Driver InformationTo qualify for an Oregon driver license, instruction permit, or identification card you must be a resident of or domiciled in Oregon and be a United States citizen, permanent resident, or temporarily legally present in the United States. You will be required to provide proof of legal presence, full legal name and current residence address. See pages 9-11 for the identification requirements.

If you are a temporary resident in the United States applying for a driver license or identification card, you will be issued a limited-term driver license or identification card. A limited-term driver license or identification card will be valid for:

• Your authorized length of stay, but no longer than eight years from the date of issuance; or

• One year from the date of issuance if your length of stay is for an indefinite period of time.

If you are a temporary resident in the United States applying for a driver permit, you will be issued a limited term driver permit valid for:

• Your authorized length of stay, but no longer than the period of time for which the driver permit of the same type is issued for by the department; or

• One year from the date of issuance if your length of stay is for an indefinite period of time.

Class C Provisional License• Drivers 16 and 17 years of age• Subject to graduated licensing restrictions• Eligible to drive the same vehicles as a class C

Class C LicenseA Class C license allows a person who is at least 18 years old to drive:

• A car or any single vehicle (mopeds, passenger cars, vans, and pickups) with a loaded weight and gross vehicle weight rating (GVWR) of not more than 26,000 pounds that is exempt from a commercial driver license (CDL) or motorcycle requirements

• A recreational vehicle—including motor homes and vehicles with campers—for personal use

• An emergency fire vehicle if operated by a firefighter• An emergency vehicle if operated by a qualified

emergency service volunteer

In addition, a holder of a Class C license may tow:

• A travel trailer for personal use• A trailer with loaded weight or GVWR over

10,000 pounds, if the combined weight and GVWR of the towing vehicle and trailer is not more than 26,000 pounds

Mandatory Motorcycle Rider Education If you apply for an unrestricted motorcycle endorsement and you do not have a valid motorcycle endorsement from another state, you must complete an approved motorcycle rider education course:

Provisional Driver Licensing InformationIf you’re under 18 years of age and getting your license for the first time, you must meet the following requirements:

Requirements1. You must have had an instruction permit

from Oregon, another state, or the District of Columbia, for at least six months. Temporary driver permits do not count toward the requirement to have an instruction permit for six months for a provisional driver license.

2. You must be able to certify that you have had at least 50 hours of driving experience while supervised by someone at least 21 years of age who has had a valid driver license for at least three years, AND show that you have completed an ODOT-approved driver education course; OR certify that you have had 100 hours of supervised driving experience.

3. Your parent or legal guardian must certify you meet school attendance or exemption requirements.

4. Your application must be signed by your parent or legal guardian.

NOTE: If you are under 18 and have been issued a driver license by another state and you surrender that license when applying for an Oregon driver license, you are not subject to requirements 1 and 2 above.

Commercial License or Commercial Learner Permit• You must be at least 18 years of age to drive in

intrastate commerce• You must be at least 21 years of age to drive in

interstate commerce• Have a class C non-commercial or higher

Testing FeesYou must pay test fees prior to testing. If you fail a written, drive, or skills test, you need to pay the test fee again each time you take the test. In addition, DMV requires you to present proof of identity and date of birth every time you test.

Check office hours prior to taking your test. Offices do not conduct knowledge tests if the applicant is not assisted at the counter at least one hour before closing times. In addition, smaller offices may close during lunch hours. www.oregondmv.com has more information on office testing hours.

Taking the Drive Test • You must pass the knowledge and vision tests

before you make an appointment for a behind the- wheel drive test.

• DMV may waive the drive test if: ○ A student between 15-17 years of age has successfully completed a program from an ODOT approved driver education provider and has been issued a completion card that waives the drive test. This card will be acceptable for up to two years from the issue date.

• If you have a valid out-of-state driver license, the drive test may be waived.

• Call to schedule your appointment for a drive test up to 30 days in advance or you may also take drive test scored by an approved business.

• You may also schedule a drive test through a Class C non-commercial third-party drive testing business.

