposes to gradually increase the age of eligibility for the OAS pension and the Guaranteed Income Supplement (GIS) between the years 2023 and 2029, from 65 to 67. People currently receiving OAS benefits will not be affected by the proposed changes.The Government of Canada also proposes to gradually increase the ages at which the Allowance and the Allowance for the Survivor are provided, from 60-64 today to 62-66.

Guaranteed Income Supplement (GIS)People getting the Old Age Security pension who need financial help may be eligible for the Guaranteed Income Supplements (GIS). To find out if you are entitled for GIS you must fill out a GIS application form. An application for GIS is mailed, at your request, once your Old Age Security Pension is approved for payment.

Voluntary deferral of the Old Age Security pensionThe Government of Canada also proposes a voluntary deferral of the Old Age Security (OAS) pension that will give people the option to defer take-up of their OAS pension by up to five years past the age of eligibility, and subsequently receive a higher, actuarially adjusted pension.

The Allowance BenefitsA 60 to 64-year-old spouse or common-law partner of a person who is getting OAS may be able to get Allowance Benefits. The spouse or common-law partner must meet residence requirements and the couple’s income must be within set limits. You must fill out an application each year if you still need Allowance Benefits, otherwise payment will be discontinued.

Allowance for SurvivorsFinancial help is offered to the legal or common-law spouse; the survivor must be 60 to 64 years old and meet both residence and income rules.

Canada Pension Plan (CPP)The Canada Pension Plan offers retirement pensions, surviving spouse benefits and disability benefits. If you are at least 60 years of age, you

may be eligible for full or partial CPP retirement benefits, provided you have contributed to the plan.

A person who has turned 60 years of age may choose to begin receiving CPP benefits at that age. The CPP adjusts the amount of your pension by 0.5% for each month before or after your 65th birthday from the time you begin to receive your pension. The adjustment is permanent. This means that if you choose to start your pension early, the payment does not increase when you reach 65. For example, if you start your pension at 60, your monthly payment is 30% lower than if you wait until you’re 65. However, by starting it sooner, you will likely receive it for a longer time. If you start your pension at 70, your monthly payment is 30% higher than if you had taken it at 65. You may be able to share your Canada Pension Plan benefits with your spouse or common-law partner. You must apply to get these benefits.

PassportsPassports are valid for 5 years now. Effective July 1, 2013, a new 10-year ePassport was introduced with new fees for 5 and 10-year passports. Persons born in Canada must submit an original birth certificate. Those born outside of Canada must submit an original certificate of citizenship. Unlike the General Application Process, the Simplified Renewal Application Process does not require applicants to submit documentary evidence of citizenship (birth certificate, citizenship card, etc.) or provide supporting identification. The application form is shorter and no guarantor declaration is necessary.The Government of Canada continues to recommend that travellers carry a valid Canadian passport for all visits abroad, including the United States. A passport is the only universally accepted identification document and it proves that you have a right to return to Canada. Canadians are encouraged to plan ahead to avoid peak periods. Community Care Access CentreYour Community Care Access Centre (CCAC) connects you with the care you need at home and in your communnity by offering information, referral and access to services to help you maintain health, independence and quality of life. Services are available for people of all ages.

Services include:Information and ReferralCase Management and care coordination, including such services as:

• Nursing • Nutritional Counselling• Personal Care/Support• Physiotherapy• Occupational Therapy• Respite Care• Speech Language Pathology• Social Work• Long-Term Care Home Placement• Care Connector service - connecting people with family physicians

In-Home Services:• All residents of Lanark, Leeds and Grenville

with a valid Ontario Health Card may access in-home health services

• Referrals can be made by individuals, families, friends, physicians, hospitals, long-term care homes, and community agencies.

Case ManagementThe Care Coordinator works closely with individuals, their caregivers and service providers to develop an ongoing care plan which may include a range of services and resources.

