  • Vital Documents Replacement:

    Replacing all vital documents that were lost or destroyed can be overwhelming. These general steps can help you get started.

    1. Replace your birth certificate. Check to find out if you can obtain a certified copy of your birth certificate without any identification and follow the instructions. A few states don’t require a government-issued photo ID, or accept other solutions like a sworn statement of your identity. Some states allow your mother or father whose name is on the birth certificate to submit a notarized letter with a copy of their photo ID. If you do need your own government-issued photo ID, to get a copy of your birth certificate start with step 2.

    2. Replace your driver’s license or ID. Get this first if you cannot get your birth certificate. In some states, you can do it online. Present TWO original documents with your name and Indiana residential address. Documents must contain a residential address, not PO Box. Examples would be: utility company, credit card, doctor or hospital bill issues within 60 days of application, rental contract, court/probation mail. You may use this address as the location to have your ID mailed to: One Stop Shop, 825 Highway 31 N., Austin, IN 47102. MAKE SURE YOU CALL 812-794-2730 OR GO BY THEIR OFFICE TO LET THEM KNOW YOU HAVE AN ID COMING, SO THEY CAN HOLD IT FOR YOU. GIVE THEM YOUR CONTACT INFO SO THEY CAN CALL YOU WHEN IT ARRIVES.

    3. Replace your Social Security card. You will need a government-issued photo ID. Getting a replacement card is free.

    Government agencies usually mail replacement vital documents. But if your home was destroyed in a disaster, you might not be able to get your mail. Contact your local post office and ask if you can pick up your mail there or request to have your mail forwarded to a temporary location.

    They may also be purchased at the Health Department of the county where you were born.

    Social Security Office:This is where you get you social security card. They are closed New Year’s Day, Martin Luther King Jr. Day, President’s day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. You do not need a proof of address but must have an address to send the card too. You need either: driver’s license, passport, military ID, insurance card or certified medical records.     SCHEDULING A RIDE
Because our appointment times fill up quickly, please call us as soon as possible. Calling at least ten (10) to fourteen (14) days in advance typically ensures we will be able to meet the requested appointment time. We are able to schedule appointments eight weeks in advance. We will make every effort to meet transportation needs with short notice or to suggest an alternative solution. During the phone call to our Dispatcher, we will ask important information each time, including name, address, telephone number, destination address and special needs. All information is kept confidential. At times, our Dispatchers are away from the phone, so please leave a message on our answering machine and someone will return the call as soon as possible. 

    PICK-UPS AND RETURN TRIPS
Our drivers have a demanding schedule and to make sure we get individuals to appointments on time, please be ready at least 15 minutes in advance of the scheduled pick-up time.  Due to weather, road conditions and traffic jams, the pick-up time could change.  If we find that the driver will be later than 15 minutes past the pick-up time, the Dispatcher will call. On the day prior to the appointment, office personnel will call to confirm the appointment. The driver will give each rider a card with a phone number to call the Dispatcher when the appointment is over and it is time to return home from the appointment.  If it is a medical appointment, please have the nurse or receptionist call us and the Dispatcher will schedule a van for return trip.  All return trips must be scheduled by 2:30 p.m.

    WHERE WE CAN PROVIDE RIDES TO
While there are exceptions, our transportation priority focuses on medical trips including Dialysis, Chemotherapy/Radiation, doctors’ offices, hospitals and dentists.

    MEDICAID 
LifeSpan Transportation is an approved Medicaid provider and eligible for reimbursement from Medicaid when we provide transportation to a Medicaid approved appointment. It is very important to make us aware of your Medicaid eligibility when scheduling an appointment time.

    PROJECT 180 (LifeSpring)Project 180 aims to bridge the gap between the criminal justice system and behavioral health by reducing barriers for the criminal justice-involved client in accessing behavioral health services. We accomplish this by having a quick referral process and intensive evidence-based interventions that not only meet the needs of the client, but promote a pro-social lifestyle to enhance community safety.

    Project 180 is designed to provide services to the criminal justice-involved client with the following criteria:• Transitioning out of incarceration from either prison/jail, OR• Under Pre-trial Release/or Diversion OR• Under community supervision, such as Probation, Parole, or Community Corrections; AND• Has a mental illness or a co-occurring behavioral health and substance use disorder

    The client referred to Project 180 will receive quick and efficient interventions including: • Comprehensive Clinical Assessment that identifies both clinical and social needs• Individual Recovery/Treatment Plan addressing clinical needs and criminogenic risk• Referral to Evidence-Based Treatment groups + Skill-Building Programming• Care Management Services that include assisting client in the navigations of auxiliary needs• Ongoing communication and collaboration with referral sources
