About this Document

The Michigan Department of Health and Human Services (MDHHS) is pleased to announce that Clinical Quality Measure (CQM) submissions for the Promoting Interoperability Program (formerly known as Meaningful Use or the Medicaid EHR Incentive Program) will now be done electronically, utilizing files derived from your electronic health record system and submitted to the Clinical Quality Measure Reporting and Repository Service (CQMRR®, pronounced “Skimmer”).

This guide was developed to assist providers in:

1. Registering/Onboarding with MiHIN

2. Test data submission (submitting a test file)

3. Production/Final data submission (submitting a final, production file)

4. Data Aggregation for multiple CAT 3 QRDA files

Registering/Onboarding with MiHIN

To submit CQMs via CQMRR® requires a provider or their representative to register/onboard with MiHIN. If the registering/onboarding process was completed previously, you do not have to complete this process again.

**Please remember that after a production file has been successfully submitted, providers must still log in to the EHR MIPP subsystem of CHAMPS to incorporate the CQM data, submitted via the CQMRR® process, into their Medicaid Promoting Interoperability registration/attestation.

As shown in the screen shot below, select the “Click here” link that is highlighted to register/onboard.

Note: A single user account may submit for multiple providers and/or groups. Each separate associated CMS Registration ID is not required to register.

On the registration screen, complete the required fields and click “Register”.

After successfully registering/onboarding, all future submissions will be made by logging in at the following page https://healthdirectories.force.com/QMI/s/login/. If you are unable to log in, due to an expired password, please update it at this point.

After logging in, you will begin on the screen below. To submit a test file, or to submit your actual production file, please click on the “Submit eCQMs” tab.

Test Data Submission After registering/onboarding and logging in, you will be taken to the following screen where users can upload a test file or submit their production file to the State of Michigan. Although it is not required, it is highly recommended that providers upload a test file to help determine if there are any potential formatting issues which may delay the actual submission of their production file to the State. A test file submission is still recommended even if there was a successful production file submission in a prior year.

To upload a test file, click “Test” (1) and follow the instructions (2). You may either drag your file or click the “Select Files” icon to upload your file. Please make sure your file is in a .XML file extension format (3). Click “Upload Files” to submit the file selected (4). It is important that you remember whether the file uploaded is for an individual, or for a group, so that the correct selection can be made in the EHR MIPP subsystem of CHAMPS where this data will eventually be populated.

After clicking “Upload Files” you may see the following pop-up. Click the “Ok” button to submit the file(s).

After submitting a test file, you can check it’s status by clicking the “Upload Status” tab (1) which will list the file(s) that were uploaded (2). After the file is analyzed, check the “Status” and “Status Message” (3) to see if the file passed validation. The validation process should only take a few minutes. If the file did not pass the validation process, please reference the email that will be sent to the email address on file for additional details.

After the file is analyzed, and within a few minutes you should also receive an email that lists the file(s) that were submitted along with their status. In the example below, the test file submitted passed the validation process.

In this example, the test file uploaded did not pass validation.

If the file submitted does not pass the validation process, please contact MiHIN for additional assistance at [email protected] and reference the Transaction ID of the file that did not pass the validation process. Additional test files can be submitted, if changes were made. For any QRDA file submission that is not easily corrected, MDHHS will work with these providers on an individual/case-by-case basis, but only after the registering/onboarding and test file submission process has been completed with MiHIN. Production/Final Data Submission Once testing is complete, the file that contains data for submission as part of the Promoting Interoperability (formerly known as Meaningful Use or the Medicaid EHR Incentive) program will need to be uploaded.

This process is the same as submitting a test file, but this time requires clicking on “Submit to State”(1), following the instructions (2), dragging your file or clicking the “Select Files” icon(3), and finally clicking “Upload Files” (4) to submit the file selected.

Once this file has been uploaded, analyzed, and an email is received confirming that it passed the validation process, the provider will then need to log in to the EHR MIPP module of CHAMPS to incorporate the eCQM data into their Promoting Interoperability registration/attestation. Instructions for this process are contained within the EP State Registration Guide available under the Resources section on the https://michiganhealthit.org website.

Data Aggregation for multiple Individual CAT 3 QRDA files For providers who have multiple Individual QRDA files, data aggregation may be able to assist in combining these files in to one useable aggregated Individual CAT 3 QRDA file for the Medicaid Promoting Interoperability program. On the home page, click “Services” and then from the drop down, select “CAT 3 Aggregation”.

To request data aggregation service, read the information at the top of the screen, complete the required information, and click the “Upload File” link.

After clicking the “Upload File” link, the following pop up will appear. Upload the QRDA files that need to be aggregated. These files must be a .XML file format.

Once the files have been identified and uploaded, click “Done”.

The files that were uploaded will now be displayed on the bottom of the CAT 3 Aggregation Service screen. Confirm that the correct files to be used are displayed, then click “Submit”.

Upon a successful submission, a confirmation of your request will be displayed at the top of the screen and a Case Number will be assigned.

An email will also be sent containing the case number.

After the aggregation of the QRDA files is complete, the completed/aggregated file will be returned to the email address listed, assuming there were no issues with the file. This aggregated file will still need to be submitted, following the steps outlined earlier in this document under, Production/Final Data Submission. If any issues are identified in the aggregation process, a follow up email will be sent with further instructions.
