Outreach began with patient letters mailed to encourage screenings.

The program has evolved over the past decade to include proactive outreach with a combination of touches (mail, phone, email, text), preventa-tive care at-home fecal immunochemical test

(FIT) kit distribution, patient surveillance, and EMR “closed loop” reporting.

In 2017, Kaiser screened 90 percent of its population in Southern California by performing 2,106,123CRCscreenings — upfrom45percenta decade ago.

SOLUTIONKaiser is focused on providing large-scale, con-tinuous patient outreach solutions. Recognizing the link between early detection, screening and improved outcomes for colorectal cancer, SCPMG engaged Bridgecom on an ongoing basis to mail FIT kits and instructions directly to patients’ homes to be screened based on age or family history.

Its innovative, patented packaging process for FIT kits meets USPS regulations for biohazards and costs the equivalent of mailing one letter.

The platform helps Kaiser manage and opti-mize its CRC screening outreach process, drive compliance, and improve education, awareness and participation. RESULTSIn California, Kaiser Permanente members between the ages 50 and 75 are screened for CRC at 90 percent. SCPMG regularly sees a 50 to 75 percent CRC screening response rate (industry average is 17 to 20 percent) and saves an average of $300,000 per patient in late stage treatment avoidance costs.

As a direct result of this standardized CRC disease screening and detection outreach program, Kaiser Permanente Southern California’s Medicare Plan received a 5 out of 5 rating from the National Committee on Quality Assurance (NCQA) in its Medicare Health Insurance Plan Ratings for 2015-2016.
