Choosing Your Doctor and Physician GroupTo make it easy for you to enroll, we have arranged the information in this directory

in several different ways.

Based on the availability of your current doctorTo see if your doctor is available for selection as a Salud con HealthNet primary care physician (PCP), turn to the Primary Care Physician Index. This index lists, in alphabetical order, available PCPs and the pages to reference for further enrollment information. For members selecting SIMNSA in Mexico or for California members who wish to receive care from SIMNSA, please refer to the SIMNSA Network of participating providers.

Based on where the doctors office is locatedTo choose a doctor based on convenience to your home or work location, turn to the listing of Primary Care Physicians by City. In this listing, the name of each PCP is grouped in alphabetical order by the city in which his or her offices are located.

Based on the doctors physician group affiliationTo select a doctor based on affiliation with a particular physician group, turn to the Participating Physician Group Index. In this listing, the names of Salud con HealthNet contracted physician groups are arranged in alphabetical order. An overview of affiliated PCPs and hospitals is provided for each physician group, in addition to information regarding Wellness Program availability.

Based on the doctors hospital affiliationIf you want to choose a doctor based on whether he or she has admitting privileges with a particular hospital, then youll want to turn to the Hospital Index. Here, hospitals affiliated with Salud con HealthNet in California are arranged in alphabetical order.

Next to the name of each hospital is a list of all affiliated physician groups. From this list, you can choose a physician group and turn to the Participating Physician Group Index to select your PCP.

Based on affiliation with a particular specialistAlthough you cant select a specialist physician as your PCP, you can select a PCP who belongs to the same physician group (and simplify your ability to be referred to that particular specialist). Simply look up the name of your desired specialist physician in the Specialist Index. Once youve located the name, turn to the page in the Participating Physician Group Index. There, youll find a listing of Salud con HealthNet PCPs who belong to the same physician group as the referral specialist.

How to Complete Your Enrollment FormAfter youve chosen a HealthNet PCP and/or physician group or the SIMNSA provider group, youre ready to complete your Salud con HealthNet enrollment form. Just follow the steps below:

For California members1. Use one of the methods mentioned under

Choosing Your Doctor and Physician Group to choose a PCP and/or physician group. You can choose a different PCP and/or physician group for each member of your family.

2. Verify physician group affiliation and location. Confirm that the physician group/clinic location

is convenient for you by ensuring that you live or work within 30 miles of your chosen doctors office.

3. On your enrollment form, write the PCP number, the name of the participating physician group, and the physician group number that youve chosen.

For members in Mexico1. You must select the SIMNSA physician group and

may not obtain any services in California, except in the case of emergency care or urgently needed care.

2. Write the SIMNSA physician group number only. There is no need to select a SIMNSA PCP.

How Salud con HealthNet HMO y Ms WorksSalud con HealthNet HMO y Ms is specifically designed for people living in the HealthNet Salud service area with dependents who live in California or Mexico, and who may wish to receive care in Mexico themselves. Providers in the HealthNet

Salud Network (Salud Network) have been selected to provide services to members of this plan who live in California. A network of physicians contracting with Sistemas Mdicos Nacionales S.A. de C.V. (SIMNSA) has been selected to provide services in Mexico.

HospitalizationYour PCP or referral specialist may find it necessary to refer you to a hospital for further care, on either an inpatient or an outpatient basis. You will usually be hospitalized where your physician has admitting privileges. (See your PCPs affiliated physician group listing in this directory for a listing of hospitals where your PCP has admitting privileges.)

It is important to remember that your PCP or authorized referral specialist must first be contacted for initial treatment and consultation before you receive any care or treatment through a hospital.1 Your PCP or authorized referral specialist will be responsible for determining the most appropriate site and location for your additional care.

Admission to and treatment rendered by any hospital listed in this directory must be authorized by your selected PCP or authorized referral specialist.

For members enrolled with the SIMNSA Network or for California members who receive care from SIMNSA, when directed by your SIMNSA physician, you will be hospitalized at a SIMNSA affiliated hospital in Mexico.

Managed Health Network (MHN)Not all Health Net members have access to the practitioners listed in this directory. Check your Summary of Benefits, or call our Customer Contact Center to determine whether there is a specific provider network for you to use. For more information about MHN, or for a referral to a counselor, call MHN at the number listed on the back of your ID card, or call our Customer Contact Center.

How to receive careIf you or your family members live in California

You may receive care in California from your Salud Network provider, or you may self-refer at any time to a SIMNSA provider in Mexico.

