RIDE CONNECTION

SENIOR CONNECTIONS RIDE CONNECTION is a mobility management service to help community members in need. We are able to provide two (2) round-trip rides per month to non-emergency medical appointments for persons who qualify and live in the City of Richmond and the Counties

of Charles City County, Chesterfield, Goochland, Hanover, Henrico, New Kent and Powhatan. We also provide travel training and referral services to other transportation providers

(including GRTC’s Care Van) if we are not able to provide a ride for a client.

Eligibility: Individuals with Disabilities (age 18 or over) or Older Adults (Age 60+)

To book a ride or request tickets or other services: Call our hotline at (804) 672-4495 AND LEAVE A DETAILED MESSAGE with the following information:

The hotline is a recorded line that is monitored throughout each business day. It is not “live.” Please leave a clear, detailed message. Rides are scheduled on a first-come, first-served basis. We require seven

(7) business days’ notice to schedule rides, which are subject to availability.

Some clients are asked to pay for a portion of their ride, depending

on where they fall on the Virginia Poverty Sliding Scale.

TRANSPORTATION TERMS

Paratransit is recognized in North America as special transportation services for people with disabilities, often provided as a supplement to fixed-route bus and rail systems by public transit agencies. Paratransit services may vary considerably on the degree of flexibility they provide their customers. Ambulatory means “able to walk unassisted or with a cane or walker.” Non-Emergency Medical Transportation can be defined as a transportation service provided to individuals who are not in an emergency situation (does not require an ambulance) but need more assistance than a taxi service typically provides. Human Service Transportation refers to services provided by or on behalf of a human service agency to provide access to agency services and/or to meet the basic, day-to-day mobility needs of transportation-disadvantaged populations, especially individuals with disabilities, older adults, and people with low incomes. Mobility Management consists of short‐range planning and management activities and projects for improving coordination among public transportation and other transportation service providers carried out by a recipient or sub-recipient through an agreement entered into with a person, including a government entity, under 49 U.S.C. Chapter 53 (other than Section 5309). Mobility management funding does not include operations of public transportation service.

CHESTERFIELD COUNTY CITIZEN INFORMATION & RESOURCES’ MOBILITY SERVICES

Do you need a ride to medical appointments, grocery shopping or to visit family or friends? Do friends or family members have to take time off from work to take you to appointments? Do you want to be independent again?

Mobility Services can help. We have two service options available to Chesterfield County residents registered with the Mobility Services. To register you must be a Chesterfield County resident and:

be 60 or over; or

be a person with a disability; or

live in a low-income household

SENIOR CONNECTIONS’ RIDE CONNECTION – GRTC BUS TICKETS

Our grant-funded program is in place to support individuals aged 60+ who are disabled with non-emergency medical transportation. We have a limited number of discounted GRTC bus tickets available. You must register to receive tickets by calling 804-672-4495. Please note: Persons under the age of 60 must submit proof of disability in the form of a SSDI statement and must register with Care Van. As a member of Care Van you can ride any GRTC bus at no charge.

How to request tickets?

Please call our Transportation Hotline at (804) 672-4495 between 9:00 am and 4 pm and leave your Name, Address and Phone Number and if you will be picking up your tickets or want us to mail them. We will return your call within 24 to 48 to confirm.

Please note, due to funding limitations we will only issue 4 tickets per month.

If you would like to make a donation to Senior Connections towards the cost of your bus tickets – please call and request a donation envelop. The cost to Senior Connections for your bus tickets is $7.00 a month.

Maximize your tickets! The GRTC tickets you receive from Ride Connection are All Day Passes. You can transfer as many times as you need. Plan on scheduling multiple medical, pharmacy appointments and other medical appointments and support as you can in one day.

