Practice Nurse – Hedge End Medical Centre

Thank you for your interest in joining the Practice Nurse team at Hedge End Medical Centre.The Practice is committed to delivering high quality patient care which requires us to have a supportive and cohesive team.We have a number of changes in our nursing team coming up and we are looking for people with the right skills to join the team. The position is to start as soon as possible. If you are interested in the role and can demonstrate experience or skills in one or more area we are looking for, we would be pleased to have your application.Experience of General Practice is preferred; we are particularly interested in nurses with Chronic Disease experience.

One of our diabetic nurses is shortly taking maternity leave, so we are looking for temporary support in this area.

We are looking for 8 hours a week respiratory (asthma and COPD). We are looking for a new Nurse Team Lead. The role currently involves rota and cover

organisation for the team, PGDs, immunisation and flu lead, ensuring training and updates are maintained, ensuring stock levels are kept (with support), organising Nurse meetings and various other roles. We would also like this role to include appraisal lead for the nursing team.

The role will include some extended hours working including evenings and weekends.We are ideally looking for around 24 hours a week over 4-5 days, but we are happy to consider a job share for these hours, and can be flexible for the right person.We would welcome visits to the practice – please arrange these with the Operational Manager, Jenny Farmer.If you believe that you have the skills and enthusiasm for the task, we very much look forward to receiving your application.

The Practice

The Practice is situated in the village of Hedge End, which has excellent amenities, schools, housing and transport links. We are a few minutes from junctions 7 and 8 of the M27.

Our list size is just over 14000, and our patients are concentrated in Hedge End and Botley. Our aim is to provide high quality, accessible medical care to our patients, whilst maintaining a supportive and pleasant working environment for clinicians and staff.

The history of the Practice goes back to 1959 when a house was built at 26 Lower Northam Road and the founding GP commenced practicing. In 1988 number 24 was purchased and the Medical Centre was established by converting and joining the two houses. Number 28 was later purchased and opened in March 1995.

We are part of West Hampshire Clinical Commissioning Group, and offer Primary Medical Services to our patients. .

We are a training practice, and take registrar doctors and also teach medical students from Southampton School of Medicine.

The practice has seven Partners. The Partnership team is Dr Bush, Dr Padday, Dr Hollands, Dr Cropley, Dr Graham, Dr Heal and Dr Yates.

We employ a salaried GP, an ANP, a Nursing team of eight, and an admin team of thirty.

The practice uses Emis web as a clinical system. We are paperlight, so scan all incoming correspondence onto patient’s medical records.

We are a modern, thriving practice with an innovative outlook, an increasing patient list, and a real desire to care for our patients.

JOB DESCRIPTION

JOB TITLE: Practice Nurse

REPORTS TO: The Partners (Clinically)The Practice / Operational Manager (Administratively)

HOURS: 24 per week

Job Summary:

Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients.

The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately.

Clinical Duties and Responsibilities:

Management of Chronic Disease

Identifying significant abnormalities Diagnosis, monitoring and development of individual management plans; agreeing

these as appropriate with the patient and other health professionals Ensure that all Quality and Outcome Framework (QOF) targets are achieved

Therapeutic Monitoring

Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach

Recognising abnormalities Identifying the impact of treatment and implementing or recommending changes as

appropriate.

Wound Care and Management

Assessing routine wounds including trauma Assessing [and prescribing] to support good wound management in complex

situations (e.g. use of Doppler technique)
