PROCEDURE FOR THE SELECTION OF LABOUR ONLY CONTRACTORS, SUB-CONTRACTORS, CONSULTANTS AND SUPPLIERS. Definition Competence is viewed not merely as a matter of technical qualifications or training but the commitment to and the ownership of health, safety, welfare and related matters. Further, it prompts the wider abilities which can be brought to bear on construction, installation and maintenance activities, including the knowledge and experience of health, safety and the environment, the corresponding risks, management and control of the risks posed and identification of workable and effective precautions. Resources, include consideration of factors such as personnel with the appropriate SKATE (skill, knowledge, ability, training, and experience) to carry out and complete the required duties, the appropriate equipment, technical facilities, finances and of course adequate time. Assessment The CDM Regulations 2015 regulations require those carrying out the roles of Client, Principal Designer, Designer , Principal Contractor and Contractors to be competent, but in addition that they should assess the competence and resources of any parties they themselves appoint. The actual assessment process may be governed by the risks involved in the nature of the work being carried out. The assessment must be proportionate, appropriate and documented. The Company act in the capacity of Contractor for the purposes of the CDM Regulations 2015 and will ensure that they implement procedures to assess and verify the competence of any self employed labour only contractors or any other sub contractor or consultant that we engage to carry out work on our behalf.



The following identifies areas that Lias Industrial may consider as part of the assessment process: � commitment and understanding of duties (legal or otherwise) with respect to

health, safety, welfare and the environment. � past history / track record on comparable activities including relevant specialist

expertise and technical facilities and knowledge. � process and procedures to carry out assessment of risk and safe methods of

working including their management, monitoring and control. � knowledge of health, safety and environmental matters and the facilities for

management, communication and integration into the organisation including the continuing development and update of information.

� health, safety and environmental initiatives and the understanding about incidents,

accidents and ill health which arise from construction, installation, maintenance activities and the workplace and safety performance records including the investigation of accident / incidents.

� standard operating procedures, legislative compliance, management systems and

good working practices with respect to health, safety and the environment. � performance monitoring, reviews and audits. � Financial resources. � Recommendations from other businesses. � Request from our Clients to use one of their preferred suppliers. The Company has focused these aspects into a pre-qualification questionnaire, which is to be the adopted standard format for the selection of subcontractors, consultants and suppliers of goods and services for construction, installation, maintenance and servicing operations. Sub-contractors who carry out construction, installation, servicing and maintenance services will be selected via an agreed process. This selection process will ensure that all sub-contractors working under the control of The Company are competent and have made adequate provision for health, safety, quality and environmental requirements. Self employed “labour only” contractors will be required to complete our Labour Only Competence Questionnaire. They will not require a full assessment where they have demonstrated capability to prepare and understand risk assessments and method statements and hold the relevant competence for the tasks they will undertake.



Goods or services not related to construction or safety critical services When selecting a supplier that will not be involved in construction, installation, servicing, testing or maintenance operations or whose services are not safety critical then a simpler approach will be adopted but there will be no requirement to complete our questionnaire if they meet pre determined criteria.

Suppliers will be considered if they can fulfill two or more of the following conditions:

� Accredited to OHSAS 18001

� Accredited to ISO 9001

� Accredited to ISO 14001

� Accredited to CHAS, SMAS, SAFEcontractor, Acclaim, Altius or similar safety schemes

� Sole supplier of the goods or services

� That the price(s) quoted are competitive, based on the fact that the goods/service can be obtained from more than one source

� That the goods or services are to be supplied to the current standard and meet all relevant criteria including where appropriate the Essential Health and Safety Requirements for Machinery Safety

� The supplier is a member of a relevant organisation or holds company accreditation for the type of service being offered

� The supplier is insured to carry out the work and the persons carrying out the work hold the relevant competence and it is current

� That all certificates of conformity, declarations of incorporation, statutory examination/inspection records, calibration certificates, operation & maintenance manuals, technical files, waste transfer notes (as applicable) are to be supplied with the goods/services or on completion of the work

� The supplier can provide reliable references of similar types of work

� They can complete the work or services in the required timescales



Selection of self employed contractors or consultants Where Lias Industrial engages a self employed (labour only) contractor to carry out construction, maintenance, installation, servicing, testing or other services a short assessment form will be used to ensure that the individual has the appropriate SKATE (skill, knowledge, ability, training, and experience) to carry out the required duties. Checks will be made to ensure that relevant competencies and accreditations are valid and if appropriate insurances are in place. There will be no particular requirement for our “labour only” self employed contractors to hold insurances if they can be covered under our own employers and public liability insurance. Where time permits references will be obtained from previous employers or clients although these may be sought verbally. Selection of suppliers or construction, installation, testing and maintenance services for site works. Where a supplier, consultant or sub-contractor is to provide site installation, construction, testing or maintenance services they will only be accepted on satisfactory completion of our Pre-Qualification Questionnaire and provided they meet our assessment criteria. All new suppliers / sub contractors / consultants will initially receive Conditional Approval if they meet our assessment criteria. Assessment will be formally recorded on an assessment form. Full Approval and inclusion on our Approved Supplier List will only be granted following feedback from a Manager, Engineer or Foreman about the supplier performance. Final approval will be authorised by the Managing Director. Sub contractors, suppliers and consultants who do not meet the required assessment standards will not be approved to work on behalf of Lias Industrial. Monitoring Performance Once a supplier, sub contractor or consultant is approved their performance will be monitored and will be reviewed as and when required for continuation on the Approved Supplier List. Monitoring will be completed by site Inspections and audits and feedback from a Manager, Engineer or Foreman. A periodic submission of our pre-qualification questionnaire may be required. This will generally be risked based.



SELF EMPLOYED CONTRACTOR (LABOUR ONLY QUESTIONNAIRE)



PRE QUALIFICATION QUESTIONNAIRE ASSESSMENT This section is to be completed by Lias Industrial upon receipt of the returned questionnaire and supporting documentation to determine the sub contractor / consultant / suppliers grading and suitability for supplying goods or services. It may be that in certain circumstances a desk top assessment exercise is insufficient and an audit of the supplier at their premises or a work site may be required.



SUPPLIER APPROVAL This section is to be completed by Lias Industrial to initially record a buyer’s review or for a buyer to request a review by the Health and Safety Advisor. All suppliers/sub contractors are to be graded as “not approved” or “conditionally approved”. Sub Contractors must be monitored on site and Suppliers deliveries monitored and undergo a performance review before being approved for inclusion on the approved suppliers list.



SUPPLIER MONITORING Any new supplier / sub contractor / consultant will be monitored to ensure that they provide acceptable goods or services. A review will be carried out and recorded on the following performance record sheet.
