2. สามารถใชไดหลายแบบ เชน การศกษาระบาดวทยา, การ

1. ขอมลอาจไมครบ หรอไมถกตอง • อาจม selection bias (อคตจากการเลอกผปวย) • อาจม recall bias หรอ misclassification bias • อาจม confounder (risk factor อนทไมไดท าการบนทกไว)

2. อาจไมไดค าตอบถงประสทธภาพการรกษาทนาเชอถอได เนองจากไมม randomization และ blinding

• การศกษาความชก (prevalence) ของ case SLE ในแผนกอายรศาสตร • การศกษาอบตการณ (incidence) ทพบ case SLE ใหมตอเดอน

ค าถามวจยตองการการพรรณนา (description)

• อาจตอดวยการพรรณาวาผปวยมลกษณะอยางไร (อาย, เพศ, อาชพ) ทเปน SLE แลว และแตละกลมไดรบรกษาแบบ OPD, IPD หรอ ICU

• ผลการศกษามกเปน สดสวน, รอยละ, คากลาง (mean, median), การกระจาย (standard deviation, range, interquartile range) หรอน าเสนอดวยตาราง, กราฟ

3. Interval (ไมตอเนอง): คอการวดทเรมจากศนยสมมต และชวงระหวาง 1 หนวยมคาเทากน เชน อณหภมในระบบเซลเซยส, คะแนนสอบ, score ตางๆ, จ านวนบตร

• การศกษาหาความสมพนธระหวางระดบโฟเลตในแม (ratio scale) กบ คะแนนอารมณของแม (emotional score ซงเปน interval scale) ใช Pearson correlation (normality) หรอ Spearman Rank correlation (non-normality)

Odds = โอกาสเกด / โอกาสไมเกด เชน โอกาสทอดลกเตาได 4 = 1/5 = 20% Odds ratio เปนการวดระดบ Association ระหวาง exposure กบ outcome จากสตร Odds ratio = Odds ท outcome จะเกดหากม exposure / Odds ท outcome จะเกดหากไมม exposure ใชใน case control study

• การศกษาเปรยบเทยบชนดและปรมาณของ resuscitation fluid กบอตราการรอดชวตใน ICU

• ควรมขอมลทเกยวของ เพอใหสามารถปองกนอคต (bias) เชน septic shock, DM, APACHE score และผลจากการรกษาอนทใหรวมกน (co-intervention) เชน การใหเลอดหรอยา

• ผลการศกษามกใช mean, median, range การวเคราะหอาจใช univariate (t-test, Mann-Whitney U test, chi squared test) หรอ multivariate analysis (logistic regression) หรอ odds ratio

3. มค าจ ากดความของตววด (operational terminology)

• สมมตวาจะศกษา burn-out, respiratory failure ตองใหจ ากดความวาคออะไร

• ถาศกษาผลเลอด ตองระบคาปกตและ range ทปกตคออะไร • ถาศกษาความปวด (pain) อาจล าบาก เชน ปวดตอๆ ปวด

แปลบๆ จงตองมการใชมาตรวดทเปนมาตรฐาน เชน numeric rating scale, McGill Pain Questionnaire

• ตองคนวรรณกรรมมากอนวานกวจยอนๆใชอะไรและมค าจ ากดความวาอยางไร

4. ตองสามารถดงขอมลมากรอกอยางถกตอง

ผมหนาทดงขอมลจาก chart มากรอกใน Case record form • มการอบรมใหเขาใจค าจ ากดความของ variable ตางๆ • มคมอการดงและกรอกขอมล • ลองซอมกรอกขอมลจาก chart ดวามขอของใจอะไรหรอไม • ผวจยตองมาตรวจสอบความถกตองของการกรอกขอมล • มการประชมกบผกรอกและตองตดตามดตลอดวากรอก

ขอมลทนหรอไม (accuracy, consistency, timeliness) • ผกรอกไมควรมอคต ดงนนควรไมรวากลมใดคอวธอะไร