GRTC BUS TRAVEL TRAINING

GRTC’s Family of Travel Training Services provides customers, 15 years of age or older, the opportunity to learn skills that will enable them to follow a GRTC bus route. Travel training is a process of teaching individuals how to ride public transportation safely and independently. Travel Training Instructors create individualized travel plans that aim to meet the customer’s transit goals and show them how to use fixed route transportation in order to increase transit options. Travel training is free, except for the bus fare cost. As always, all CARE customers may ride local fixed route bus service for free. Family of Travel Training Services

Basic Bus Orientation o Initial meeting with the Travel Trainer to discuss travel goals and complete one or two bus trips. o This meeting may be conducted one-on-one or for small groups.

Personal Mobility Device Familiarization o Exclusively for CARE customers. o CARE customers practice boarding and alighting from fixed-route buses at GRTC HQ.

One-on-One Training o Personal training for specific trip destinations. o Includes an initial meeting with the Travel Trainer to discuss specific travel goals. o Open to anyone! o Can be catered to new transit riders, individuals with disabilities, minors aged 15 and older, and seniors.

Specializes in travel training for seniors and people with disabilities. Kelsey has worked with individuals with disabilities for more than a decade.

Has a Masters in Teaching from James Madison University (JMU) with a concentration in Special Education. In her words: "I know the importance of being an active member in the community. Travel training will increase

participants' independence and community access." Who qualifies for Travel Training?

Anyone aged 15 and older. How much does the program cost?

This program is FREE. Valid GRTC bus fare required.

o Current CARE customers ride GRTC local fixed route for free. o Reduced fares are available for certain riders.

Are CARE customers required to switch to riding fixed route?

No! CARE customers may continue to utilize CARE service, but can ride for FREE on any local fixed route bus by swiping their CARE cards.

GRTC CARE

CARE offers curb-to-curb transportation for individuals with disabilities. GRTC Transit System’s CARE and CARE Plus services provide origin-to-destination service under the guidelines of the Americans with Disabilities Act (ADA) for the citizens of the Richmond Region. CARE and CARE Plus provide public transportation access to individuals with disabilities who may not be reasonably able to use GRTC fixed route bus service. According to the ADA, "origin-to-destination" service means providing service from a passenger's origin to the passenger's destination. GRTC primarily provides curb-to-curb mode of service. However, GRTC also provides assistance to those passengers who need assistance beyond the curb in order to use the service unless such assistance would result in a fundamental alteration or direct threat. As of March 2018, there are 80 cutaway vans and 8 sedans in the fleet. GRTC contracts with First Transit, Inc. to operate paratransit services using GRTC vans (look for the silver van with a GRTC logo). Additionally, VanGo is a subcontractor of First Transit also providing paratransit services using VanGo vehicles (look for the white vehicle with a VanGo logo). Service Areas & Operating Hours All CARE trips are identified as either CARE or CARE Plus service. CARE and CARE Plus services are available in the City of Richmond, Henrico County, and portions of Chesterfield County.

*(Hours may vary depending on fixed route bus service at the desired time of travel. Additionally, one fixed route does operate until 4:00 am on weekdays.)

CARE trips are required by the Americans with Disabilities Act (ADA). This service operates within GRTC’s fixed route coverage area and including 3/4 of a mile beyond GRTC’s fixed route bus lines.

CARE Plus: 6:00 am - 8:00 pm (City of Richmond residents)

6:00 am - 11:00 pm (Henrico County residents)

CARE Plus service is not required by the Americans with Disabilities Act (ADA). A trip will be designated as a CARE Plus trip if the origin or destination location is more than 3/4 of a mile from GRTC’s fixed route bus line, or if travel is desired to a destination in Henrico County on a day or time when GRTC’s fixed route buses are not running in Henrico County.

