GOAL #1OUTREACH

• Enrollment via engagement of Veterans in their community or surrounding areas.

• Registration – assist with the completion of form 1010 for healthcare.

• Primary Care and Mental Health service connection.• Provide additional information and assistance on form

21-526 (with the support of VSO offices). • Assist in the navigation of the Department of Veterans

Affairs system……or at least refer to the appropriate department.

• Reach out to as many Veterans in Rural American as is possible.

GOAL #2EDUCATIONAL ENHANCEMENT

• Increase the veteran’s knowledge of, compliance with and awareness of chronic diseases such as Cardiovascular Disease (CVD), Congestive Heart Failure (CHF)and Diabetes Mellitus (DM).

• Present and provide education and guidance on healthcare information that is geared towards the 9 healthy choices as defined by VHA National Center for Health and Disease Prevention.

• Identify and partner with community organizations to create forums in the rural areas for health education classes, programs and outreach.

• Identify and partner with community leaders to ensure that Veterans have every opportunity to maximize their health outcomes through VHA, VBA and NCA services.