Living WillsA Living Will is a document that states your wishes regarding consent to or refusal of specific treatment. A Living Will is different from a Last Will and Testament because a Living Will addresses your health care, whereas a Last Will and Testament deals with matters of property. Both documents are essential for every adult.The Living Will allows you to direct your family, physician, and others about your health care choices, based on your own values and beliefs. A completed Living Will should be very specific as to what type of health care choices you wish in end of life situations. An example of what a Living Will addresses includes whether the person wishes to be hooked to a ventilator or respirator. Community Health Centres~ a place where everyone matters!

At the 73 Community Health Centre’s across Ontario, the health and well-being of every individual, every family, every community matters! In our area, four Community Health Centres (CHCs) work closely together to provide a full range of primary health care and health promotion programs and services. They are: Country Roads Community Health Centre (in Portland); Merrickville District Community Health Centre (in Merrickville); the North Lanark Community Health Centre (located in Lanark Village); and the Smiths Falls Community Health Centre in Smiths Falls.

Community Health Centres are an effective and unique model of primary health care where an inter-disciplinary team of health care professionals work together to respond to people’s individual health concerns. At all CHC’s in our area, this team includes: doctors, nurse practitioners, nurses, dietitians, social workers, chiropodists, respiratory therapists, health promoters and community developers.

In addition to primary care services, CHC’s also offer a wide range of free health promotion, education and prevention programs to address the broader health needs of the people and the communities they serve. In most cases, you do not have to be a client of the CHC in order to access these programs. While each CHC has their own specific group programs based on the unique needs of their community, they often include groups where people: learn how to manage their diabetes, asthma and other chronic conditions; get support to quit smoking; participate in community-based exercise classes, walking groups or yoga classes; learn relaxation, meditation and other techniques for stress reduction; and learn how to cook and purchase healthy foods on a budget.

Day Hospital is a coordinated outpatient service which provides assessment, treatment, education and support to adults who have suffered illness or injury. The purpose of Day Hospital is to improve the client’s general condition, health and independence. Clients receive therapy from

Nurses, Physiotherapists, Occupational Therapists and Speech and Language Pathologists. A team of health care professionals work with people who have complicated health care needs. The composition of the team depends on the type of rehabilitation required.

The rehabilitation programs are doctor referred and include both neurological and pulmonary. The Pulmonary Rehabilitation Program is an educational and exercise program for people living with COPD.

Alzheimer Disease & Related DementiasAlzheimer’s Disease is a degenerative brain disorder that destroys vital brain cells. There is no cure at this time however there are medications and treatments that can slow the progression down. Dementia can affect people as young as 40 years of age. The risk factor for dementia doubles every five years after the age of 65. There are 747,000 Canadians living with a dementia. It is estimated that in the next 20 years there will be close to 1.4 million cases of Alzheimer’s disease or a related dementia in Canada. Is it Alzheimer’s Disease? 10 warning signs: 1. Memory loss that affects day-to-day function 2. Difficulty performing familiar tasks 3. Problems with language 4. Disorientation of time and place 5. Poor or decreased judgment 6. Problems with abstract thinking 7. Misplacing things 8. Changes in mood or behaviour 9. Changes in personality 10. Loss of initiative

There are 2 local chapters of the Alzheimer Society in Lanark, Leeds and Grenville, each providing a variety of services for persons with Alzheimer’s disease and their caregivers. Alzheimer Society of Lanark County and Alzheimer Society of Leeds-Grenville.

Support & Education is available to individuals who have been diagnosed with Alzheimer’s Disease or a related dementia, their family members and caregivers. We can offer this by phone, email or in person.Support groups are provided in the local communities of Lanark, Leeds and Grenville. These groups include: support for Women, Caregivers, Spouses, Children with Parents Affected, Early Stage support and a Men’s Cooking group. These series are also for the ones that care for people with the disease and health care professionals. It covers a wide variety of topics such as What is Dementia, Coping Strategies, Legal and Financial Issues, Long Term Care and Home Care. We also offer Workshops, Forums and Expo’s that cover all topics related to Alzheimer’s disease and a related dementia as well as promoting a healthy lifestyle with our Heads Up for a Healthier Brian.

