• West Farm closed June 1, 2012.• RFP issued on May 31, 2012 targeting an older population including out of
state youth returning to SD and focusing on re‐entry and transitional services. All potential providers notified of the release of RFP #26170.
• Written proposals from providers due back August 3, 2012 .• Written proposals evaluated by selected DOC team.p p y• Providers also gave in person presentation on written proposals.• Sequel Youth & Family Services selected as the provider from proposals
submitted DOC began negotiating with provider on services/ratesubmitted. DOC began negotiating with provider on services/rate.• Agreement signed and effective on February 15, 2013 and new program to
begin operation on June 1, 2013 to be named Sequel Transition Academy to be located @West Farmto be located @ West Farm.

Sequel Youth and Family Services (“Sequel”) was founded in 1999 to operate a single school for adjudicated delinquent boys. Since then the company has experienced tremendous growth and now serves a wide range of populations inexperienced tremendous growth, and now serves a wide range of populations in a wide range of settings. Our programs currently operate in 16 states and serve clients from 36 states and territories.

Sequel Transition Academy UpdateSequel Transition Academy Update
• May 2013 – STA granted group care license by the DSS to serve 32 males.• Agreements signed by STA with West Central School District to provide
full range of alternative educational services and to proivde on site foodfull range of alternative educational services and to proivde on site food service.
• Summer School provided from June 13‐ August 9.A t ith l l id f b t b d ff d• Agreements with local providers for substance abuse and sex offenders and medical services were signed and in place.
• June 3 ‐ Admitted the first 3 students.• As of September 19, 16 students at STA. • Youth are engaged in high school, local jobs, & post secondary.• OPEN HOUSE –October 24, 2013 from 10:00 AM‐Noon
