1. Purpose :

The main purpose of this safety plan is to prevent any kind of accident that will affect the

property, the environment, and worker to achieve Zero Accident during construction

activities at the project site.

2. Scope :

The Safety Plan steps for this project are describe below :

1. Personel and equipment mobilization :

a. Equipment operation must be competent licensed and checked to conforming

safety by supervision.

b. HSE audit and inspection of equipment to be carried out in accordance with steel

structure erection procedure.

c. Ensure personnel were physically fit, competent and qualified for the job complete

PT.PP (Persero) Tbk induction and job orientation.

d. Ensure all personnel were equipped with prescribe PPE requirement (safety glass,

safety shoes, helmet, etc) and fire extinguisher and first aid kit were provided at

job site.

2. Loading, Unloading and Erect material and equipment :

a. Good and clear job instruction to all persons involved assign a rigger to ensure that

equipment or material position is as close as possible to lifting and raise.

b. Ensure the load weight prior lift is not exceeding with crane capacity. Never walk

crossing under the load. The leader shall closely monitor the measurement of the

required wire rope standard and aware of the potential hazards with good rigging

technique.

c. Ensure the ground condition is free from sink hole and hard enough for positioning

of equipment/machine prior commencement work. The crane outriggers shall be

positioned onto steel plate or wood if found soft ground.

d. Pre-job safety meeting/pre lift briefing shall be held prior commencing work with

all crew involved. All worker shall be concentrated to the job especially on their

technique and will do observation on before, during and after doing the job.

e. Erection area will be closed with sign or barricade.

3. Working at height :

a. Worker shall be provided with personnel protective equipment such as safety

helmet, chain stap, hand glove, eye protection, safety shoes and ear protection

necessary as possible. Avoid using of detective, improperly connected apparatus.

b. Working at height shall be provided with full body harness and it’s lanyard shall

be tied off on strong materials or life line. Full body harness completed with

double lanyard. All access shall be provided for worker required to work at height.

c. Work permit requirements to be completed and signed before commencement of

any work.

d. Lifting worker will use man basket (especially to release webbing rope for erect

column)

4. Erection and lifting use scaffolding & bucket :

a. Scafollder or worker must use full body harness to prevent accident fall.

b. The scaffold must be erected, inspected, certified and tagged by competent and

qualified person.

5. Welding, grinding and cutting :

a. Hot work permit requirements is to be completed and signed before

commencement of any work as required.

b. Welding, grinding and cutting must be used special PPE requirement.