CARE Plus: $6.00 (City of Richmond residents)

Pre-purchased tickets are the only accepted form of payment on CARE vans. Tickets are sold in books of six (6) and ten (10). A ticket book of 6 costs $18.00. A ticket book of 10 costs $30.00. Tickets may be purchased from select GRTC ticket outlets or by mail. CARE ticket books cannot be purchased at GRTC Headquarters. Call (804) 358-GRTC for information about mail order and ticket outlet locations. Go Cards or cash cannot be accepted on CARE vans. Every CARE customer must present a ticket to ride the service. A ticket must be provided for each ride for each rider, including children. Only one child, under the age of 5 and the personal care assistant (PCA) may be permitted to ride for free, with a paying CARE customer. Anyone who does not present a ticket will not be transported. Local Route Bus Fares The CARE ID card enables a passenger to ride most GRTC local routes for free. The rider must present a valid CARE ID card when boarding the bus. Free fare is not available on express routes or the 19 Pemberton. The entire fleet of GRTC vehicles is equipped with wheelchair lifts, and each operator is properly trained and prepared to provide immediate assistance to individuals with disabilities and seniors who require it. Be sure to provide the customer’s first & last name and a callback phone number along with the reservation request. A CARE representative will call back to confirm. No-Show/Late Cancellation Policy Effective July 1, 2014: The revised No-Show / Late Cancellation Policy will go into effect for all CARE customers. Click here to view the policy or you may contact the CARE Customer Service Center at (804) 782-CARE (2273) to request a copy by mail.

Replacement CARE Cards To request a replacement CARE card, please download and complete the Replacement Card Request Form below.

Holiday Schedules On the following holidays, there is a service change. For the regular, fixed route GRTC service, GRTC will operate a holiday schedule and CARE will run a reduced schedule. For the CARE service this means ALL standing reservations are canceled, and customers will have to book their trips for that day. To book that trip you will need to call the reservation line at 804-782-2273 within seven (7) days prior to traveling, and follow the prompts. You can also fax or email your requests. There are seven (7) days that are considered holidays, for service, and they are:

New Year’s Day - (CARE Customer Service also CLOSED this holiday)

GRTC C-VAN Through C-VAN, GRTC Transit System is helping the Department of Social Services remove transportation as a barrier to employment. C-VAN provides transportation assistance for Virginia Initiative for Employment not Welfare (VIEW) participants. Transportation options include curb-to-curb service through public transit and shared-ride services to work and daycare facilities. are, please call your Benefits Program Specialist.

Transportation to medical care is a required Medicaid benefit by federal law. While all Medicaid recipients must have this

basic benefit, some CCC Plus MCO’s do provide additional transportation benefits as well. For example, Anthem offers its

members up to 3 rides every 3 months to community events, grocery stores and other places. Aetna offers up to 30 round

trips a year to the grocery store, church, or social activities.

ROAD TO RECOVERY

Every day thousands of cancer patients need a ride to treatment, but some may not have a way to get there. The American Cancer Society Road To Recovery program provides transportation to and from treatment for people with cancer who do not have a ride or are unable to drive themselves. How does the program work? Depending on your individual needs and what’s available in your area, we will:

Coordinate a ride with an American Cancer Society volunteer driver Coordinate a ride with a local organization that has partnered with us to provide transportation Refer you to a local resource you can contact for help

Eligibility Patients must be traveling to a cancer-related medical appointment. Other eligibility requirements may apply. For example, a caregiver may need to accompany a patient who cannot walk without help, or is under age 18. Contact us to find out what is available in your area, and what the specific requirements are. The Hanover Senior Rides program offers rides to Hanover County seniors (60+) who live within the defined service areas. Currently, there are two faith organizations that work together to provide those rides. MCEF (Mechanicsville Churches Emergency Functions) provides rides in the Mechanicsville area. It encompasses zip codes 23111, 23116 and a portion of 23069. ACES (Ashland Christian Emergency Services) provides rides in the central Hanover County area (Ashland, Elmont and Hanover Courthouse areas). It encompasses zip codes 23005, 23059 and 23069. The Hanover Senior Rides program is a free transportation program is for scheduled medical appointments, grocery shopping and for personal business, such as banking. While there is no charge for the rides, we do ask the rider to consider a donation to the appropriate agency to help sustain the service. Even though our organizations work together, each agency has its own coordinators and its own drivers and the program is sustained by donors in their respective communities. Rides must be requested 7 to 14 days in advance of the appointment so that there is enough time to schedule the trip and secure a volunteer driver. Our riders must be ambulatory – however, a cane or a walker is permissible to use. While the focus of Hanover Senior Rises is on seniors and those with disabilities, we also provide rides to anyone who may have a medical condition that requires frequent trips for medical treatments such as dialysis or chemotherapy. All our ride coordinators and drivers are volunteers who give their time to operate this program. The drivers use their own vehicle to provide the rides and are paid 25 cents per mile. They are provided additional liability insurance coverage through the Department of Risk Management. Potential drivers are required to provide a valid driver’s license, a certified copy of their DMV record and give us permission to request a background check. They are also required to complete 8 hours of driver’s