Foot CareOlder people often find it difficult to reach their feet and therefore have trouble caring for them. Simple procedures such as nail cutting can usually be performed by caregivers. Elders with diabetes or circulation problems, however, should have a podiatrist or chiropodist attend to their complete foot care.

Most Home Support Programs and Para-Med Health Services sponsor weekly footcare clinics. For clients who cannot attend the clinics, agencies can arrange in-home footcare.

To Keep Feet Healthy• After washing, dry feet carefully especially

between the toes. Then generously use a fine talcum powder.

• To prevent ingrown toe nails, trim nails flat across the top.

• Exercise feet when sitting by rotating them at the ankles and by alternately stretching and contracting the toes.

• Use a foot bath with vibrating pads. Then massage the feet using a special foot lotion.

Geriatric ProgramsSpecialized Geriatrics (SG)(formerly Southeastern Regional Geriatric Program (RGP)Comprehensive, short-term assessment and treatment/rehabilitation for seniors (65+) with complex health needs who are likely to benefit from a multi-disciplinary approach. Services include in-home (including long term care) outreach and community hospital assessment, specialized clinics, a Day Hospital, a Geriatric Rehabilitation In-Patient Unit, and acute care consultation at Kingston General Hospital. All SG clinical services are provided in association with each client’s family physician and covered by OHIP. Referrals accepted from family doctors, other health care professionals, seniors themselves, family members and/or other informal caregivers. Some flags for referral: recent decline in function, more difficulty getting around, recent falls, concern about medications, recent change in ability to cope, several recent admissions to hospital or visits to emergency or family physician, unexplained change in memory or cognition. The Canadian Hearing Society (CHS) is helping people stay connected.

The charitable agency offers services and products to individuals with varying degrees of hearing loss.

The Brockville CHS office located at 68 William St. (Suite 205) in Brockville covers all of Leeds, Grenville and Lanark counties. Residents of South Dundas are directed to the CHS office in Ottawa.

The program assistant is usually the first person with whom visitors and callers interact. They provide information on programs that CHS provides as well as on communication devices that are available. In many cases, clients are referred to a hearing care counsellor for follow-up.

Individual counselling sessions can take place in the CHS office or in the client’s home; these services are offered free of charge. The society does not offer hearing tests but can provide a list of audiologists and service providers.

The society’s mandate is to provide unbiased information to enable individuals and their families to make informed choices.

The Hearing Care Counselling Program assists older adults by educating clients and providing them with options to help manage difficult listening situations. Home visits can be conducted, and communication device demonstrations and recommendations are provided. Devices include visual smoke alarms and amplified phones; the society wants to ensure they are the right item for the client and will improve quality of life in the home.

CHS holds hearing care clinics at the Brockville office four times a year. There are also clinics at locations in Lanark County. Hearing tests are not conducted at the clinics; visitors can ask questions about cleaning their hearing aids or changing the batteries as well as other aspects of dealing with hearing loss.

CHS to individuals between the ages of 16 and 54. Counsellors attend the Brockville office on a regular basis (usually once every two weeks).

Statistics indicate it takes most people between six and 10 years to notice a hearing deficiency and do something about it.

CHS is also available to make presentations to community groups and organizations.

The local CHS office is open on Tuesdays, Wednesdays and Thursdays from 9 a.m. to 5 p.m. (closed for lunch between 12:30 and 1:30 p.m.).

The Geriatric Mental Health Community Team is a multi-disciplinary team of Psychiatrists, registered nurses, social workers, occupational therapists and psychologists working with family physicians to diagnose and assist with the treatment of depression, dementias (Alzheimer’s, Vascular, etc.) and other mental illnesses challenging seniors today. Service is provided in the patient’s home, homes or in hospital.

The Mental Health Crisis Teams are health professionals such as registered nurses, social workers, physicians and psychiatrists who help to provide compassionate care, ensuring the privacy, dignity and self-respect of the person, family and others. The Crisis teams work in collaboration with the Inpatient, Outpatient teams at Elmgrove and the hospitals of Lanark, Leeds and Grenville.