NOTEDMV requires that you pay for tests in advance. If you are taking a test, please bring a separate payment (for example, two checks): one for your test(s) and one for issuing your license, or instruction permit.

Legal Presence RequirementWhen you apply for an original, renewal, or replacement driver license, driver permit, or identification card, you are required to present proof of legal presence in the U.S.

PHOTOCOPIES ARE NOT ACCEPTABLE.Be certain you know the most current acceptable proof of legal presence, identity, date of birth, full legal name, SSN and residence address before you come to DMV. NOTE

All documents submitted must be acceptable to DMV. DMV has discretion to require additional evidence to verify legal presence, full legal name, identity, date of birth, SSN, or current residence address.

Acceptable Documents For Proof of Legal Presence, Full Legal Name, Identity & Date of Birth• A U.S. government-issued birth certificate:

○ DMV will not accept birth certificates issued by a non-Government agency, such as a hospital birth certificate, birth card, birth registration, or a baptismal certificate;

○ Laminated birth certificates are not acceptable.

• A U.S. consular Report of Birth Abroad (FS-240);• A U.S. government-issued Certification of Birth

(DS-1350 or FS-545);• Request for Verification of Birth (DD372) must

include verification from Vital Statistics with appropriate endorsements;

• Valid U.S. Passport or Passport Card, cannot be expired more than 5 years;

• Valid U.S. Territory Passport, cannot be expired more than 5 years;

• Tribal ID card from or affiliated with an Oregon federally recognized tribe, approved by DMV;

• Valid foreign passport, not expired, must include; ○ Arrival/Departure Record (I-94, CBP I-94A, current admittance stamp or I-797A) with appropriate entry endorsement;

• U.S. Department of Homeland Security documents, not expired:

○ Employment Authorization Document (I-766); ○ Certificate of Citizenship; ○ Resident Alien Card or Permanent Resident Card (I-551);

○ Certificate of Naturalization; ○ Permit to Re-enter (I-327); ○ Refugee Travel Document (I-571).

Full Legal NameWhen you apply for a driver license, instruction permit, or identification card, you must use your full legal name. “Full legal name” is the name shown on your proof of identity and date of birth document. If your name is different than the name on your proof of identity document, you will be required to present additional documents. Acceptable documents include, but are not limited to, a government issued marriage certificate/license, divorce decree, judgment of dissolution or annulment of marriage or domestic partnership, certificate of registered domestic partnership, adoption decree or a court order. Documents being presented must be original or a certified copy from the issuing agency.

Social Security Number RequirementWhen you apply for an original, renewal, or replacement driver license, instruction permit, or identification card you are required to provide your Social Security Number (SSN) issued by the U.S. Social Security Administration (SSA) on the application.

If you are not eligible for a SSN, you must present proof of ineligibility and sign a Statement of Not Eligible for Social Security Number, Form 735-7255.

The SSN on an application is confidential information. It will not be printed on your driver license, permit, or identification card and will not

be released on DMV records. Access to your SSN is allowed only to qualified government agencies.

Acceptable Documents for Proof of Ineligibility for SSNDocuments issued by the SSA, the Department of Homeland Security or other federal agencies or federal courts, which demonstrate you are not eligible for a SSN.

Proof of Residence AddressWhen you apply for a license, permit, or ID card you must provide proof of your current residence address. Oregon law allows you to use a mailing address, such as a post office box or business address, in addition to a residence address, but never in place of one.

Acceptable proof of your residence address includes:• Mortgage document;• Property tax statement;• Mail from a verifiable business or

government agency, (if mail is from Oregon DMV the resident and mailing address must be the same);

• Oregon vehicle title or registration card containing residence address only;

• Payment booklet;• Voter registration, selective service, medical, or

health card;• Utility hookup order or statement of account;• U.S. Educational institution transcript for the

current year;• Unexpired U.S. Government issued

professional license;• Oregon DMV address sticker with the same

residence and mailing address.

Changing Your Address• Notify DMV within 30 days of your move.• Address changes can be done online at:

www.oregondmv.com

• Address forms are available at DMV office self-service counters and on the Internet.