Drivers are asked to try to give at least 4 rides per month. To ensure that all rides can be accommodated, MCEF and ACES have entered a partnership with Senior Connections and UZURV to provide rides when a volunteer driver is not available. The partnership allows us to guarantee the ride for our clients. Additionally, through this partnership, residents of western Hanover are offered rides for medical appointments. MY MOBILITY PLAN Make a plan today. Stay independent tomorrow.

WHAT CAN YOU DO TO STAY INDEPENDENT? Many people make financial plans for retirement, but not everyone plans for other changes that may come with age. This includes changes in your mobility—your ability to get around. No, it’s not always easy to talk about, but as we get older, physical changes can make it more difficult to get around and do things we want or need to do: driving, shopping, or doing household chores. ACT NOW You might not have mobility problems now, but you can’t predict the future. Think pro-actively. There are actions you and your loved ones can take today, and as you age, to help keep you safe and independent tomorrow. A PLAN TO STAY INDEPENDENT Staying healthy and managing chronic conditions help maintain your mobility. To start building your plan, complete this checklist:

Get a physical checkup each year. Some health issues may increase your risk of falling (such as leg weakness and balance problems).

Review all your medicines with a doctor or pharmacist. Certain medicines can have side effects that can change your ability to drive, walk, or get around safely.

Follow a regular activity program to increase your strength and balance. Strength and balance activities, done at least 3 times a week, can reduce your risk of falling. Other activities, like walking, are good for you, but they don’t help prevent falls.

Get a medical eye exam each year. Eye problems can increase your risk of falling or being in a car crash.

Write things down. Write EVERYTHING DOWN. We all have to do this. Write down all your doctor’s appointments and the arrangements you made for transportation to and from. Write down reminders to yourself about when prescriptions will need to be refilled to avoid last-minute crises.

A PLAN TO STAY SAFE AT HOME To continue your plan, schedule a time to go through the following home safety checklist to help prevent falls.

Check the FLOORS in each room and reduce tripping hazards:

Keep objects off the floor.

Remove or tape down rugs.

Coil or tape cords and wires next to the wall and out of the way. Check the KITCHEN:

Put often-used items within easy reach (about waist level, if possible).

For items not within easy reach, always use a step stool. NEVER use a chair. Check the BEDROOMS:

Use bright light bulbs. A 40 watt bulb is not going to get the job done.

Place lamps close to the bed where they are within reach.

Install night-lights to be able to see a path in the dark. They can increase your feeling of being oriented and safe. For areas that don’t have electrical outlets, consider battery-operated lights that you can get at Walmart, Lowe’s, or any discount store.

Check inside and outside STAIRS and STEPS:

Check for loose or uneven steps. Repair if needed.

Make sure carpet is firmly attached to every step, or remove carpet and attach non-slip rubber treads.

Check for loose or broken handrails. Repair if needed.

Consider installing handrails on both sides of the stairs, if that is feasible.

Use bright overhead lighting at the top and bottom of the stairs.

Consider putting light switches at both the top and bottom of the stairs (if that is a financially feasible option).