At the time of enrollment, members who live in California must select a Salud Network physician group close enough to their residence or place of work to allow reasonable access to medical care. You and your family members may select different participating physician groups.

Your Salud Network or SIMNSA provider will authorize and coordinate all your care, providing you with medical services or supplies.

If your family members live in Mexico

Family members who live in the approved service area in Mexico must select SIMNSA as their provider group and can access any SIMNSA provider for covered medical services. SIMNSA will authorize and coordinate all care, providing medical services or supplies. Family members living in Mexico may not obtain any services in California, except in the case of emergency care or urgently needed care.

About your Salud con HealthNet HMO y Ms ID cardAfter you enroll in HealthNet, you will receive a Salud con HealthNet HMO ID card. This card is what you need to receive all of your care as a Salud con HealthNet member. Because its so important, its a good idea to keep your Salud con HealthNet HMO ID card with you at all times. If you lose your ID card, call our Customer Contact Center or order a new one online. Go to www.healthnet.com, log in and click Order my ID card.

Frequently Asked QuestionsBecause you may come across some terms that are unfamiliar to you, weve included

some commonly asked questions relating to these terms.

What is a participating physician group?A participating physician group (PPG) is a group of physicians from various specialties assisted by nurses, technicians and other health care professionals contracted with Salud con HealthNet to perform services for Salud con HealthNet members. You may notice that some of our physician group listings include the words medical group, Independent Physicians Association or IPA in the physician group name. Typically, in the physician groups that include the term medical group in their name, most of the primary care physicians have their offices at one principal location. In an IPA, the PCPs typically practice at different locations. Some people prefer the under one-roof convenience of a medical group; others prefer IPAs, since having offices in different locations may be more convenient for them.

The Salud con HealthNet HMO y Ms Plan also allows members who live in California to receive covered services in either California (from their HealthNet Salud Network physician group) or in Mexico (from a SIMNSA provider). Members living in Mexico may only receive covered services from SIMNSA providers in Mexico.

How is a referral specialist different from a primary care physician?Your primary care physician (PCP) is your main medical doctor. He or she is the doctor who is responsible for coordinating all of your medical care and authorizing all specialized treatment or hospitalization. As a member of Salud con HealthNet HMO y Ms, you must obtain a referral from your PCP to see a specialist. Referral specialists are not available for selection as your PCP. SIMNSA members, or California members who wish to receive care from SIMNSA, do not have to have a referral from their PCP to see a SIMNSA Network specialist.

What is an affiliate office and how does it affect my selection of a PCP?Affiliate office locations often have limited physician availability and different office hours. Its also possible that your doctor offers office hours in more than one location. In addition, affiliates will often refer patients to the primary location of the physician group for laboratory and urgent care services.


Whats the difference between emergency care and urgent care?As you go through this directory, you may notice that we indicate hours for urgent care at each physician group location. So whats the difference between an emergency and an urgent care situation? Emergency care includes care for severe pain, active labor and psychiatric screening and treatment for which a reasonable person with an average knowledge of health and medicine would believe requires immediate medical attention. Urgent care, on the other hand, is any other care that a reasonable person with an average knowledge of health and medicine believes that, if treatment is delayed until a return to the HealthNet Salud service area, such delay may result in serious deterioration of his or her health, or, if pregnant, the health of the baby.

Your Salud con HealthNet HMO y Ms plan covers care received from any licensed provider or treatment center anywhere in the world for any emergency situation. For California members, situations requiring urgent care are covered when received from the members PCP and physician group while in the groups service area, and from any licensed provider or treatment center while out-of-area. All non-emergency and nonurgent care must be provided by or through the members PCP or physician group.

Members in Mexico, or California members who wish to receive care from SIMNSA, must receive care from SIMNSA participating providers.

How can I get a list of sub-acute care facilities?Sub-acute facilities provide sub-acute care for the special nursing needs of patients who do not need acute care and are too ill to be cared for by most skilled nursing facilities (SNFs). These include transitional settings following acute hospitalization (such as those discharged from the hospital but not fully recovered).

To request a sub-acute care facility list, call the Salud con HealthNet Customer Contact Center at the number listed on your ID card.

