In September of 2014, the City of Rockford opened the City of Rockford Wellness Center, which serves asa near-site health center for its employees. The health center is operated by Marathon Health, one of thenation’s leading providers of worksite healthcare. Marathon Health provides comprehensive care similarto the care provided by a family physician. Services include care for common illnesses and injuries, labdraws, routine physical exams, immunizations, and health assessment coaching.

When it pursued this endeavor in 2013, the City of Rockford contemplated that it would expand its near-site clinic and/or its facilities to community partners. The current clinic is located at 120 N 3rd Street,Rockford, IL 61107, in a renovated space that used to serve as the “showroom” for a car dealership.Approximately 2,433 square feet of this part of the building could be retrofitted and utilized by acommunity partner to conduct operations. More significantly, the building has approximately 15,240square feet of unused space in the “service department” part of the old dealership that could berenovated for the operations of community partner(s). The building also is surrounded by several parkinglots that could be utilized in a new endeavor.

While the City intends that the ultimate renovation of the facility at 120 N 3rd Street will complement itsnear-site clinic in general areas of health and wellness, such future community partnership(s) do not haveto be directly linked to the near-site clinic and can be open to the general public. In similar near-site clinicsacross the country, such facilities also host operations in areas such as physical therapy, chiropractic,pharmacy, vision and optometry, occupational health, orthopedics, behavioral counseling, gym space,laboratory and radiology, kitchen coaching, and/or general restaurant space.

As part of this process, the City is also interested in gauging interest in community partners that mightwish to offer near-site clinic services to their employees as part of an addition to the City’s current near-site clinic facilities.

Goal of this RFIGather information and assess the ability of respondents that wish to initiate operations in some, or all ofthe remaining available space at 120 N 3rd Street. It is intended through this RFI process to select firm(s)as finalists and with whom we will continue moving forward.

Implementation DateThe City of Rockford is requesting information in order for respondents to build out the remaining spaceat 120 N 3rd Street for an opening in 2019.

To that end, the City of Rockford is seeking a partner or partners to: improve the health and wellness of our employees, families and community aid the City in leading both our citizens and providers in health and wellness stimulate development in our health and wellness economy

We intend to create a facility with entities that can partner with our existing clinic that will serve as a hubof health promotion, coaching, lifestyle education, disease and condition management, as well as toensure our members continued access to high quality services and serve as a place where the communitycan actively engage in health and wellness.

Architectural Drawings and ConstructionAttached to this RFI are preliminary architectural drawings that show available space and certain ideas forbuilding out the facility. Bidders should not feel limited to these ideas but rather use them as examples.We encourage creativity and collaboration.

The City of Rockford intends to plan the next stage of this project in coordination to the responses to thisRFI, including a determination about the extent to which the City will be involved in the final architecturaldesign and the build-out of the future facility.

4.8.1 PHYSICAL AND PHYSIO THERAPY: The COR is strongly considering adding this therapy tocompliment the services provided through the near-site wellness center. With an activeworkforce of fire and police as well public works persons, personnel are subject torepetitive use and fatigue related injuries and musculoskeletal disorders. It is the COR’sdesire to find a partner that can offer a comprehensive approach, will assist in addressingroot cause issues, assist in developing effective communication, education, andworkplace protocols where you can observe and educate employees in their real-lifesituations. The partner selected should be able to provide an integrated approach of

Also, the COR initiativeis founded upon data and accountability: The partner selected should be able to providereporting and data, i.e. track clinical outcomes, report financial impact of servicesprovided, report results and trends, etc.