Services Include: Toll free crisis telephone line 24 hours a day, 7 days a week; Timely initial screening to determine urgency of client and family needs; Access to comprehensive, confidential assessment; Preventative short-term crisis intervention, stabilization of acute phase of illness and support; Shared mental health care service provision in collaboration with family physicians and other community partners; Mobile outreach in partnership with community agencies; Help to facilitate an admission if required; Community consultation, education and training, (ASIST, NCE, etc.); access to the Psychosocial Response Teams; and Information, liaison, advocacy, referral and follow up.

Our priority is persons with severe mental illness and/or mental health problems but our agency will provide a point of access for all adults across a continuum of mental health needs including persons who are experiencing acute situational distress.

Palliative Care

Brockville & District HospicePallative Care Service The goal of the palliative care service is to provide the highest quality of life possible for individuals living with a life-limiting illness. The Hospice Palliative Care Team includes professionals and volunteers who have extensive training in palliative care. The team provides assessment consultations, counseling, advice in pain and symptom management, as well as physical, emotional and spiritual support.The program also provides volunteer visiting and a Day Hospice Program offered on a weekly basis that provides social interaction, therapies, activities and nursing consultations for individuals. Bereavement Support is offered through group or individual counseling for those who have experienced the death of a loved one.The Hospice Palliative Care service offers support at home, in hospital or in a long-term care or retirement facility in Brockville and surrounding area. There is also a six-bed Palliative Care Unit at the Brockville General Hospital Garden Street Site.The service accepts self-referral or referral from a health care professional. In Canada, tens of thousands of cats and dogs, many of them healthy, are put to death annually in pounds and shelters, for lack of homes for them. More die on the streets from sickness, injury and starvation. To help diminish the numbers of these unfortunate animals our FUND FOR SPAYING/NEUTERING of cats and dogs provides for this purpose limited financial assistance to animal rescuers, caretakers of feral cat colonies, and individuals with limited income.

By donating to our non-animal-using ALTERNATIVE RESEARCH FUND (ARF) you can reduce the suffering of thousands of animals in biomedical laboratories while supporting first rate biomedical research. ARF promotes excellent biomedical work by relying on ethical clinical research with humans, human tissue and organ culture, epidemiology, simulators, computer science, and more.

Several ARF grants have been issued to date, to researchers and schools at Health Canada’s Environmental Health Centre, Laval University, University of Saskatchewan, and most recently to the Advanced Trauma Life Support Program at Ottawa Hospital and University of Ottawa Skills and Simulation Centre.

As people age, their dentures don’t change, but their mouths do. If you have dentures, they should fit and you should be wearing them. Proper care and regular dental visits are important factors for keeping your mouth healthy and your dentures in good shape.

Problems from Poor Fitting DenturesWith age, the gum ridges in our mouths can shrink, causing dentures to become loose. Bone can also shrink, causing jaws not to line up properly. Loose dentures can cause sore spots in your mouth as well as stomach problems from not being able to chew food properly. A loose denture could also cause changes in your facial features.

When do Dentures Need to be Replaced?If your dentures are in a drawer because they just “don’t feel right”, they are loose or make sore spots in your mouth, you should see a Denturist to have them evaluated and possibly adjusted, relined or remade. If the teeth in your dentures are considerably worn out, talk to your Denturist about having some new ones made. It is also important to keep regular check up appointments to make sure your teeth are fitting properly as well as to check your mouth tissues for any changes. Your Denturist can tell you how often you need to come in for check-ups.

Caring for your DenturesProper care can keep your dentures and your mouth in good shape. You should brush your dentures every day with a soft bristled toothbrush to remove food and plaque. The same goes for your gums, and tongue. When you are not wearing your dentures, they should always be kept in water (not hot) or in a denture solution, recommended by your Denturist.