How can the HealthNet coordinator help me?In California, when youre choosing a PCP, its a good idea to call the HealthNet coordinator at the physician group to make sure the PCP you want is still accepting new patients. The HealthNet coordinator, a specially trained staff member at each groups administrative office, has the most up-to-date information, including each physicians office hours. The HealthNet coordinator will also have the names of new physicians who may have recently been added, since were constantly adding new physicians to our network. (If the doctor youre currently seeing is listed in our directory as not accepting new patients, dont worry You can continue to see your current physician as a member of Salud con HealthNet HMO y Ms.) For information regarding SIMNSA participating providers, call SIMNSA Member Services at 1-800-424-4652.

Should I choose a doctor close to home or close to work?That decision is entirely up to you. But your choice will probably depend on when you think youll access care most often. If you prefer to make weekday appointments, choosing a doctor whos close to your work location can make sense. However, for those times when you wake up feeling ill, youll probably appreciate having a doctor whose office is closer to home. Our general advice to ensure that you have the greatest access to the care you need is that you choose a doctor whose office is located within 30 miles of either your work or your home.

You should also research where you can access after-hours urgent care through your participating physician group to ensure the highest level of care.

When can I change to a new doctor?You can switch to another PCP within the same physician group at any time, for any reason. If you want to choose a new PCP affiliated with a different physician group in the HealthNet Salud Network, you can make a change on a monthly basis. If you choose a new PCP and physician group in the California service area before the 15th of the month, you can begin seeing your new PCP anytime after the 1st of the next month. If you choose a new PCP after the 15th, youll need to wait as long as six weeks before you can begin seeing your new PCP. For example, if you call the Customer Contact Center on August 12, you can begin seeing your PCP anytime after September 1. If you call on August 18, youll be able to see your new PCP on October 1. Members seeing a SIMNSA provider can change at any time. There is no need to call.

Can I go back and forth between the U.S. and Mexico to get services?The Salud con HealthNet HMO y Ms Plan also allows members who live in California to receive covered services in either California (from their HealthNet Salud Network physician group) or in Mexico (from a SIMNSA provider). Members living in Mexico may only receive covered services from SIMNSA providers in Mexico.

How can I obtain information about doctors in the HealthNet Salud Network?When doctors agree to provide care to Salud con HealthNet HMO members, they give us some key information to help you make informed decisions about your care. We make much of this information available in our directories, on our website (for California doctors) or by calling the Customer Contact Center. For doctors in the SIMNSA Network, please refer to the SIMNSA directory or call SIMNSA at 1-800-424-4652.

Cmo Funciona Salud con HealthNet HMO y MsSalud con Health Net HMO y Ms est especficamente diseado para personas que viven dentro del rea de servicio de Salud con Health Net, con dependientes que viven en California o en Mxico y que desean recibir atencin en Mxico. Los proveedores de la Red de Salud con Health Net (Red Salud) han sido seleccionados para brindar servicios a los afiliados a este plan que viven en California. Para brindarles servicios en Mxico se ha seleccionado una red de mdicos contratados con Sistemas Mdicos Nacionales S.A. de C.V. (SIMNSA).

Cmo recibir atencinSi usted o sus familiares viven en California

Usted puede recibir atencin en California de su proveedor de la Red Salud o puede autorremitirse en cualquier momento a un proveedor de SIMNSA en Mxico.

En el momento de la inscripcin, los afiliados que viven en California deben seleccionar un grupo mdico de la Red Salud que se encuentre lo suficientemente cerca de su residencia o lugar de trabajo como para que le permita un acceso razonable a la atencin mdica. Usted y sus familiares pueden seleccionar diferentes grupos mdicos participantes.

Su proveedor de la Red Salud o de SIMNSA autorizar y coordinar toda su atencin, brindndole servicios o suministros mdicos.

Si sus familiares viven en Mxico

Sus familiares que vivan en el rea de servicio de Mxico deben seleccionar a SIMNSA como su grupo de proveedores y pueden tener acceso a cualquier

proveedor de SIMNSA para obtener servicios mdicos cubiertos. SIMNSA autorizar y coordinar toda la atencin, brindndole servicios o suministros mdicos. Sus familiares que vivan en Mxico no pueden recibir ningn servicio en California, excepto en el caso de atencin de emergencia o de atencin requerida de urgencia.

