Click Accept at the bottom of the page.
Non-Exclusive Distribution License is for the Scholars’ Mine.This form is required.
Click box and then Save & Continue.
Fill out all contact information
Either check box for Permanent Address, or fill inpermanent address information. Then clickSave & Continue.
Fill out Dissertation/Thesis Details.
When completed, click Save & Continue.
You may want to copy and paste yourabstract in the abstract box. This will helpto ensure the abstract in the box and in the manuscript match.
Upload a PDF of your thesis or dissertation. Fill out the Copyright permission section and then Save & Continue.

Insert any notes to your Administratorin the box at the left. Notes are optional.
Copyright registration is optional. If there is a hold on your work, you are strongly discouraged from using this option.
As mentioned in the instructions, regardless of when your manuscript is submitted, it will not bedelivered to ProQuest until 6 to 8 weeks after the actual commencement date for that semester.Any processing time required by ProQuest for copies ordered will begin after the manuscript has been delivered. This means it could take several months for your ordered to be fulfilled. Your credit card will be charged immediately, regardless of how long it takes to fulfill your order.
Ordering copies is optional. If there is a hold onyour work, you are strongly discouraged from usingthis option.
Submitted information may now be reviewed. Wheninformation is correct, click Submit Dissertation/Thesis.
This screen shows the total fee for any copies ordered or forthe fee for copyright information. Click Done to finish.
Your submission is now complete. Click Sign Out to logout of the ETD System. You will receive an automatede-mail notification that the submission was successful.