5. ควรใชแบบฟอรมในการกรอกขอมล • ชวยลดความผดพลาดในการกรอกขอมล • ควรไดรบการออกแบบใหกรอกงาย, ไปตาม flow ของขอมล • อาจเปนกระดาษซงราคาถกแตควรม guideline และ pre-print data

form และอาจมปญหาเรองลายมอ • อาจเปน electronic ซงมขอดส าหรบขอมลจ านวนมาก ลดการเกบ

รกษาและการลอกผด • ในโครงการทมคณภาพจะมการมอบหมายผกรอก, อบรมวธแกไข

6. ควรใชคมอ (manual) ในการกรอกขอมล

• เพอความเขาใจตรงกนระหวางผกรอกขอมลและผตรวจสอบ

• มประโยชนมาก ถามผกรอกหลายคน หรอมหลาย site

• อธบายวธการ ขนตอน ในการดงขอมลมากรอก อาจใชรปชวย

• มการเกบตวยอ ค าจ ากดความตางๆอยางเปนระบบ

7. ควรม inclusion, exclusion criteria ทชดเจน

• ทงในโครงการวจยและในคมอการกรอกควรม criteria ชดเจน • Exclusion criteria เชน เมอขาดขอมลทส าคญหลายอน, เมอม

โรคหรอความผดปกตทจะเปน confounding co-morbidities หรอสงทจะลดความถกตองของการวจยอยางรายแรง

• ผกรอกตองไดรบการอบรมเรอง inclusion, exclusion criteria • ควรมการน า chart ทถก excluded มาพจารณาภายหลงวา

8. ควรมการค านวณ inter-rater, intra-rater reliability

• เพอประเมนวาผกรอกขอมลหลายๆคน หรอคนเดยวหลายๆครง มการกรอกขอมลแบบเดยวกน (consistency) หรอไม

• Inter-rater reliability หาโดยใช Cohen’s kappa ซงควร >+0.6 • Intra-rater reliability หาโดยใช intra-class correlation (ICC)

9. ควรม pilot study

• การท ากลมเลกซงไมสามารถใชวเคราะหผลหรอตอบค าถามวจยได แตมประโยชนในการประเมนความเปนไปได (feasibility) ของรปแบบการวจย การด าเนนการวจย การดงขอมล การกรอกขอมล รวมทงขอมลทหายไป ทดสอบ inclusion, exclusion criteria และ reliability

• โดยทวไปใชจ านวน 10% ของ sample size จรง • Pilot study ทท าในคน ตองขอการรบรองของ IRB ดวย

10. ตองค านงถง Confidentiality และ Ethical consideration

• ตองมการรกษาความลบผปวยอยางเครงครด, ใชรหสแทนชอ • ตองค านงถงผลเสยตอชอเสยงของผปวย หรอชมชน • ตองไดรบการรบรองจากคณะกรรมการจรยธรรมฯ กอน • Health Insurance Portability and Accountability Act of 1996

(HIPAA) ของสหรฐ ตองไดรบ consent จากผปวยเจาของขอมลกอน(ยกเวนไดรบอนญาตพเศษ)

• ในประเทศไทย ทปฏบตกนอย เมอผาน IRB แลวตองไดรบการอนมตจากผอ านวยการโรงพยาบาลกอน

• หวขอใดไมเกยวของ ขอใหเขยนวา “ไมเกยวของ” • ตองแนบจดหมายขออนญาตจาก ผอ. แตจะเกบขอมลไดตองหลงจากไดรบการรบรองจาก IRB แลว

การยกเวนการขอค ายนยอม (Waiver of informed consent) IRB จะท าไดตอเมอ:

• งานวจยนนมความเสยงต า • การยกเวนนไมมผลเสยตอสทธและความปลอดภยของ ผป. • การวจยจะท าไปไมไดเลยถาตองไปขอค ายนยอม และ • เมอเปนไปได, ผป.จะไดรบการแจงขอมลหลงเขารวม

การวจยทไดเขาขายยกเวนการขอค ายนยอม จะไดรบ Waiver of HIPAA Authorization ดวย

พรบ.สขภาพ 2550 ของไทย “…..ประสงคจะใชผรบบรการเปนสวนหนงของการทดลองในงานวจย …..ตองแจงใหผรบบรการทราบลวงหนา และตองไดรบความยนยอมเปนหนงสอกอนจงจะด าเนนการ”