• DMV will mail your new address label to you to place on your driver license, permit, or ID card.

• There is no fee for the address label.

Organ Transplant and Tissue Donation• You may choose to have your driver license,

permit or ID card coded with a “D” to identify you as a donor.

• You must be at least 15 years of age.• There is no fee if you add the code when you

renew your license. A $26.50 replacement fee is required to add the code between renewal periods for a license or permit.

• Inform your immediate relatives of your wishes.• Ensure your intent to be a donor is known by

registering at www.donatelifenw.org.

Veteran Designation If you are a veteran you may request that DMV add a veteran designation on your license, permit or ID card at the time of application. You must present a Certification of Release or Discharge from Active Duty, Form DD214, or a Correction to DD214, Form DD215, which indicates you were discharged from the U.S. Armed Forces under honorable conditions.

Renewing Your Driving Privilege• Application forms are available at DMV offices,

through the mail, on the Internet at www.oregondmv.com or by calling (503) 945-5000.

• Renew at a DMV office.• Bring applicable proof of identity and date of

birth for the driving privilege you are renewing. Visit www.oregondmv.com.

• If your address has changed since your last renewal or replacement, bring in proof of your current residence address (see page 11).

• If you will be 50 years of age or older when your license expires, you will be required to take and pass a vision test.

• Fee: $40.00 Driver license $23.50 Instructor permit including

Replacing a Driver License, or Permit, DMV will issue a replacement driver license, permit, or identification card if:

• Your license, permit, or ID card is lost, stolen, damaged, or destroyed; or

• Your residence address changed and you choose to get a new license; or

• You have a name change; or• You are exchanging a valid-without-photo

license for a license with a photograph; or• You are replacing a license, or ID card issued

with an error; or• You are obtaining a license, or ID card after

reinstatement of a suspension; or• You are replacing a license, or ID card

confiscated by law enforcement or the courts.To be issued a replacement you must present proof of legal presence, identity and date of birth.

If you are obtaining a replacement due to name change you must present acceptable proof of a legal name change (see page 10).

If you are changing your address or your address has changed since your last renewal or replacement, you must present proof of your current residence address (see page 11).

Interim DocumentIf you meet all the requirements to obtain Oregon driving privileges or an Oregon ID card, you will be issued an interim card at the DMV office. The

permanent card will be produced at a central location and mailed to your address on record. Because you will receive your card in the mail, it is important that your address on file with DMV is correct. Customers typically get their permanent cards in five to ten days. The post office will not forward your driver license, permit, or ID card. If your card cannot be delivered to the address the DMV has on file it will be returned to the DMV as undeliverable.

Parking Permits for Persons with DisabilitiesParking permits are available if you are temporarily or permanently disabled. Wheelchair Users disabled parking permits are available to persons whose physician indicates on the application that they qualify. Permits are also available to programs and families that transport persons with disabilities and to foreign visitors. There is no fee for the parking permits.• You must have a current, valid Oregon driver

license, ID card, or parking ID card to obtain a disabled parking permit.

• Permanent disabled permits expire on the same date as your driver license, or identification card.

• Temporary disabled permits are issued for a period of not more than six months and do not require a valid driver license, or ID card.

• You will need to submit an application and a certificate of disability (Form 265) signed by a doctor. You may call (503) 945-5000 to receive a form. The form is also available on the Internet at: www.oregondmv.com. Click on “Driver Forms.”

Parking Identification CardsParking ID cards are issued to individuals who need a disabled parking permit and are not eligible for a driver license, or non-driver ID card or who are unable to appear in person at a DMV office. Parking ID cards are available only from DMV

Headquarters. You will need to submit Form 735-265NPP signed by your physician. You may call (503) 945-5114 to receive a form. The fee for a parking ID card is $44.50.

Filing an Accident ReportYou must report any accident within 72 hours if:• Damage to the vehicle you were driving is

over $2,500;• Damage to any vehicle is over $2,500 and any

vehicle is towed from the scene as a result of damages from the accident;

• Injury or death resulted from the accident; or• Damages to any one person’s property other than

a vehicle involved in the accident is over $2,500.Persons should make a copy of the report before filing it with DMV.

