1. ( SHNF) Appointment You are hereby appointed as an investigating officerpursuantto Army Regulation (AR) 15-6 to investigate the facts and circumstancessurrounding allegations that a Task Force kinetic strike against ( b )(1 ( )conducted on 28 July 2015, resulted in civilian casualties (CIVCAS) You mayincorporate all information and evidence previously collected into this investigation. Thisinvestigation is your primary duty and takes precedence over all other duties assigned.

2. (U Legal Orientation. Before you begin your investigation, you must receive abriefing from the TF Legal Advisor, (b)(3) (10 U.S.C. ), ( ) 6 ), (b ) He will

serve as your legal advisor during the investigation Consult with him regardingallaspects of this investigation , including preparing findings and recommendations.

3. (U Procedures. You are to conduct this investigation using the informal proceduresoutlined in Chapters 3 and 4 , AR 15-6. Thoroughly document all witness interviews inwriting using DA Form 2823 , Sworn Statement Ifat any time during interviews orcollection of statements you suspect misconduct, consult your legal advisor and, beforecontinuing, advise the individual of his or her rights by executing a DA Form 3881 ,Rights Warning Certificate .
