STEPS TO DOWNLOAD AND SUBMIT: 1. Click on the link to access the scholarship application form. 2. Download the form to your local drive on your own computer and note the location of the saved file. 3. Open the Adobe Acrobat Reader application. 4. On the Acrobat Reader menu, go to File->Open. Open the file from the location in which you saved it. 5. Complete the form and save the changes before e-mailing or printing. 6. E-mail the form to [email protected] with the subject line LAHC Scholarship Application. Adobe Reader File à Open E-mail completed form to [email protected] Subject Line: LAHC Scholarship Application Download form to local drive.
