ADDENDUM NO. 1
ISSUE DATE: October 2, 2017
Bidders on this project are hereby notified that this Addendum shall be attached to and made a part of the above named bid documents due October 10, 2017.
The following items add to, modify, and clarify the bid documents and shall have the full force and effect of the original documents. Bids shall conform to those items and the cost change, if any, of these items shall be included in this bid price. This Addendum shall be acknowledged by the bidder on Appendix C, Addendum Receipt and Anti-Collusion Form.
Bid Documentation
ADD Appendix A – Pricing Form, Page 2 found in Addendum No. 1 (next page). The
new form must be submitted with RFP response. Failure to do so will make the
submittal unresponsive.
ADD Appendix F – Existing and New Building Floor Plans (last five pages of
Addendum No. 1). Floor plans are representative of the actual buildings and show the
approximate locations of door, stair, elevator, and furniture locations, as envisioned by
an architect. There may be minor differences between the drawings and the actual
building(s).

Moving Services

QUESTIONS
1. Question: Will we be able to move on Friday all day or just after 5:00 PM?
Answer: We are looking for the successful vendor to recommend a timeline for the
most efficient move.
2. Question: Will we be able to move on Monday all day or just before 8:00 AM? Answer: We are looking for the successful vendor to recommend a timeline for the
most efficient move.
3. Question: What percentage of the move will go to each floor in the new building,
i.e., 33% to each of the three floors or 20%-1st floor, 40%-2nd floor, 40%-3rd floor? Answer: Workstation moves will be approximately ~15% 1st floor, 45% 2nd floor,
40% 3rd floor.
4. Question: What is the total number of TV’s in the building? Answer: The following list includes TV’s and other A/V equipment that will be removed from walls or ceilings:

5. Question: What is the total number of clinical rooms? Answer: There are sixteen (16) medical clinical rooms and three (3) dental clinical rooms.
6. Question: Are the dental chairs hardwired or plug-in; do we need an electrician to
disconnect? Answer: The dental chair electric connection is a plug-in connection.
7. Question: Would the County pack storage & break rooms to save money if we
provided crates or fast packs? Answer: Prepare your bid with your firm providing all packing; the County will
unpack. The possibility of the County packing may be an item of negotiation with
the successful vendor.
8. Question: Are crates for storage/supply rooms and file cabinets an extra charge
item? Answer: No; all crating, except the typical plastic moving crates for employees, is to
be provided in the base bid. The base bid will include all items detailed in the RFP
document. Items outside the Scope of Work discovered during inventory with the
selected vendor will be negotiated and confirmed in the contract.
9. Question: Can we get floor plans of both buildings? Answer: Yes, see Appendix F, added as part of Addendum No. 1.