Don’t forget to Smile!With proper care and regular dental visits, your dentures should fit good and stay in good shape for a long time. Your Denturist will let you know when they need to be replaced. In the meantime, take care of your dentures, visit your Denturist regularly and get out there and smile.

FEATURE | Legacy gift to CHEO generates $4.6 millionWeldon Cochrane was an Ottawa chartered accountant and partner with the accounting fi rm of Coopers and Lybrand (now PriceWaterhouseCoopers) and was also the Treasurer on the CHEO Foundation Board. Mr. Cochrane had an appreciation for the importance of leaving a legacy and understood how to make an impact when he decided to make a gift in his Will to CHEO. He left the residual of his estate with instructions for it to be endowed; meaning that the capital would be preserved in perpetuity and the annual interest would be used to fund the important work at CHEO.

When Mr. Cochrane died in 1985 the CHEO Foundation received $540,000 from his estate and established the Weldon Cochrane Endowment Fund as directed in his Will. Much has happened and changed at CHEO since his death and in that time his legacy gift has grown to $4.6 million. Moving forward 80 percent of the interest generated from this fund will be invested in medical equipment, research and hospital programs at CHEO. The remaining 20 percent will be reinvested to allow the fund to continue to grow. In 25 years $6.4 million (80%) will go to the hospital and $1.6 million (20%) will be reinvested bringing the total value of the fund to $6.2 million. In 50 years $15 million (80%) will go to the hospital and $3.8 million (20%) will be reinvested bringing the total value of the fund to $8.4 million. In 100 years $42.2 million (80%) will go to the hospital and $10.6 million (20%) will be reinvested bringing the total value of the fund to $15.2 million.

The impact of Mr. Cochrane’s generous gift is probably beyond what he could have even imagined when he made it over 30 years ago. His legacy lives on and continues to make a difference in the lives of young patients at CHEO today, and for future generations.

As CHEO marked its 40th anniversary last year, we look back and honour not only those in our community who made our local children’s hospital a reality, but also donors like Mr. Cochrane who made children a priority. Community Home Support Lanark County is a non-prof-it, charitable organization for seniors, adults with physical disabilities and persons who have been diagnosed with a life-limiting illness and/or palliative diagnosis.

We strive to provide services that help our clients continue to reside in their own homes and remain a part of their community.

Volunteers are the heart and soul of our agency. Without them our services would not exist. New volunteers are always needed. If you can help, please call the offi ce near-est you.

Services offered:• Transportation to medical appointments – Rides to out-of-town medical appointments or essen-tial services are provided by volunteer drivers using their own vehicles. Escorted transportation for local appointments is also available for those who qualify. A nominal cost applies.

• Meals On Wheels – Volunteers deliver nutritious and delicious meals right to your home. Most special diets can be accommodated. The service is available up to 7 times per week and can be delivered hot, chilled or frozen. A nominal cost applies. The deliv-ery of the meal provides social contact and a safety check for the client.

• Diners Club – Enjoy a delicious and nutritious noon meal and a short social or educational pro-gram. A great way to meet old and new friends. Rides are avail-able.

• Foot care – A qualifi ed nurse provides professional foot care. Services include the trimming of toenails, corns, etc. and a general health assessment. This service is available by appointment only. A nominal cost applies.

• Friendly visiting – A volunteer regularly visits clients in their homes, providing friendship and support. These visits may include reading to the sight impaired or going for a walk. No charge.

• Telephone reassurance – A regular telephone call provides friendship and reassurance to

persons who are housebound or isolated. This may be a short or long-term arrangement. No charge.

• Home help/home maintenance – This service is a brokerage arrangement offering assistance with housekeeping, homemaking chores, grounds-keeping and home maintenance. The services providers are screened by the agency and the service provider and client determine the fee.

• Client intervention and assistance – A support service providing assistance in critical situations to vulnerable and at risk seniors and adults with physical disabilities and/or their caregivers. No fee.

• Hospice-Palliative Care service – Trained hospice volunteers give people with a life-limiting illness and/or palliative diagnosis the chance to be cared for at home, by the people they love. We help people live out their lives in comfort and in dignity. We also give support to people who are grieving. Anyone can refer to this service. No fee.

