Access the NCQA Store
Access the NCQA Store to purchase a multi-site application/ISS Survey tool by following these steps:
A. Enter store.ncqa.org into the address window of your internet browser or search engine.
B. Select Log In beside the Search engine at the top of the page.

Log-in / Registration
Log-in to the NCQA Store account under Registered Customers with your e-mail address and password.
Select Create an Account and complete the form, if you do not have an account.
NOTE: If you have access to another NCQA site, e.g., Policy/Product Clarification Support (PCS), you may use the same log-in information to log into the NCQA Store.

After completing the registration form, select My Account to confirm your NCQA Store account information in correct.

Ordering a Recognition Product
A. Under the Recognition tab, select the applicable program, e.g., Patient-Centered Medical Home (PCMH), from the dropdown menu.
B. Use the Search engine to locate the appropriate product, e.g., 2014 PCMH Online Application, then click Add to Cart.

C. Once item moved to the Shopping Cart:
a. Confirm the correct product has been selected.
b. Confirm the quantity under the Qty column.
c. Enter any applicable Discount Codes, then click Apply Voucher.
d. Get Shipping & Tax estimate quote, by selecting Get a Quote.
e. Click Proceed to Checkout.

D. After selecting Proceed to Checkout, you will be required to Confirm the following:
a. Billing Information.
b. Shipping Method.

c. Payment Information.

E. Review and confirm order information under Order Review.
F. Select Place Order.
G. Log Out, after confirmation order has been received by NCQA.

E-mail Confirmation of Purchase
You will receive confirmation e-mails from NCQA Sales <[email protected]> that includes your order information.

Online Application Tool Log-in
The link to the Online Application Tool, https://recognitionportal.ncqa.org/rponlineapp/Login.aspx, will be included in your confirmation e-mail.
A. Log-in with your full e-mail address and assigned password.
B. Select “I agree”, click Submit.

C. Under My Accounts, click on the applicable account.
NOTE: Account name should be the same as the organization name. If the name requires editing following these steps:
Step 1: Click Account Manager tab.
Step 2: Click Edit in Account Information window and update the information.

Submit Multi-Site Request Form
A. After entering the account, click the New Multi-Site / Multi Specialty Application button.
B. Complete the Add Multi-Site Request form.

C. Click Save.
D. Your Multi-Site Request will either be accepted or declined depending if the responses meet
the eligibility criteria. However, the final determination will be made during the Multi-site Call.

Add Practice Sites to Multi-Site Application
1. Log back into the Online Application Tool. (see pg. 11)
2. Click the Practice Site tab.

3. Click the Add Practice Site link to enter the information for each practice site.
4. Complete the form for each practice as thoroughly as possible.
5. You may edit the practice information by clicking on the name of the practice.

Multi-site Call
A. After the Multi-site Request Form is submitted, an RP Administrative Analyst will be in contact to setup an appointment for a call date and time with you and the assigned RP program manager. The call is designed to confirm that the practices have at least the minimum number of sites to meet the eligibility criteria.
B. During the Multi-site Call, the RP program manager will re-confirm multi-site eligibility. If eligible, the multi-site application and survey process is explained in detail and the request status is changed to Approved.
C. After the Multi-Site Call, the request status on the Multi Site Manager page will be changed to Approved.

1. After approval, open the Multi-site Group window, click on the EHR product name (default).
2. Click in the Group Name field and replace the EHR product name with the correct [Multi-Site Admin] Name.
3. Click Save.
4. Reenter the Multi-Site Group window and check the boxes of the practice sites that are a part of the Multi-Site survey.
5. Click Save.
NOTE: If more practice sites need to be added to the multi-site, return to the Practice Site tab and enter the additional sites. Make sure to only add sites that correspond to practices that have identical EHR systems.

6. Under Multi-Site Groups, the [Multi-Site Admin] Name will be updated as well as the number of Site(s) to coincide with the practice sites selected in the Multi-Site Group window.

Order Multi-Site ISS Survey Tools
1. Click the Multi-Site Manager tab on the top menu bar.
2. Click the name of the Multi-site Group.
3. In the Multi-Site Group window confirm the boxes for all the appropriate practices are checked.
4. If not, check the boxes and click Save.
5. Under Multi-Site Forms, click Survey Tool Order Form.
6. Complete the Practice Site list form:
a. Check the boxes of the sites requiring an ISS Survey Tool and enter payment information.
b. Print and Fax, 202-955-3599 (ATTN: Customer Support), OR Scan and E-mail,
IMPORTANT NOTE: If you purchase an additional product, e.g., PCSP 2013, and have 3 or more
practices with a different Electronic Health Record (EHR), those practices will be listed with the
other practice sites list in the Multi-Site Group window. Unselect the previously checked boxes
and select only those pertaining to the new product or EHR system.

7. To calculate the cost of your Multi-Site survey, click Fee Calculator under Multi-Site Forms.
8. Follow the instructions in the fee calculator.
