PLAYER REGISTRATION FORM 2018

In addition to completing this page, a player is also required to complete the following: BFQ Player Registration Form (Saturday pm competition). (see reverse)

In signing up to play, you accept the following conditions.

1. Riverlife Baptist Church (RBC) is the sponsor of the Riverlife Baptist Football Club

(RBFC). At training and during matches players are required to adopt Christian standards that reflect the values of Riverlife Baptist Church.

2. The officials of the Club i.e. Coaches/ Managers/ Chaplains are volunteers and require and value your support and respect in the performance of their duties.

3. Our Club expects you to play to the best of your ability which will mean players need to look after their fitness and commit to developing team skills by making training a priority. Players are expected to communicate an explanation to their coach / manager if they are unable to attend a training session or scheduled game: the sooner the better. Failure to notify the team coach/manager of your absence from training may result in you being excluded from the next match.

4. Grading of all players occurs during pre-season training and pre-season games. Team selection is predominantly based on player ability and commitment.

5. FEES FOR SEASON - 2018. $390 (Women) $390 (Men)

Fees cover costs of field maintenance, lighting, equipment, insurance, association costs and referees. Payment Terms are as follows.

• Full Fees are $390 and must be paid by the end of April.

• A discount of $40 is available if full fees are paid by the Tuesday 31st March.

Players must provide their own socks, shorts (available for sale), shin pads and boots.

Casual players must register before playing any game - the initial cost is $110 for 5 games (the first game is free) and $110 for each subsequent 4-game extension.

6. Registration and payment of fees can be made online at the Riverlife Baptist Church Store.

7. MEDICAL INSURANCE: The BFQ / QCSA provides registered players some injury

insurance to assist with expenses for major injury incidents only. See additional information on the relevant Association player registration form.

I hereby apply for Registration with Baptist Football Queensland to play for Riverlife Baptist FC for the 2018 season in the Saturday afternoon competition. I agree to abide by the Constitution, Competition Rules and Players Code of Conduct of BFQ Incorporated. I understand that:

• In the interest of safety for all players, Baptist Football Queensland requires that ALL players shall wear appropriate shin pads and footwear as approved by the referee controlling each game during all matches sanctioned by BFQ Incorporated.

• The association offers player’s injury insurance to all of its registered players whilst participating in or training for BFQ Incorporated sanctioned activities.

• The provision of personal income protection insurance to cover me when training for and playing matches sanctioned by BFQ Incorporated is my own responsibility, and should I require such insurance it is my personal responsibility to arrange this independently of the association through a suitably registered insurer or insurance agent.

• I understand and agree that whilst BFQ Incorporated will strive to enhance player safety at all of its sanctioned games BFQ Incorporated and its Office Bearers accepts no responsibility or liability for any and all injuries or loss of income incurred by me as a player involved in or training for BFQ Incorporated sanctioned matches

• I declare that I have NO pre-existing medical conditions or ailments that could be harmful to myself or others whilst training for or playing in BFQ Incorporated sanctioned activities.
