Who can be covered? Everyone without access to major medical can be covered. Employees, spouses and families can be covered by Limited Benefit Medical and the optional Dental and Term Life coverages. Employees only can be covered by the Short Term Disability coverage.

Do employees have to use certain doctors, dentists, or hospitals? No. Employees have access to use any licensed doctor or dentist, or any certified hospital. However, under the Limited Benefit Medical plans, employees can save money by using a network provider.

Are visits to the chiropractor covered under the Limited Benefit Medical plans? Yes, chiropractic office visits are covered; however, chiropractic adjustments, manipulations or modalities are not covered.

Optional Benefits Enhance the PlanDental Rider

• Employees may visit any dentist; no network restrictions

• Covers most common services

• Type I - Preventive and Diagnostic (80% of charges; no waiting period)

• Type II - Basic Services (60% of charges; 3 month waiting period)

• Type III - Major Services (50% of charges; 12 month waiting period)

• Offers enrolled dependents the same coverage

• $500 plan year maximum per person and $50 plan year deductible per person for all services combined

Term Life & Short Term Disability

The Term Life benefit includes a $10,000 employee life benefit and matching $10,000 accidental death benefit. Spouse and dependent child coverage is also available. Employee benefits reduce by 50% at age 70. Spouse coverage ends at age 70.

The Short Term Disability benefit offers 26 weeks of coverage when the employee is totally disabled (after a 14 day elimination period; none if hospitalized). Benefit amount is 50% of base pay up to $125 per week. Employee benefits reduce by 50% at age 70. Short Term Disability coverage is only available for employees; there is no dependent coverage.
