3) Once on this page, you will see 2 bold-text headlines (see red arrow and box below) entitled:

NON-GAMING VENDOR FORMS (4 non-gaming Vendor Forms are listed) NON-GAMING VENDOR LINKS (2 information links for non-gaming vendors are listed)

4) Under NON-GAMING VENDOR FORMS, a vendor will need to select only one* applicable Vendor Form, depending upon the $ amount of business the Vendor intends to conduct with a casino.

[* Form 1024 – Vendor *Emergency Services* Notification Form - is NOT TO BE USED by any vendor applicant. Use of this Form is reserved for Casino Representative personnel ONLY]. Please note that only these three (3) Vendor Forms are applicable to vendors during the application process.

Form 1021 – Vendor Certification Form Click on the [PDF] and note that to complete this Form, a vendor applicant:

- Must be contracted to provide $100,000 or more in non-gaming goods & services; - Pay a non-refundable $500 application fee to MLGCA (see Page 3 for details); - Obtain and submit a signed and notarized Page 13 from a Casino Representative.

******************************************************************************** Form 1021CC – Vendor Certification Form (Construction Version) Click on the [PDF] and note that to complete this Form, a vendor applicant:

- Must be contracted to provide $100,000 or more in Construction business; - Pay a non-refundable $500 application fee to MLGCA;

- Obtain and submit a signed and notarized Page 13 from either a Casino Representative -OR- an Authorized Construction Representative for the Casino. ******************************************************************************** Form 1023 – Vendor Registration Form Click on the [PDF] and note that to complete this Form, a vendor applicant:

- Must be contracted to provide between $2,500 and $99,999 in non-gaming goods & services; - There is NO FEE to apply for Vendor Registration; - This is a 3-page document and the bottom of Page 2 must be completed/signed by a Casino Rep; - Page 3 of the document must be notarized.

It is the responsibility of the Registered Vendor to know when it is approaching the $100,000 mark of the contracted $ amount of business conducted with a casino - or multiple casinos. Once the Vendor approaches or reaches the $100,000 business mark, the Vendor must then apply to *UPGRADE* its status with MLGCA, from Approved Registered to Approved Certified Vendor status, and complete a Form 1021 – Vendor Certification Form – and pay a non-refundable application fee of $500 to MLGCA. *******************************************************************************

Once a Vendor is approved by MLGCA, that vendor is then authorized to conduct business with any / all casinos in the State of Maryland with whom they have established a contracted Business Relationship with, for a 5-year period, which time span begins on the date that the Vendor is approved by MLGCA.

5) Under NON-GAMING VENDOR LINKS, a vendor will see 2 selections. The first selection is:

Vendors – a “How To” Guide (2 pages long)

It is imperative that every prospective non-gaming vendor applicant thoroughly read the information presented within this 2-page document. This document addresses the following information: - The requirement of adherence to Maryland Law, per COMAR 36.03.02.17; - The definition of a Vendor according to COMAR 36.03.02.17; - The definition of a business relationship between a vendor and a casino (facility); - The 4 sequential steps that must be adhered to before a Vendor Form will be processed in our office; - The requirement that all businesses must be listed as having “Good Standing” status with the

Maryland State Department of Assessments and Taxation (MD SDAT). [Note: There are only three (3) exceptions to the requirement that a business entity possess “Good Standing” status. The business must be one of the following: a Sole Proprietorship; or a Single-Member LLC; or a General Partnership. When any of these 3 types of ownership status apply, then the vendor must submit a Federal Tax Form W-9 in lieu of a Good Standing Certificate with MD SDAT]. 7) The next drop down is APPROVED VENDORS.

This drop-down provides the public with an alphabetical listing of Vendors approved, by the Maryland Lottery and Gaming Control Commission, to conduct business with any / all licensed Maryland casinos.

(NOTE - Later this year, this list will be improved to include not only the Approved Vendor’s name, but also the type of Vendor they are [Registered, Certified or Certified-Construction], the expiration date of

their 5-year approved vendor status, and a brief description of the goods and services they provide to facilities (casinos) in Maryland).

Under the COMAR regulations, certain suppliers of a few, specific non-gaming goods and services have been deemed exempt, but in general, most suppliers must be Registered or Certified to conduct business with a Maryland Casino.

10) No prospective vendor applicant may submit any Vendor Form to our agency without first having a contracted Business Relationship established with one initial casino.
