Record Notes on Level 03 OFFSITE Mock Drill Conducted At BORL-COT Vadinar, Jamnagar, Gujarat 1 SCENARIO FULL SURFACE FIRE IN CRUDE OIL TANK FR-01D 3 DATE & TIME 14.03.2014 @ 10:50 Hrs. 4 LOCATION TANK-FR-01D Crude Oil Tank Area As per MSIHC rules 1989, an Off-site mock drill (Level-03) was conducted on 14.03.2014 at 10.50 hrs. The Scenario was manual cleaning and painting job is being done on the wind girder & roof of Crude Tank FR- 01D.Suddenly smoke is observed on the tank roof by the Contract workers (02 nos.) and they rush down the tank from the staircase and fallen down in a hurry and became unconscious. Upon noticing the smoke on the roof of Crude Tank- D, the operator breaks the Manual Call Point (MCP) and initiates emergency and also informs on PA system to Fire control room. Immediately the first turn out taken and arrives at site. With fire in rim seal first response started & on instruction of IC Level -1 siren was raised. The emergency team upon reaching the spot observed the rim seal fire was spread into full surface tank fire and decision was taken to call for additional fire tenders and Mutual Aid Support. 1 SCENARIO FULL SURFACE FIRE IN CRUDE OIL TANK FR-01D

3 DATE & TIME 14.03.2014 @ 10:50 Hrs.

4 LOCATION TANK-FR-01D Crude Oil Tank Area

As per MSIHC rules 1989, an Off-site mock drill (Level-03) was conducted on 14.03.2014 at 10.50 hrs. POSITIVE OBSERVATIONS:

Fire water header pressure maintain between 8.5 to 10.5 kg/cm2 during mock drill. Water level in fire water reservoir maintained up to the mark. Good communication was observed between fire pump operator, fire control room and IC. Good knowledge of operational safety was exhibited by crude transfer from ill fated tank based on

temperature monitoring of crude inside. Co-ordination between all co-ordinators was clear & effective. Tank shell cooling of Tank-D and adjacent tanks was effective. First mutual aid fire tender from IOCL was reported to site within 10 minutes of declaring level 3

Emergency.

First casualty was rescued, kept in Ambulance and taken to KPT hospital. 10.55

Crude transfer was started from the tank by lining up another tank F as the temperature of crude was < 50 degree C

SMC arrived in ECC (Port Control Room) and took charge of Incident. 11.00

Fire Tender FT-01 was started foam pouring to Tank from Southside. 11.01

Rim seal fire of Tank D was converted into full surface tank fire and HVLR of Tank D was started from North side.Level-3 was declared as situation was out of control.

IC informed SMC about the requirement of more fire tenders from Reliance, ESSAR, IOCL and DCC.Request to send fire tenders was passed on to neibhouring industries.

Upon request of additional manpowers from IC to SMC, ECC informed Fire & Security departments to arrange additional off duty manpower from camp.

District collector, Insp. of Police, Assist. Director-DISH visited to the site along with Sr.VP-COT & VBPL to assess the situation.

Second casualty was taken to KPT hospital in Ambulance. 11.14

Control Room Officer(CRO) informed at Thamna, Bewani and Bina station through CCTV net work that due to emergency situation they have stopped crude supply.

5. AREAS FOR IMPROVEMENT:

IC asked Security co-ordinator to shift non-essential workers at main assembly point near at Fire Station immediately.

Extra manpower from fire and security camps were reported at Fire Station & left to the site for help.

SMC asked to IC for any further help. 11.16

SMC confirmed for information passed to MAS and Govt. Officials. 11.17

Tank- D MOV and mixer power supply was switched off from substation

One Fire tender and one Foam Nurser from ESSAR were reported at site.

BORL FT-01 came to Fire Station for replenishing foam compound. 11.23

Message received from IC to SMC that Tank D fire was extinguished and Tank F shell cooling was stopped. Tank D & Tank C cooling continued.

Two firemen were sent for inspection of all around Tank D for inspection & checking if anything abnormal.

Reliance Fire Tender reported at site and kept as standby. 11.28

GSFC Fire tender reported at site. 11.33

All clear message was confirmed by IC from SMC. All clear siren blown.

Jamnagar Municipal Corporation (JMC) Fire Tender was reported at site.

Inside Firewater pump house: RCC slab not properly covered near pump 106 to the cable tray on flooring which may result injury to fire pump operator during emergency operation of fire pumps.

Opening near pump 106 shall be covered with grating or metallic plate.

Fresh labeling of all fire water pumps shall be carried out for easy identification.

In case of L3 emergency station ESD may be used to avoid delay in shutdown operation.

Suitable instruction may be given to close station ESD during L3 emergency.

Roof drain of Tank FR-01D was not closed and dyke valve also found in open condition.

In full surface tank fire condition roof drain of ill fated tank shall be closed, however, dyke valve shall be closed immediately.

From the foam pourer pipes of Tank FR01D, water was leaking due to drop in pressure, it may slow down fire extinguishing activities.

Foam pourer line shall be checked for any blockage.
