ANTONIO MEDRANO MEMORIAL SCHOLARSHIP(A One Year Scholarship for $1,000.00

GENERAL INFORMATION:The Antonio Medrano Memorial Scholarship was established by a group of administrators and friends of Mr. Medrano. Mr. Medrano, Principal of Cesar Chavez Elementary School, passed away at the age of 51 on December 12, 2014. To honor him, a network of principals, with the blessing of his family, established this scholarship. Antonio, a life-long educator, worked hard supporting the families in his community. By establishing this scholarship, the trustees hope to continue his legacy and honoring all the hard work he did in supporting the community.

THE SCHOLARSHIP AWARD:There shall be one award annually to a student graduating from a high school in the Sacramento City Unified School District. The amount of this cash award will be determined annually by the trustees. The Trustees of the Antonio Medrano Memorial Scholarship shall review all applications and rate them according to the selection criteria described herein. The committee shall select the winning candidate.

WHO IS ELIGIBLE:The scholarship is open to all senior students in any of the Sacramento City Unified School District’s high schools. Applicants must enroll full-time in an accredited two- or four-year college, or vocational school.

Preference will be given to students who were enrolled in a bilingual program within the Sacramento City Unified School District, or attended Ethel Phillips, Edward Kemble, Washington Elementary, or Cesar Chavez Intermediate School.

NOTIFICATION OF WINNERS:Scholarship winners will be announced at the selected student’s High School Awards Assembly.

DUE DATE:Applications must be received by the Grant Bennett Awards Committee no later than, Monday, April 3, 2017. The transcript and all letters of recommendation are to be opened and stapled with the packet. Packets can either be dropped off or mailed to the following address:

APPLICATION PROCEDURE

Any student who meets the stated qualifications must completely fill out an application. The completed application must be accompanied by the following documents:

(Suggestion: Use this list as a final check-off before submitting the application packet.)

1. Student’s Application : (if not typed, please print clearly) Name, address, phone numbers, email, and Student ID number. List schools attended. Financial information must include: Father’s/Guardian’s name, occupation and

total yearly income and Mother’s/Guardian’s name, occupation and total yearly income.

Names of accredited colleges or vocational/trade schools you may attend.

2. Individual Student Profile : Complete all five areas listed. Sign and have a parent/guardian sign. Attach a

separate sheet if more space is needed.

3. Two Letters of Recommendation (opened and attached to packet) :Both letters must include a phone number and contact information, and if possible, should be on official letterhead. One from an educator (i.e., teacher, counselor, principal, etc.) at the student’s

high school; and One from an adult community member who is not related to you (i.e.,

community organization representative, church or religious leader, employment supervisor, etc.)

4. Student Essays (3 separate 500 word essays-- must be typed) :Students must provide complete responses to the three questions listed below. Each answer should be 500 words or less. Essays written in Spanish are acceptable.1. Leadership: Discuss your leadership ability and potential. Provide examples of

when you demonstrated leadership skills or character traits.2. Autobiography: Describe the world you came from: your family, community,

schools, and tell us how your world has shaped your dreams and aspirations. What is an experience that has occurred in your life that is important, and why?

3. Impact of a school leader: Explain your relationship with staff and leadership during your school age years. Specifically, what were some of your memorable interactions and how did it impact your life.

5. Transcript : An unofficial transcript of student’s high school record, including the first

semester of your senior year.