Informacin sobre su tarjeta de identificacin de Salud con Health Net HMO y MsLuego de inscribirse en Health Net, recibir una tarjeta de identificacin de Health Net. Esta tarjeta es lo que necesita para recibir toda la atencin como afiliado a Salud con Health Net. Debido a que es muy importante, es bueno llevar siempre su tarjeta de identificacin de Salud con Health Net HMO con usted. Si pierde su tarjeta de identificacin, llame al Centro de Comunicacin con el Cliente para obtener una nueva tarjeta u ordene otra en lnea. HospitalizacinSu PCP o el especialista de remisin pueden considerar necesario remitirle a un hospital para que reciba ms atencin, ya sea como paciente hospitalizado o como paciente ambulatorio. Generalmente usted ser hospitalizado donde su mdico tenga privilegios de admisin. Algunas personas prefieren la conveniencia de un grupo mdico bajo un mismo techo, otros prefieren las IPA, ya que tener distintos consultorios en distintos lugares puede ser ms conveniente para ellos.

El Plan Salud con Health NetHMO y Ms tambin les permite a los afiliados que viven en California recibir servicios cubiertos en California (brindados por su grupo mdico de la Red Salud de HealthNet) o en Mxico (brindados por un proveedor de SIMNSA). Los afiliados que viven en Mxico slo pueden recibir servicios cubiertos de los proveedores de SIMNSA que se encuentran en Mxico.

En qu se diferencia un especialista de remisin de un mdico de atencin primaria?Su mdico de atencin primaria (PCP) es su mdico principal. Es responsable de la coordinacin de toda su atencin mdica y de la autorizacin de todos los tratamientos especializados y de las hospitalizaciones. Como afiliado a Salud con Health Net HMO y Ms, para consultar a un especialista usted debe obtener una remisin de su PCP. Los especialistas de remisin no se encuentran disponibles para ser elegidos como su PCP. Los afiliados a SIMNSA o los afiliados de California que deseen recibir atencin de SIMNSA no tienen que contar con una remisin de su PCP para consultar a un especialista de la Red SIMNSA.

Qu es un consultorio afiliado y de qu manera afecta mi seleccin de un PCP?Generalmente las ubicaciones de los consultorios afiliados tienen una disponibilidad de mdicos limitada y distintos horarios de atencin. Tambin es posible que su mdico ofrezca horarios de atencin en ms de una ubicacin. Adems, los afiliados generalmente remitirn a los pacientes a la ubicacin primaria del grupo mdico para los servicios de laboratorio y de atencin de urgencia.

Cul es la diferencia entre la atencin de emergencia y la atencin de urgencia?A medida que avance en este directorio, puede notar que indicamos los horarios para la atencin de urgencia en cada ubicacin de un grupo mdico. Entonces, cul es la diferencia entre una situacin de atencin de emergencia y de atencin de urgencia? La atencin de emergencia incluye cuidado para un dolor severo, parto activo, evaluacin o tratamiento psiquitrico para el cual una persona razonable con conocimiento promedio de salud o medicina concluya que se necesita atencin mdica inmediata. La atencin de urgencia, por otra parte, es cualquier otro es cualquier otro tipo de atencin que una persona razonable, con un conocimiento promedio de salud y medicina, crea que si el tratamiento se retrasa hasta regresar al rea de servicio de Health Net el retraso causar un deterioro grave de su salud, o si est embarazada, de la salud del beb.

Los afiliados en Mxico o los afiliados en California que deseen recibir atencin de SIMNSA deben recibir atencin de los proveedores participantes de SIMNSA.


Cmo puedo obtener una lista de los Centros de Atencin Subaguda?Los centros de atencin subaguda brindan dicha atencin para las necesidades especiales de enfermera de los pacientes que no necesitan atencin aguda y que estn demasiado enfermos para ser atendidos por la mayora de los centros de enfermera especializada (por sus siglas en ingls, SNF). Esto incluye los entornos de transicin posteriores a la hospitalizacin aguda (como por ejemplo, los pacientes que son dados de alta en el hospital pero que no estn completamente recuperados).

Debo elegir un mdico que est cerca de mi hogar o de mi trabajo?Esa decisin es exclusivamente suya. Cundo puedo cambiarme a un nuevo mdico? Usted puede cambiarse a otro PCP dentro del mismo grupo mdico, en cualquier momento y por cualquier motivo. Si desea elegir un nuevo PCP afiliado a un grupo mdico distinto dentro de la Red Salud de Health Net, puede hacer un cambio por mes. Si elige un nuevo mdico y un nuevo grupo mdico dentro del rea de servicio de California antes del da 15 del mes, puede comenzar a consultar al nuevo mdico en cualquier momento despus del da 1 del mes siguiente. Si elige un nuevo mdico despus del da 15, deber esperar seis semanas antes de poder comenzar a consultar al nuevo mdico.