Reinstating Your Driver License• Requirements depend on the type of suspension.

VEHICLE INFORMATIONVehicles used on Oregon’s roads and highways must be titled and registered in Oregon, unless they are properly titled and registered in the owner’s home state. To be eligible to register a vehicle in Oregon, you must be a resident of this state, be domiciled here, or otherwise be required to register your vehicle in Oregon, as is the case for certain commercial vehicles. If you are an Oregon resident, you must register your vehicle here. To do so, you must first have an Oregon title or apply for a title at the same time you register your vehicle.

Titling and Registering a New Passenger Vehicle• Application forms are available at DMV offices,

through the mail, on the Internet at www.oregondmv.com or by calling (503) 945-5000.

• Complete and sign the Application for Title and Registration, Form 735-226.

• Be sure to include the odometer reading.• Attach a properly completed Manufacturer’s

Certificate of Origin for the vehicle.• Your vehicle may be subject to the Oregon Low

Emissions Vehicle (LEV) Standards. If your vehicle is subject to LEV, DMV will need to verify compliance prior to registration being issued.

Titling and Registering Passenger Vehicles Moved to Oregon from Another State1) Title application forms are available at DMV

offices, through the mail, on the Internet at www.oregondmv.com, or by calling (503) 945-5000.

2) Complete and sign the Application for Title and Registration, Form 735-226.

3) Be sure to include the odometer reading. You do not have to provide an odometer reading if your vehicle is ten years old or older.

4) Bring your vehicle to a DMV field office to have the Vehicle Identification Number (VIN) inspected. Fee: $7.00

5) Your vehicle must pass the DEQ emissions test if you live within the Portland Metropolitan Service District or the Rogue Valley in Jackson County. For information call Portland area (503) 229-5066; Medford area (877) 476-0583.

6) If your vehicle passes DEQ, you will be issued a Certificate of Compliance. You must present the certificate with your application to DMV if you are applying for registration.

7) Your vehicle may be subject to the Oregon Low Emissions Vehicle (LEV) Standards. If your vehicle is subject to LEV, DMV or DEQ will need to verify compliance prior to registration being issued.

8) Bring the out-of-state title to DMV. Be sure you have the signatures needed to release interest for any name that won’t be shown on the Oregon title.

9) If the title to your vehicle is in the possession of a lien holder, you will need to forward the DMV paperwork, including evidence of a VIN inspection and DEQ compliance (if required), together with appropriate title and registration fees to the lien holder of the vehicle. DMV can provide you with an instructional transmittal letter for your lien holder. Your lien holder

will forward the original title, the DMV paperwork and fees to DMV Headquarters in Salem, Oregon. DMV will return an Oregon title to your lien holder and forward to you by mail the registration card, plates and stickers.

NOTETransactions requiring documents from lien holders cannot be processed by DMV until all requirements, including receipt of the original out-of-state title from your lien holder, have been met. In order to expedite your request, contact your lien holder for correct mailing address information as your title may be held in a different location than where you send your payments.

10) If you purchased the vehicle from an out of state dealer and the odometer has less than 7,500 miles you will owe a tax of 0.5% of the sales price.

11) If the out-of-state title is either a junk certificate, junk title, or similar ownership document, DMV will not issue an Oregon certificate of title under ORS 803.045.

Titling and Registering Vehicles from Other Countries• Check with DMV Headquarters for

Notice of Sale of a Vehicle• Seller must notify DMV within 10 days upon sale

of a vehicle. • Notice to DMV should include buyer information.• Notice of Sale forms can be done on-line at:

www.oregondmv.com. Notice of Sale forms are also available from DMV or on the Internet.

Transferring an Oregon Title• Buyer must transfer the title within 30 days to

avoid a late transfer fee of $25 or $50.• Complete and sign the Application for Title and

Registration, Form 735-226.• Applications are available at DMV offices,

through the mail, on the Internet at www.oregondmv.com or by calling DMV at (503) 945-5000.