FEATURE | Managing Your Money Reverse mortgage, home equity line or downsizing?

Home is where the heart is and a lot of your money, too. The kids are gone, retirement is on the horizon, or for a number of other personal reasons, you may be at a point in your life where you’re looking to unlock the equity you’ve built in your home over the years. Let’s look at three popular strategies for making that happen.

Reverse mortgage This is a mortgage secured by the equity in your home but unlike a regular mortgage you do not have to make principle or interest payments – a reverse mortgage pays you. Generally available only to those over 55 years of age, a reverse mortgage allows you to receive up to 50 per cent of the value of your home and you can choose how you want to receive the money, either as a lump sum, in monthly payments, or a combination of the two. The money is tax-free, you keep ownership of all and remaining equity in, your home but the full amount of the reverse mortgage comes due when you sell your home or move out.

Home equity line of credit (HELOC)This is a relatively new cash management option that allows you to combine your mortgage, loans, line of credit, and chequing and savings accounts into a single line of credit based on the equity in your home. With a HELOC, you can access up to 80 per cent of the value of your home (reverse mortgages are capped at 50 per cent) and take any amount of money (up to your approved limit) on your own schedule. Many fi nancial institutions offer this HELOC option: You deposit money in your HELOC account (your paycheck, for example) and take money when you need it to cover day-to-day expenses or for any other reason. Any unused money from your

deposits is applied to your loan principal, reducing the loan balance and minimizing interest charges.

DownsizingWith the kids gone, your home may seem too big. You can replace it with a smaller one at a lower price, move to a cottage (if you have one) or by renting and using the difference to augment your income, invest for regular returns, or pay down debt.

There are pros and cons to each of these equity-liberating options. Ultimately, choosing the right one for you will depend on your fi nancial situation and the lifestyle you want now and in retirement. CCAC delivers care in the communityCommunity Care Access Centres (CCACs) get you the care you need in your home and in the community. This care is focused on optimizing your health and independence; supporting you through a health-care transition (for example, discharge from hospital); or providing care following an illness or compromised health status. We are a caring team of health professionals who can provide support in your home, at school, or in the community. How do I know what services I need?Once you are referred to the CCAC and become a patient, you will be assigned a Care Coordinator. She or he will provide an assessment and help determine what services and supports you may need.

What is a Care Coordinator?CCAC Care Coordinators are regulated health professionals, with expertise in nursing, social work, occupational therapy, physiotherapy or speech therapy. Care Coordinators are connected to every part of the health-care system and can serve as your single-point of contact in obtaining services and information. Care Coordinators use their professional health knowledge and assessment skills to understand your individual needs, making recommendations based on your needs and goals. We work hand in hand with you and your family to fi nd out what care you need, and then work with you to develop a care plan to supplement your care that is right for you – whether it’s nursing care, meal delivery, a day program, or help fi nding a family doctor. How do I get care?Community care can take many forms – from nursing care to personal support. In order to determine what services are right for you, the CCAC will set up an assessment with a Care Coordinator.

Nursing services are provided in

clinics as well as in the home. In some cases, your treatment may begin at home and as your condition improves you may become eligible to attend the clinic.

Clinics were designed to allow patients the opportunity to live their lives more independently by allowing them the fl exibility to make an appointment that fi ts their schedule. These are not walk-in clinics and all appointments are pre-scheduled by the clinic. Only clients that are admitted to CCAC can attend a CCAC nursing clinic.

Clinics are open 7 days/week and offer a variety of appointment times. CCAC Nursing Clinics are conveniently located throughout the area to minimize required travel time.

Effective use of clinics ensures nurses are able to see the growing number of those homebound individuals who need care at home at their most vulnerable time.

The South East CCAC has run fi ve clinics since 2009. In a survey of more than 900 patients, 98 per cent indicated that they were satisfi ed with the care they received in the clinic setting.
