The Georgian Associatio ofTranspla tologists was founded 10years ago, on January 31, 1997.More than 50 doctors and fourmedical organizations participated inthe foundation.
The aim of the association is tosupport the development of organtransplantation in Georgia. TheAssociation has been headed byprofessor Gia Tomadze since the dayof its foundation.
The Association is located in Tbilisi,on the territory of City Hospital 1,General Surgery Clinic.

NTR is one of the main subdivi-sions of the Association, whichwas created in 1997 headed by Dr.Ani Devnosadze untill 2003 and byDr. Alisa Kireulishvili since 2003.
The NTR contains 5 registries:
1. Transplant registry;2. Waiting list;3. Donor registry;4. Donor refusal registry;5. Living donor registry.
Using the NTR electronic data ba-se (www.gat.com.ge) the transplantpatients follow-up has beencarried out, as well as statisticaland clinical analysis of performedtransplantations. In cooperationwith relevant institutions themedical insurance policies hasbeen issned. The monitoring oflaboratory and other medicalparameters has been done in a"Diary of patients with transplantedorgans", We perform control andregistry of the immunosuppre-ssive medications (Neoral, Cell-cept, Myfortic etc.), purchased byGovernment.
The certain laboratory investi-gations such as CBC, urinanalysis,creatinine, urea, etc. are free ofcharge for the patients with trans-planted organs and their donors.

The total number of the transplan-ted patients during the last 10 yearsperiod was increased for 4 times by2007. In 1995 it was 20, in 2007 be-came 89.
It should be mentioned that annu-ally there are about 2-3 patientsregistered who were operated for ab-road, which we take into considera-tion when carrying out calculationsfor medicament provision.

In 2000 the Parliament of Georgiahas passed a law on organ transplan-tation, which is based on informedconsent. According to the law, anorgan donor can be a genetic relativeor a spouse. The amendment werereceived in 2002, following to what adonor of an organ can be a personwho is non relative, but involved inthe organ exchange program .
Currently, kidney, cornea and bonetransplantation is performed inGeorgia. Each year 9-10 kidney trans-plantations are financed by the stateprogram.
In 1977, together with the collegesfrom Moscow the first kidney transpl-antation was performed at the Insti-tute of Surgery. Since then, 87 kidneytransplantation operations have beencarried out utill now. Among them 59operations were done in the NationalCenter of Urology (Dir. Prof. L.Mana-

In the recent period, along with theincrease of experience, a significantpositive dynamics had been observedin the field of technical complicationsafter kidney transplantation, as wellas from the point of view of the let-hality of the recipients. If we considerthem by stages, we shall see that in1995-99 the indicator of lethality afteroperation was 27%, while in 2000-2007it decreased till 5%.

According to the law of Georgia onorgan transplantation of 2000 the liverelated donation as well as cadaverdonation is allowed. But no cadaverdonors have been used for transplan-tation yet.
The main reason for this is negati-ve approach of population to cadave-ric donation, which is time consumingprocess and needs active invol-vements of mass media.
In this respect the patients withdiagnosis of brain death are the bestdonors for cadaveric transplantation.The reason for that is the retainedblood circulation which has a positiveeffect on graft's postopera tivefunctioning.
The criteria of brain death have be-en worked out in 2000, and it's de-finition has been included in the lawon Health Care of Georgia since 1997.

However, there was no cases ofofficial, legal diagnosis of brain deathin Georgia. The criteria for thisdiagnosis includes the clinical andinstrumental approoval twice, withminimum of 12 hour intervals (EEG ortranscranial doppler).
Since 2007 the mobile team ofdoctors has been created, who areinvolved in evaluation of official diag-nosis of brain death.
During the last 18 months 12 patientswith suspected diagnosis of braindeath were tested and in 2 cases thisdiagnosis was prooved officially.
The medical professionals as well asthe population need to change theirattitude and become adapted to thisdiagnosis.

The representatives of CatholicChurch accept the organ transplanta-tion more positively than the Ortho-dox believers. The correspondancebetween the representatives of theOrthodox and the Catholic Churchvary to a great extent: 100% of the Ca-tholics are for deceased donations,and only 18% of the Orthodox Churchis for such type of operations.
The outcomes of these researcheswere reported at various internation-al forums, and has become the basisfor working out the relevant legis-lation.
At present social research iscarried out, the respondents of whichare the live relative donors of thepatients with transplanted organsresiding in Georgia. The results of theresearch will be published in 2008.

For the last 10 years one of the maindirections of the Association waselaboration of the legislation. In closecooperation with the legislative groupof the Ministry of Health (G. Kiknadze,G. Javashvili, G. Simonia) and theHealth Committee of Parliament wehave worked out corrections andamendments during the preparation ofthe law of Georgia on organtransplantation. The Associationworked out all the law-abiding actsstipulated by the law on organtransplantation.
In 2000 with the active participationof the Association, the criteria of braindeath were worked out, which wereapproved by the order of the Presidentin 2001.

In the same year provisions onexport-import of transplant organs,Transplant Council, Information Centerwere worked out and recommendationson the principales of crossoverdonation have been prepared.
Our Association actively cooperateswith international organizations suchas the Committee of the Experts ofCouncil of Europe in transplantation,the European Board of the WHO, theEuropean and International lAssocia-tions of transplantologists.
In 1999 the 1st Congress of theTransplantologists of the Caucasiancountries was organized by theassociation, and the representatives of8 countries participated (Armenia,Russia, England, Greece, Azerbaijan,Czech Republic, Israel, Georgia). Thecongress materials were published.
By the initiative of the GeorgianAssociation of TransplantologistsGeorgia was entered as a member stateof the European transplantation mapand since 2000 the activity of Georgia inthis direction has been reflected in thejournal published by the Council ofEurope.
Association organized workshopson brain death held in 2000 and 2006,with participation of neurologists andICU doctors from the leading clinics ofGeorgia.
In 2005 by the initiative of theAssociation, and under the aegis of theCoE, a regional workshop dedicated tothe organizational problems was held inGeorgia, in which, apart from Georgia,participated the represen--- tatives ofArmenia, Azerbaijan, and Ukraine. Onthe basis of the results of the workshoprecommendations of the CoE expertsCommittee for the governments of theparticipating countries were prepared.

At the present stage our Asso-ciation considers the establishmentof the national program for the deve-lopment of transplantation in Geo-rgia to be the main direction of thesecond decade of its functioning.
We hope that the relevant Gover-nmental bodies will support the re-commendations of the CoE and WHOof 2005-2006 for creation of stateprogram for the development oforgan donation and transplantation.
Real and concrete steps are nece-ssary to start cadaveric donationprogramm.
Our work has to go over to a newstage. It must become more global,including the organ, as well as tissueand cell transplantations. The tissueransplantation banks must be crea-ted. Concrete steps must be taken inthe direction of the development ofother organs, tissues and cells apart