• Be sure to include an odometer disclosure signed by both the seller and buyer. You do not have to provide an odometer statement if the vehicle is ten years old or older.

• Mail or bring the title with all former owner’s releasing signatures to DMV.

• Fee: $93.00 Title fee $90.00 Title fee for motor vehicles

with GVWR over 26,000 pounds and trailers over 8,000 pounds (does not include recreational vehicles)

Renewing Passenger Vehicle Registration While Transferring TitleIn addition to the requirements above for titling:• Your vehicle must pass the DEQ emissions test if

you live within the Portland Metropolitan Service District or the Rogue Valley in Jackson County.

• If your vehicle passes DEQ, you will be issued a Certificate of Compliance. You must present the certificate with your application to DMV.

• You must provide your insurance company name and policy number. (DMV will not accept the name of your insurance agent in lieu of insurance company name.)

• Fees: $93.00 Title fee $112.00* Registration fee when

keeping your current plates• To view plates and fees, go to DMV’s website

at www.oregondmv.com or call for fee information at (503) 945-5000.

*County registration fees may also apply.

Renewing Your Registration• DMV will mail a renewal reminder to you 45–60

days before the registration expires.• If your vehicle is not in a DEQ area and you

have your renewal reminder, you can renew online up to 75 days following expiration at: www.oregondmv.com. Your registration stickers will be mailed to you.

• Complete the renewal application following the instructions and mail it using the envelope provided or you can bring it to DMV.

• Be sure to include your insurance company name and policy number on the back of the renewal application form.

• Sign and date the renewal application where indicated.

• List any address changes on the front of the renewal application. Address changes are not accepted at DEQ in the Portland-Metro area.

• Your vehicle must pass the DEQ emissions test if you live within the Portland Metropolitan Service District or the Rogue Valley in Jackson County.

• You may submit your vehicle renewal application to any DEQ station to renew your vehicle registration at the time your vehicle is being tested.

• If your vehicle passes DEQ and you don’t renew your registration at DEQ, submit the certificate of compliance form with your renewal application to DMV, or renew online.

• Submit the fee that is indicated on the renewal reminder application form.

Replacing Vehicle Plates• Applications are available at all DMV

offices, through the mail, on the Internet at www.oregondmv.com, or by calling (503) 945-5000.

• Your vehicle must be currently registered. The fees below are the replacement fees and do not include the registration renewal fee or any applicable plate surcharge fees.

• Fees: $29.00 for two-plated vehicles at the time of renewal

$17.00 for one-plated vehicles at the time of renewal

$34.00 for two-plated vehicles other than at renewal

$22.00 for one-plated vehicles other than at renewal

MISCELLANEOUS SERVICES

Trip PermitsNote: Have insurance information available.Permits are available at local DMV offices and through the mail. Fees vary depending on the type of permit needed. In general, requirements for permits are based on:

• whether the vehicle is registered• the type of vehicle• use of the vehicle

Trip permits for passenger vehicles:

• Permits are issued for a 21-day time period. • A maximum of 42 days (2 permits) in a 12-month

period is allowed for passenger vehicles. • The fee for a 21-day trip permit is $32.00.

• Trip permits for recreational vehicles:

• The permit can be issued for up to 10 days. No more than a total of 10 days may be issued in a 12 month period.

• The fee is $32.00 for each permit issued.• Person must show proof that they are an owner of

the recreational vehicle.

Sno-Park Permits• Permits are available at DMV offices and most

Oregon recreational ski areas and many ski shops.• You must have a permit to park in designated

winter recreation areas between November 1 and April 30.

• Fees are based on the length of the permit.• Fees: $4.00 for a daily permit $9.00 for a three-day permit $25.00 for an annual permit

Purchasing DMV RecordsAll persons who request DMV record information must qualify to receive personal information under ORS 802.175–802.191. The purpose of this law is to make personal information contained in DMV records private. This protects people from having personal information in their DMV records given out to individuals not authorized to receive the information. People can order records containing their own personal information but not another person’s personal record information.

• Records may be ordered in person or by mail from DMV. The record will be mailed to you.
